ABSTRACT

Objetivo: Analizar los determinantes de la demanda de servicios de salud del adulto mayor entre los segmentos más pobres de Colombia. Materiales y métodos: Estudio de datos panel a partir de datos de riesgo individualizado de una aseguradora del régimen subsidiado en Colombia. Se elaboraron modelos de regresión lineal para determinar la demanda de salud bajo dos escenarios. Resultados: El hacinamiento, la exposición al humo y roedores mantienen una relación positiva con la demanda de servicios de salud. Por el contrario, la educación, ocupación, realizar actividad física y el no consumo de alcohol son factores protectores. Conclusión: La transición demográfica y epidemiológica que experimenta Colombia trae consigo retos para el sistema de salud. La identificación de determinantes de la demanda contribuye al establecimiento de estrategias para el mejoramiento del sistema de salud vigente y futuro.


Objective: To analyze the determinants of the demand for health services of the elderly among the poorest segments in Colombia. Materials and methods: Panel data study based on individualized risk data from an insurer of the subsidized regime in Colombia. Linear regression models were developed to determine health demand under two scenarios. Results: Overcrowding, exposure to smoke and rodents maintain a positive relationship with the demand for health services. On the contrary, education, occupation, physical activity, and non-alcohol consumption are protective factors. Conclusion: The demographic and epidemiological transition that Colombia is experiencing brings challenges to the health system. The identification of determinants of demand contributes to the implementation of strategies to improve the current and future health system.

ABSTRACT

O presente estudo parte das experiências das mulheres nos bairros populares de uma cidade do interior do Maranhão, Caxias. Objetivou-se conhecer as histórias de vida das mulheres desses bairros, os seus modos de vida e resistência em suas vivencias diárias. Trata-se de uma pesquisa qualitativa que tem como base a produção de sentido no cotidiano. Participaram cinco mulheres de quatro bairros populares do município. Foram utilizadas a observação no cotidiano, conversas no cotidiano e entrevista semiestruturada. Para a análise foi utilizado o mapa dialógico de produção de sentidos. Percebeu-se que as mulheres atuam enquanto sujeitas ativas em seus bairros buscando por melhores condições de vida, em prol de educação e infraestrutura, a partir da formação de uma rede de sociabilidade e solidariedade, à frente de clube de mães, associação de moradores e grupo de dança enquanto formas de resistência. Logo, entende-se que esse estudo pode contribuir com a literatura e discussões sobre mulheres de contextos periféricos de regiões não metropolitanas.


The present study is based on the experiences of women from the low-income neighborhoods of Caxias, a country town in the Brazilian State of Maranhão. The purpose was to investigate the life stories of women who live in that neighborhoods, their ways of life, and resistance in their daily experiences. This is a qualitative research that is based on the production of meaning in everyday life. We have researched five women from four low-income neighborhoods in the city through daily observation, daily conversations, and semi-structured interviews. We have used the dialogic map of production of meaning for this analysis. It was noticed that women act as active subjects in their neighborhoods, looking for better living conditions towards education and infrastructure. They do it building a network of sociability and solidarity which includes a front of mothers' club, community association, and dance group as forms of resistance. Therefore, we realize that this study can contribute to the literature and to the discussion about women from peripheral contexts of non-metropolitan regions.


El presente estudio se basa en las experiencias de mujeres en los barrios populares de una ciudad del interior de Maranhão, Caxias. El objetivo fue conocer las historias de vida de mujeres que viven en barrios populares de Caxias do Maranhão, sus formas de vida y resistencia en sus experiencias cotidianas. Se trata de una investigación cualitativa que se fundamenta en la producción de sentido en la vida cotidiana. Participaron cinco mujeres de cuatro barrios populares de la ciudad. Se utilizó la observación diaria, conversaciones diarias y entrevistas semiestructuradas. Para el análisis se utilizó el mapa dialógico de producción de sentido. Se percibió que las mujeres actúan como sujetos activos en sus barrios buscando mejores condiciones de vida, a favor de la educación e infraestructura, a partir de la formación de una red de sociabilidad y solidaridad, al frente del club de madres, asociación de vecinos y grupo de baile como formas de resistencia. Por lo tanto, se entiende que este estudio puede contribuir a la literatura y discusiones sobre mujeres de contextos periféricos de regiones no metropolitanas.

Purpose: Baseline knowledge and attitudes regarding diabetic retinopathy (DR) have not been previously reported for type 2 diabetes mellitus adults (T2DM) in Dharavi, one of the largest slums in Mumbai. Furthermore, a pre? and post?intervention survey study regarding this topic has not been conducted to date in Dharavi. This pre? and post?intervention survey study analyzes the impacts of DR counseling sessions administered individually on the knowledge and attitudes of T2DM participants recruited from Dharavi. Methods: Potential subjects (>30 years old) from Dharavi were enrolled by community workers and screened for T2DM. Those with confirmed T2DM were each registered for an individual DR counseling session. A survey was distributed to participants before the counseling session to determine their baseline knowledge and attitudes. The same survey was distributed after the counseling session, and changes in pre? and post?survey responses were analyzed. Results: Exactly 1718 T2DM study participants were given pre? and post?surveys. Before the counseling, T2DM participants showed low baseline knowledge and attitudes regarding DR, with only 30.8% (530/1718) participants being aware that diabetes can affect the eye. Participants showed significant improvements after the counseling session, with more participants showing willingness to attend DR screenings (pre: 38.3.%, post: 56.8%; P < 0.0001) and more becoming aware that diabetes can affect the eye (90.4%, 1553/1718; P < 0.0001). Conclusion: Targeted educational interventions for T2DM individuals from the slums of western India, such as Dharavi, can lead to significant improvements in knowledge and attitudes, representing a promising avenue in tackling the burden of avoidable blindness caused by DR in India.

@#Poverty, as proven by several studies, is a driving force behind poor health and hygiene practices. This review attempts to outline common communicable and non-communicable diseases that disproportionately affect Malaysia’s 2.91 million low-income households. The current study also looks into the government’s housing and healthcare programmes for this demographic to improve their health and well-being. The initial examination yielded incredibly little research on this marginalised community, with event reporting typically generalised to the Malaysian community as a whole rather than analysing disease incidences based on household income, which would better reflect povertydriven diseases. As a result, there is an acute need for more accurate information on the epidemiology of diseases among the poor in order to address this public health issue and provide conclusions that can drive policy designs.

@#Introduction: The Lost Food Project (TLFP) is a non-profit organization who continuously working on their strategic plan in combating the food insecurity problem by distributing food surplus to low income households within Klang Valley. This cross-sectional study aims to determine the socioeconomic status, food security status and its coping strategies among TLFP recipients in Klang Valley during COVID-19 pandemic. Methods: This study was conducted from April to June 2021. There were 88 respondents recruited through convenience sampling. A set of questionnaires regarding food assistance, nutrition knowledge, food security and its coping strategies was prepared, and the respondents were interviewed either by phone, face-to-face or self-administered questionnaire. Results: The prevalence of food insecurity among households in this study was 59.1%. Most of them have moderate (37.5%) to good (27.5%) nutrition knowledge. The coping strategy that most of the respondents applied was ‘using less expensive food’ (58.3%) with at least once per week within the past one month. Food insecure households tend to adopt more coping strategies compared to those food secure households (p<0.001). Moreover, households who rent their house (p<0.01), low household income (p<0.05) and high monthly food and drinks expenditure (p<0.01) were associated with the food insecurity among TLFP recipients. Conclusion: The study concluded that lower household income, higher food and drinks expenditure and those who rent the house have negative impacts on household’s food security status. Thus, the situation forced them to apply more food-related coping strategies in their daily life. Further research should be conducted on the effectiveness of TLFP in helping them to improve Malaysian’s food security status.

@#Introduction: COVID-19 pandemic has impacted the livelihood of Malaysians and gardening activities have contributed positively to the diet quality. This study aimed to determine the factors associated with diet quality among adults in AU2 Keramat, Kuala Lumpur. Methods: The cross-sectional study involved adults aged 18 to 59 via convenience sampling. The socio-demographic, physical activity level and involvement status in gardening were obtained through a questionnaire, while food intake was from a single-day 24-hour diet recall. The diet quality was determined through Malaysian Healthy Eating Index (MHEI) and dietary misreporting was calculated using the Goldberg cut-off method. Results: A total of 117 respondents (65.8% females, 35.2% males) with a mean age of 40.98 were involved in this study. Findings showed that most respondents did not engage in gardening activities (72.6%) and the prevalence of poor diet quality in this study was 89.7%, with a mean score of 44.51. The older respondents (r= 0.20, p= 0.034) and community gardeners (t= -2.63, p= 0.011) had a significantly higher diet quality but not home gardeners (t= 0.12, p= 0.902). The respondents involved in gardening activity also had a significantly higher fruit serving intake, Mann-Whitney U= 1045.00, p= 0.036) and lower total fat intake (t= 2.27, p= 0.025). Conclusion: The diet quality of the respondents remains poor but community gardeners had significantly higher diet quality and fruit intake while lower total fat intake. Interventions need to be developed to address the persistent poor diet quality of adults in the community.

Abstract Objective: The aim of this study was to verify the adequacy of affordances in the home environment of children at risk of developmental delay and to identify factors associated with their frequency. Methods: The cross-sectional study included 97 families who responded to the Affordances in the Home Environment for Motor Development — Infant Scale (AHEMD-IS) for 3-18 months (n=63), or AHEMD - Self-Report (AHEMD-SR) for 18-42 months (n=34). The Mann-Whitney U test was used to identify the differences between the frequencies of affordances between the groups. Multiple linear regression was used to verify the association between the child's sex, mother's marital status, education, socioeconomic level, child and mother's ages, house residents' number, per capita income, and AHEMD scores (α=0.05). Results: The home affordances' frequency in the AHEMD-IS ranged from less than adequate to excellent, while in the AHEMD-SR, the highest predominance was medium. The offer of stimuli in the AHEMD-IS was significantly higher. Higher socioeconomic level and house residents' number were associated with greater affordances. Conclusions: The higher the socioeconomic level and house residents' number, the greater the affordances in the homes of children at risk of delay. It is necessary to provide families with some alternatives to make their home environments richer in affordances that favor child development.


RESUMO Objetivo Verificar a adequação das oportunidades de ação no ambiente doméstico de crianças em risco de atraso no desenvolvimento e identificar fatores associados à sua frequência. Métodos Este estudo transversal incluiu 97 famílias que responderam ao Affordances in the Home Environment for Motor Development - Infant Scale (AHEMD-IS) para 3-18 meses (n=63) ou o Self Report (AHEMD-SR) para 18-42 meses (n=34). O teste de Mann-Whitney foi utilizado para identificar as diferenças entre as frequências de oportunidades de ação entre os grupos. A regressão linear múltipla foi utilizada para verificar a associação entre o sexo da criança, o estado civil, a escolaridade e nível socioeconômico da mãe, as idades da criança e da mãe, o número de residentes da casa, a renda per capita e os escores do AHEMD (α=0,05). Resultados A frequência das oportunidades de ação dos domicílios no AHEMD-IS variou de menos do que adequado a excelente; enquanto, no AHEMD-SR, a maior predominância foi de média frequência. A oferta de estímulos no AHEMD-IS foi significativamente maior. O maior nível socioeconômico e o número de moradores da casa foram associados a maiores oportunidades de ação. Conclusões Quanto maior o nível socioeconômico e o número de moradores, maiores são as oportunidades de ação nos domicílios das crianças em risco de atraso. É necessário oferecer às famílias algumas alternativas para tornar os seus ambientes domésticos mais ricos em oportunidades de ação que favoreçam o desenvolvimento infantil.

BACKGROUND: Training and work experience are critical inputs for delivering quality health services. However, no nationwide assessment has been conducted on the status of training and the competency of Health Extension Workers (HEWs). Therefore, this study aimed to assess HEWs' pre service training status and perceived competency in Ethiopia. METHODS: The study was conducted in all regions and all HEWs training institutions in Ethiopia. We used cross sectional study design with a mixed method approach that included 585 HEWs, 1,245 HEW trainees, 192 instructors, and 43 key informants. Descriptive statistics and thematic analysis were used to analyse quantitative and quantitative data respectively. RESULT: Twenty-six percent of the HEWs said that they were competent to deliver all the HEP activities, and 73% of the HEWs said that they could confidently deliver 75% of the HEP activities. Receiving in-service training and having level III/IV qualifications are positively associated with the competency of HEWs. Similarly, HEP trainees perceived themselves as highly competent in executing their professional work, except in using computer and mobile health technology. Both instructors and trainees rated the quality of the curriculum and course materials positively. However, basic services and facilities in most training institutions were perceived to be inadequate. Additionally, individual learning, problem-solving, case-analysis, and assessment methods such as project work and portfolios were rarely practiced. CONCLUSIONS: Although the perceived competence of HEW trainees is high, the HEWs' training is not provided as per the curriculum because of limited resources. All the necessary resources should be made available to produce competent HEWs

ABSTRACT

BACKGROUND: Depression and burnout are common among healthcare workers (HCWs) and negatively affect their well being and the quality of the service they provide. However, the burden of depression and burnout among health extension workers (HEWs) in Ethiopia and their relationship has not been documented well. The objective of this study was to estimate the prevalence of depression and burnout among HEWs in Ethiopia and to investigate the relationship between these conditions. MATERIALS AND METHODS: We used a cross-sectional study design and collected data from 584 rural and 581 urban HEWs in Ethiopia, as part of the 2019 national health extension program assessment. The Patient Health Questionnaire (PHQ-9) and Burnout Self-Test were used to screen HEWs for depression and burnout, respectively. We used descriptive statistics to estimate the magnitude of depression and burnout, and logistic regression to examine their relationship. RESULT: Based on PHQ-9 cutoff scores of 10, the prevalence of major depression was 16.5% among rural and 8.9% among urban HEWs, whereas burnout risk was 39.8% among rural and 12.6% among urban HEWs. The odds of having depression among HEWs with burnout risk was relatively higher compared to those without burnout risk [For rural HEWs, the adjusted odds ratio (AOR) is 11.88 at a 95% confidence interval (CI; 5.27, 26.80), and for urban HEWs, the AOR is 11.49 at a 95% CI (5.35, 24.63)]. CONCLUSION: The prevalence of depression and burnout is high among HEWs in Ethiopia, with a significant rural­urban difference, and burnout is a significant predictor of depression. Mental health interventions that enable prevention, early detection, and management are needed especially for rural HEWs who are in charge of preventive health service delivery for the disadvantaged rural communities

ABSTRACT

O objetivo do presente estudo foi descrever a percepção de mães em situação de pobreza residentes na região metropolitana de Belém-PA sobre suas práticas de monitoria positiva e monitoria negativa. Participaram deste estudo 13 cuidadoras, que além de responderem à instrumentos para coleta de dados sociodemográficos (ISD), de pobreza familiar (IPF) e de avaliação de práticas parentais (IEP), participaram de grupos focais. As participantes foram divididas de acordo com o nível de pobreza familiar em dois grupos: mais pobres e menos pobres. As etapas de coleta dos dados foram realizadas em Centros de Referência em Assistência Social (CRAS) do município de Belém-PA, localizados em áreas periféricas da cidade e em salas de aula do Núcleo de Teoria e Pesquisa do Comportamento (NTPC). A análise qualitativa dos dados ratificou a relação entre maiores níveis de privação com a maior probabilidade de utilização de monitoria negativa. Os resultados apontam para os desafios e demandas do contexto de vulnerabilidade que enfraquecem a habilidade de exercer uma parentalidade efetiva.


The objective of this study was to describe the perception of poor mothers living in the metropolitan region of Belém-PA, about their practices of positive and negative monitoring. Participated in this study 13 guardians who, in addition to responding to the following quantitative instruments: ISD (sociodemographic inventory), IPF (family poverty index) and IEP (parenting styles inventory), participated in a focus group. Participants were divided according to the level of family poverty into two groups, namely, more and less poor. The data collection stages were respectively carried out in Social Assistance Reference Centers (Centros de Referência em Assistência Social - CRAS), located in peripheral areas of the city of Belém-PA and in classrooms of the Behavior Theory and Research Nucleus (NTPC). The qualitative data analysis confirmed the relationship between higher levels of deprivation and higher probabilities of negative monitoring. The results point to the challenges and demands of the vulnerability context that weaken the ability to exercise positive parenting.

ABSTRACT

Introduction: The impact of COVID-19 on people with Severe Mental Health Conditions (SMHCs) has been neglected. We aimed to describe the effect and explore the consequences of COVID-19 on people with SMHCs and mental health services in rural districts of Ethiopia. Methods: We conducted a mixed-method study nested within well-characterized population cohorts in Butajira and Sodo districts. We sampled 336 people (168 people with SMHCs, 168 comparisons) in a cross-sectional survey. We conducted qualitative key informant interviews with psychiatric nurses (n=3), primary health care workers (n=3), service users (n=4), family members (n=6) and community members (n=2). We assessed wellbeing (WHO wellbeing index), social support (Oslo social support scale; OSS) and food security quantitatively and used thematic analysis to explore impacts. Results: People with SMHCs reported significantly lower wellbeing (WHO wellbeing score 52 vs. 72; p<0.001), less social support (OSS score 8.68 vs. 9.29; p<0.001), worse living standards (47.0% vs. 29.0%; p<0.001) and increased food insecurity (26.0% vs. 12.5%; p<0.001). Household economic status worsened for over one-third of participants. Participants reported increased relapse, exacerbated stigma due to perceived susceptibility of people with SMHCs to COVID-19, and increased restraint. In mental healthcare settings, there was decreased patient flow but an increase in new cases. Innovations included flexible dispensing of medicines, longer appointment intervals and establishing new treatment centers. Conclusions: COVID-19 had negative consequences on people with SMHCs and mental health services, which must be anticipated and prevented in any future humanitarian crisis. Adaptive responses used during COVID may increase health system resilience

ABSTRACT

Objectives: To explore and analyse factors that facilitate and inhibit the initiation and functioning of a national and transnational Community of Practice (CoP) for health policy and systems (HPS) and Reproductive, Maternal, Newborn, Child and Adolescent Health (RMNCAH) in West Africa and to identify lessons for CoP interventions in similar multilingual low and middle-income contexts. Design: A case study, with the case defined as processes, enablers and barriers to the initiation and functioning of a national and transnational CoP for HSP and RMNCAH in West Africa and drawing on a review and analysis of secondary data from the program, workshop, country team and project reports, and training sessions. Setting: The Economic Community of West African States (ECOWAS). Participants: Professionals from two Anglophone (Ghana and Sierra Leone) and four Francophone (Burkina Faso, Cote d'Ivoire, Niger e Senegal) ECOWAS countries. Interventions: Training and mentoring of multi-disciplinary country teams supported by small research grants to undertake formative evaluation and advocacy of priority HPS and RMNCAH issues; support for CoP development within and across country teams. Results: The desire to learn from peers and mentors was a major enabler of the process. Human and financial resource availability, competing demands for time, communication in the context of a Francophone-Anglophone official language divide and the arrival of COVID-19 were all constraints. Conclusions: This study highlights the processes, achievements, and challenges of establishing country-level and transnational CoPs in West Africa. CoPs require sustained human and financial resource investments, communication and medium-to-long-term implementation support for sustainability and impact.

RESUMEN Introducción: Chile, en los últimos 40 años, ha presentado cambios sociales que han influido en el nivel de vulnerabilidad de niños, niñas y adolescentes. Este nivel es determinado a través del índice de vulnerabilidad (IVE). El objetivo de este estudio es analizar diferencias en el estado nutricional y potencia aeróbica máxima de acuerdo al IVE, en estudiantes de 14 años de establecimientos educacionales de comunas de la Región Metropolitana de Chile. Método: fueron evaluados 186 escolares de 14,1 ± 0,7 años (88 hombres y 98 mujeres), pertenecientes a 8 establecimientos educacionales, caracterizados por IVE (IVE-0 e IVE-89). Las variables antropométricas medidas fueron peso, talla, perímetro de cintura, IMC, Z-score-IMC y relación cintura estatura. La potencia aeróbica máxima fue medida por el consumo máximo de oxígeno (VO2máx) relativo al peso corporal (mlO2·kg−1·min−1) obtenido de la prueba de 20 m ida y vuelta. Resultados: Fueron observadas diferencias por IVE en Zscore-IMC [IVE-0: 0,761 (0,352 −1,19) IVE-89: 1,29 (0,341-2,45) (p<0,0001)] y potencia aeróbica máxima [IVE-0: 46 (42,1-50,2) IVE-89: 43,7 (33,2-46,3) (p<0,0001)] en los escolares. Además, se demostró que adolescentes obesos presentan 5,34 mayor probabilidad de presentar menores valores VO2máx, de igual forma, este modelo demuestra que, pertenecer al grupo con mayor vulnerabilidad (IVE 89), expone a los adolescentes a 5,09 veces la probabilidad de presentar valores bajos de VO2máx. Conclusiones: Adolescentes, pertenecientes a establecimientos educacionales con mayor índice de vulnerabilidad presentan mayor prevalencia de sobrepeso u obesidad y menor capacidad cardiorrespiratoria.


ABSTRACT Introduction: In the last 40 years, Chile has presented social changes, which influence the level of vulnerability of children and adolescents, as measured by a vulnerability index (EVI). Objective: To analyze the differences by EVI in nutritional status and cardiorespiratory fitness, as well as, the relationship between these variables in 14-year-old students. Method: Adolescents (n= 186, 88 males and 98 females) were 14.08±0.72 years of age from 8 educational establishments were characterized according to the EVI (EVI-0 and EVI-89) of their educational establishment. Weight and height were measured to determine BMI and BMI z-score, waist circumference, waist height ratio and cardiorespiratory fitness was measured by the maximum oxygen consumption (VO2max) relative to body weight (mlO2·kg−1·min−1) using the 20 m shuttle run test. Results: Differences were observed by EVI in BMI z-score [EVI-0: 0.761 (0.352 −1.19) EVI-89: 1.29 (0.341 −2.45) (p<0.0001)] and cardiorespiratory capacity of adolescents [EVI-0: 46 (42.1-50.2) EVI-89: 43.7 (33.2 - 46.3) (p<0.0001)]. In addition, it was shown that obese adolescents had 5.34 greater probability of presenting lower VO2max values, in the same way, this model shows that, belonging to the group with greater vulnerability (IVE 89), exposed adolescents to 5.09 times the probability of presenting low VO2max values. Conclusions: Adolescents belonging to educational establishments with greater vulnerability due to EVI present a higher prevalence of overweight or obesity and lower cardiorespiratory fitness than those more economically favored.

ABSTRACT

The aim of this study was to investigate the possible relations between sibling care level and the areas of interest in adolescents' life purposes. Participants were 113 adolescents. The instruments were a record of socio-demographic data, the household tasks and Sibling Care Questionnaire, and a written testimony on life purpose. We identified 347 life projects, which were grouped into the following categories: career, family, possessions, happiness, and generosity. The results of the Pearson's correlations indicated positive associations between sibling care and generosity and career. Thus, there is a tendency that adolescents who take care of their younger siblings have more life projects related to assisting the family of origin and to professional development. These results suggest that taking care of younger siblings may influence the areas of interest of adolescents' life purposes.


O objetivo deste estudo foi investigar se há relação entre nível de cuidado dos irmãos maiores para com os menores e as áreas de interesse dos projetos de vida de adolescentes. Cento e treze adolescentes participaram do estudo. Para a avaliação foram utilizados uma ficha de dados biossociodemográficos, o Questionário de Tarefas Domésticas e de Cuidado entre Irmãos e um depoimento escrito sobre projetos de vida. Computaram-se 347 projetos de vida, os quais foram agrupados nas categorias: carreira, família, bens materiais, felicidade e generosidade. Os resultados das correlações de Pearson indicaram associações positivas entre cuidado e as áreas generosidade e carreira. Dessa forma constatou-se uma tendência de que os adolescentes que cuidam mais de seus irmãos mais novos elaborem mais projetos de vida relacionados a ajudar a família e ao desenvolvimento profissional. Esses resultados sugerem que cuidar de irmãos menores pode influenciar nas áreas de interesse dos projetos de vida dos adolescentes.

RESUMO Objetivo Descrever e relacionar o desempenho do vocabulário receptivo em escolares com dificuldades de aprendizagem de diferentes gêneros, escolaridades e níveis socioeconômicos. Métodos Estudo documental, entre 2017 e 2019, considerando os dados demográficos gênero, escolaridade e nível socioeconômico da família. Os prontuários incluídos foram aqueles com avaliação fonoaudiológica completa. Para verificar o nível socioeconômico da família, foi utilizado o questionário da Associação Brasileira de Empresas de Pesquisa - ABEP e, para a habilidade do vocabulário receptivo, o Teste de Vocabulário por Figuras USP - TVfusp 139o. Nesse teste, o desempenho em vocabulário é classificado como "muito rebaixado", "rebaixado", "médio", "elevado" e "muito elevado". Para o critério de classificação apresentar-se na média, o escore de acertos por escolaridade é de 105,8 para o 2º ano, 112,8 para o 3º ano e 117,4 para o 4º ano. Resultados Dos 46 participantes da amostra, 31 (67,4%) eram do gênero masculino e 15 (32,6%) do feminino, com predomínio de estudantes no 4º ano (34,80%) e classe socioeconômica entre os níveis B2 e D-E. Na avaliação do vocabulário, 28 (60%) participantes obtiveram classificação média. Houve correlação entre a escolaridade e o desempenho em vocabulário receptivo (p=0,008) e entre a idade e o desempenho em vocabulário receptivo (p=0,007). Conclusão O desempenho em vocabulário receptivo avançou com a idade e a escolaridade, porém, não houve influência do nível socioeconômico.


ABSTRACT Purpose To describe and relate the performance of receptive vocabulary in students with learning difficulties of different genders, educational levels, and socioeconomic levels. Methods Documentary study, approved by the Research Ethics Committee under number 1,012,635, with 46 records between 2017 and 2019, considering demographic data: gender, education, and socioeconomic level of the family. The medical records included were those with complete speech-language assessments. To check the family's socioeconomic level, the questionnaire from the Brazilian Association of Research Companies - ABEP was used, and for the receptive vocabulary skill, the Picture Vocabulary Test - TVfusp139o. In this test, the classification very low, low, medium, high and very high is used. For the classification criterion to be presented as the average, the score of correct answers by education is 2nd grade (105.8), 3rd grade (112.8) and 4th grade (117.4). Results Of the 46 participants in the sample, 31 (67.4%) were male and 15 (32.6%) female, with a predominance of students in the 4th year (34.80%) and socioeconomic class between levels B2 and D-E. In the vocabulary assessment, 28 (60%) of the 46 participants obtained an "average" classification. There was a correlation between education and receptive vocabulary performance (p = 0.008) and; between age and receptive vocabulary performance (p = 0.007). Conclusion Performance in receptive vocabulary advanced with age and education, but there was no influence on a socioeconomic level.

ABSTRACT

Introducción. El abordaje del desarrollo en la primera infancia (DPI) y las dificultades del desarrollo (DD) en los países de ingresos bajos o medios (PIBM) requiere de programas aplicables y sostenibles. Se evaluó el programa de capacitación para las unidades de pediatría del desarrollo (UPD) en los centros de referencia tras siete años. Población y métodos. Diseño transversal con métodos cualicuantitativos para evaluar los servicios, la capacitación, la investigación y la promoción de las UPD. Resultados. Se mantuvieron los servicios clínicos, la capacitación, investigación y promoción. En total, 23710 niños fueron derivados. La motivación y resistencia del personal, el respaldo de los administradores y la satisfacción del paciente facilitaron el programa, aunque la invisibilidad dentro del sistema de salud fue un obstáculo. Conclusiones. En Turquía, y posiblemente otros PIBM, el programa de capacitación para las UPD es aplicable y sostenible si se abordan los obstáculos del sistema de salud.


Introduction. Addressing early childhood development (ECD) and developmental difficulties (DDs) in low and middle-income countries (LMICs) requires applicable and sustainable programs. Seven years after its implementation in 2010, we evaluated the Developmental Pediatrics Unit (DPU) Training Program in pediatric referral centers in Turkey. Population and methods. We applied crosssectional design and quantitative/qualitative methods to assess services, training, research and advocacy of the DPUs. Results. Five of six established DPUs sustained clinical services, training, research, and advocacy. A total of 23,710 children (9085 new cases in 2017) had been referred mainly for perinatal risks and chronic illness, all centers contributed with similar proportions of children. Staff motivation and endurance, hospital administrator support, and client satisfaction facilitated the program; whereas invisibility within the performance based healthcare system was a barrier. Conclusions. In Turkey and possibly other LMICs, the DPU Training Program is applicable and sustainable if health system barriers are addressed

ABSTRACT

OBJECTIVE: To investigate the association between exposure to domestic violence and increased risk of internalizing and externalizing problems and substance use among adolescents living with relatives with substance use disorder (SUD) at a low-income community of São Paulo, Brazil. METHODS: A crosssectional study was conducted with 102 adolescents aged 12-17 years (M = 14.2, SD = 1.7) who were living with relatives suffering from SUD. Outcomes were measured using the Youth Self-Report (YSR), psychosocial stress factors questionnaire, Drug Use Screening Inventory (DUSI) and Phrase Inventory of Intrafamily Child Abuse (PIICA). RESULTS: The sample presented high prevalence of emotional/behavioral problems with YSR's scores in the clinical range for Internalizing Problems (24.5%), Externalizing Problems (21.6%), and Total Problems (26.5%). The presence of mental health problems predicted substance use (PR = 2.22; 95% CI = 1.2-4.13), and substance use predicted increased risk of mental health problems. Alcohol use predicted more than double the risk of emotional/behavioral problems (PR = 2.01; 95% CI = 1.08-3.76), while illicit drug use was associated with an almost threefold increase in the prevalence of Internalizing (PR = 2.87; 95% CI = 1.19-6.89) and Externalizing Problems (PR = 3.3; 95% CI = 1.35-8.04). CONCLUSION: Adolescents of relatives with SUD are at risk of developing emotional and behavioral problems. These findings reinforce the need to develop public mental health policies, which include protective interventions to adolescents living in families affected by substance use disorders.


OBJETIVO: Investigar a associação entre exposição à violência doméstica e aumento do risco de problemas internalizantes e externalizantes e uso de substâncias entre adolescentes que vivem em famílias com transtorno por uso de substâncias (TUS) em uma comunidade de baixa renda de São Paulo, Brasil. MÉTODOS: Estudo transversal com 102 adolescentes de 12 a 17 anos (M = 14,2, DP = 1,7) que vivem com familiares com TUS. Os desfechos foram avaliados por meio do Inventário de Autoavaliação para Adolescentes (YSR), questionários de fatores de estresse psicossociais, Drug Use Screening Inventory (DUSI) e Inventário de Frases de Violência Doméstica (IFVD). RESULTADOS: A amostra apresentou altas taxas de problemas emocionais/comportamentais no YSR, sendo 24,5% com escores na faixa clínica para Problemas Internalizantes, 21,6% para Problemas Externalizantes e 26,5% para Problemas Totais. A presença de problemas de saúde mental foi preditora do uso de substâncias (RP = 2,22; IC 95% = 1,2-4,13) e o uso de substâncias foi preditor do aumento da prevalência de problemas emocionais/comportamentais. O uso de álcool prediz mais do que o dobro do risco de problemas emocionais/comportamentais (RP = 2,01; IC 95% = 1,08-3,76), enquanto o uso de substâncias ilícitas esteve associado com um aumento de quase três vezes na prevalência de Problemas Internalizantes (RP = 2,87; IC 95% = 1,19-6,89) e Externalizantes (RP = 3,3; IC 95% = 1,35-8,04). CONCLUSÃO: Adolescentes que convivem diretamente com familiares com TUS estão em risco para o desenvolvimento de problemas emocionais/comportamentais. Os achados reforçam a necessidade de políticas públicas que incluam programas de proteção para adolescentes que vivem em famílias com transtornos causados pelo uso de substâncias.

ABSTRACT

RESUMO O objetivo deste estudo foi o descrever as características e demandas das famílias em acompanhamento no Centro de Referência de Assistência Social (Cras) de Santa Marta, município de Belford Roxo (RJ), no período de 2006 a 2017. Trata-se de estudo transversal descritivo com dados extraídos das fichas de acompanhamento. Um total de 87 famílias foi acompanhada no período. A maioria dos responsáveis familiares era do sexo feminino (92,0%), menor de 40 anos (59,8%), solteira (92,0%) e com ensino fundamental incompleto (89,7%). As principais demandas se referiram ao descumprimento de condicionalidade do Programa Bolsa Família (28,7%) e evasão escolar (26,4%). O serviço de saúde (33,3%) apresentou o maior encaminhamento intersetorial. A mediana do tempo de acompanhamento foi de 53 meses. Não houve diferença no tempo de acompanhamento em função do grupo etário, sexo, vínculo de trabalho informal ou precário, para recebimento de benefício, forma de acesso, tipo de demanda e registro de plano de acompanhamento (p≥0,272). A demanda espontânea foi a razão de a maioria das famílias estar em acompanhamento (86,2%), mas não havia registro sobre sua situação de vulnerabilidade. A identificação das famílias que necessitam de acompanhamento é importante para garantir acesso aos direitos socioassistenciais, além de contribuir para superar as desigualdades sociais existentes.


ABSTRACT The objective of this study was to describe the characteristics and demands of the families under follow-up at Social Assistance Referral Center (Cras) of Santa Marta, municipality of Belford Roxo, State of Rio de Janeiro, from 2006 to 2017. This is a descriptive cross-sectional study whose data were retrieved from the families' accompanying records. A total of 87 families were followed up within the study period. Most of the family heads were female (92.0%), under 40 (59.8%), single (92.0%) and incomplete elementary school (89.7%). Main demands referred to noncompliance with the Bolsa Família Program (28.7%) and school dropout (26.4%) conditionalities. Health service (33.3%) showed the greatest intersectoral referral. The median following-up time was 53 months. There was no difference in the following-up time due to the age group, gender, informal or precarious employment relationship, benefit payment, access type, demand type, and follow-up plan registration (p≥0,272). Spontaneous demand was the reason for most of the families to be under follow-up (86.2%), although no record existed on their vulnerability situation. The identification of the families in the need to be followed is important to guarantee access to social and care rights, besides contributing to overcome the existing social inequalities.

ABSTRACT

Background: Congenital Talipes Equinovarus (CTEV) is a common musculoskeletal congenital disorder. Historically, surgical treatment of various types was popular; however, currently the gold standard of treatment is non-operative care using the Ponseti method. This work evaluates the outcome of this former method of managing CTEV before author adopted the Ponseti Method and compares it with the reported outcomes of the Ponseti method in published literature.Methods: A retrospective review of this data from January 1990 to December 2003 was done, the patients demographics, treatment given, duration and outcome of care were analysed using descriptive statistics. This finding was then compared with outcomes of Ponseti method in literature.Results: Here, 145 patients had initial non operative care; eighteen of those patients (12.4%), eventually require either a posteromedial release, a combined posteromedial and posterolateral releases or a triple arthrodesis.Conclusions: The study shows that the success rate of this method of manipulation preceding the adoption of the Ponseti method is 87.6%. This outcome is inferior when compared to the outcome of Ponseti method in published literature. Also, the extensive nature of eventual surgical intervention required to achieve correction is in contrast to the minimal surgeries needed achieve correction of residual deformities following the use of the Ponseti method.

Actualmente, Chile presenta niveles por sobre el promedio regional y mundial de sobrepeso y obesidad. Esta condición se distribuye desigualmente entre la población, concentrándose en mujeres (quienes lideran el ranking latinoamericano de obesidad), acrecentándose entre aquellas de bajos ingresos. Este dato corrobora que el problema nutricional excede con creces el ámbito biológico y que si se busca tener resultados positivos en la disminución del sobrepeso y obesidad, urge observar sus condicionantes socioculturales. Desde 2005 las políticas públicas han implementado medidas basadas principalmente en ofrecer recomendaciones e información nutricional a la población. A través de un estudio cualitativo- etnográfico nos interrogamos respecto al modo en que las mujeres perciben el discurso salubrista y las recomendaciones nutricionales. Confirmamos que los mensajes sobre la adecuada alimentación son recibidos y comprendidos. Sin embargo, se interiorizan como una imposición altamente restrictiva, que no se condice con sus estilos de vida, que exige implementar cambios para los que no se sienten capaces ni suficientemente respaldadas por su entorno cercano. Se concluye que es necesario reorientar los esfuerzos, desplazando el foco desde la difusión de información y la responsabilización individual, hacia la construcción de estrategias que pongan como eje central a las personas dentro de sus contextos particulares.


Currently, Chile has levels above the regional and the global average for overweight and obesity. This condition is distributed unequally among the population, concentrating among women (who lead the Latin American obesity ranking) and increasing among those with low incomes. This data verifies that the nutritional problem exceeds the biological scope, thus it is crucial to observe its sociocultural conditions. Since 2005, public policies have been implemented based mainly on offering recommendations and nutritional information to the population. Through a qualitative-ethnographic study, we inquired about the woman's perception of the health discourse and the nutritional recommendations. We confirm that messages about proper eating are received and understood. However, they are internalized as highly restrictive, incompatible with their lifestyles, and require changes that participants felt they were incapable of or sufficiently supported by their immediate environment. We concluded that it is necessary to redirect efforts, shifting the focus from the dissemination of information and individual responsibility, to the construction of strategies that place people at the center of their particular contexts.
