ABSTRACT

Objective:To investigate the cause of misdiagnosis of neck, shoulder, waist, and leg pain caused by bone metastases and to improve the outpatient diagnosis rate.Methods:Five misdiagnosed cases of bone metastases who were admitted to the Traditional Chinese Medicine Department of Orthopedics and Traumatology, School of Traditional Chinese Medicine, Southern Medical University from January 2019 to July 2020 were included in this study. Their clinical manifestations, diagnosis results (outside hospital diagnosis and outpatient diagnosis), and imaging manifestations were retrospectively analyzed.Results:Five cases of bone metastases were misdiagnosed to have cervical spondylosis, lumbar disc herniation, femoral head necrosis, femoral shaft fracture, and ankle sprain. Accelerated red blood cell sedimentation rate and increased C-reactive protein level were found in all five cases. Bone metastases were confirmed by X-ray, CT, and MRI examination results. All of them had warning signs for bone metastasis: age > 50 years, history of a tumor, unexplained weight loss, general malaise, neck, shoulder, waist, and leg pain without an obvious cause, sudden worsening of pain, night pain, resting pain, pain on direct palpation, no obvious improvements in these symptoms after symptomatic treatment for 1 month, disease development not conforming to the general law of cervical spondylosis, lumbar disc herniation, femoral head necrosis, femoral shaft fracture, and ankle sprain, increases in red blood cell sedimentation rate, C-reactive protein, and alkaline phosphatase levels, which are not consistent with the symptoms of cervical spondylosis, lumbar disc herniation, femoral head necrosis, femoral shaft fracture, and ankle sprain, and slight external force leading to fractures.Conclusion:Further tests and imaging examinations should be performed according to warning signs. Clinical schemes of malignant bone tumors suitable for general practitioners can easily, economically, and effectively identify malignant bone tumors.

ABSTRACT

Objective:To explore the efficacy of Tuina combined hot compress in the treatment of lumbar disc herniation (LDH).Methods:A total of 90 patients with LDH low back and leg pain in Shijiazhuang Traditional Chinese Medicine Hospital from January 2019 to June 2020 were divided into two groups, 45 in each group. The control group was treated with Aceclofenac Sustained-release Tablets and Mecobalamin, and the observation group was treated with Tuina combined hot compress. Both groups were treated for 1 month and followed up 6 months. The Visual Analogue Scale (VAS) was used to evaluate the degree of pain. The Oswestry Dysfunction Index (ODI) was used to evaluate the recovery of lumbar function, and the improvement of daily activities of patients after treatment was evaluated. The conduction velocity of tibial nerve motor conduction velocity (TNMCV) and common peroneal nerve motor conduction velocity (CPNMCV) were measured by electromyography. The clinical effect was evaluated.Results:After 6 months of follow-up, the response rate of the observation group was 51.1% (23/45) and that of the control group was 28.9% (13/45) ( χ2=3.98, P=0.046). The VAS score of the observation group was significantly lower than that of the control group ( t=11.26, P<0.01). The ODI scores at 1 month after treatment and 6 months after follow-up were significantly lower than those in the control group ( t values were 7.19, 18.30, respectively, all Ps<0.001). The observation group 1 month after CPNMCV treatment and 6 months of follow-up ( t=2.08, 1.99), 1 month after TNMCV treatment and 6 months of follow-up ( t values were 2.46, 2.12, respectively) were significantly higher than those in the control group ( P<0.05). Conclusion:Tuina combined with hot compress can improve the nerve conduction velocity of lower limbs, relieve the symptoms of low back and leg pain in patients after LDH operation.

ABSTRACT

Introducción: El dolor musculoesquelético es una consulta común en la adultez y su etiología generalmente es banal. Sin embargo, hay que tener presente que los procesos neoplásicos pueden causar este síntoma. El estudio con imágenes puede entregar valiosa información complementaria cuando los primeros síntomas derivan de la infiltración de los tejidos y el compromiso de las series hematológicas es discreto y asintomático. En el caso de la Leucemia Linfoblástica Aguda, las lesiones osteolíticas severas pueden ser la primera manifestación de la enfermedad por lo que su consideración es fundamental para el diagnóstico temprano. Caso clínico: paciente de 26 años, sin antecedentes mórbidos, con cuadro de dolor lumbar de inicio sub-agudo de 2 semanas de evolución, sin respuesta a analgesia, que evoluciona con elevación de parámetros inflamatorios y fiebre. Ante diagnóstico de lumbago con banderas rojas se solicita estudio con RNM con contraste que muestra lesiones sugerentes de síndrome linfoproliferativo. El estudio con mielograma e inmunohistoquímica confirmó el diagnóstico de Leucemia Linfoblástica Aguda. El paciente fue derivado a Servicio de Hematología para resolución con buena evolución posteriormente. Discusión y conclusiones: El dolor lumbar en el adulto es en general de tipo mecánico, respondiendo a manejo ambulatorio, no siendo necesario indagar en el estudio salvo que se acompañe de banderas rojas. Existen pocos reportes en la literatura de lumbago como debut de leucemia linfoblástica aguda en el adulto. El estudio con imágenes en estos casos es fundamental, más aún en ausencia de síntomas clásicos de leucemia y alteraciones en el perfil hematológico.


Introduction: Musculoskeletal pain is a frequent cause of consultation in adults and its etiology is generally banal. However, it should be borne in mind that neoplastic processes can cause this symptom. When the first symptoms are caused by tissue infiltration and the complete blood count is only mildly affected and asymptomatic, imaging studies can provide valuable complementary information. Severe osteolytic lesions may be the first sign of Acute Lymphoblastic Leukemia, so their consideration is fundamental for early diagnosis. Clinical case: 26 year old patient, with no medical history presented with two weeks of sub-acute lumbar pain with no response to analgesia, then developed elevated inflammatory parameters and fever. Contrast MRI done because of a red flagged diagnosis of low back pain, showed lesions suggestive of a lymphoproliferative syndrome. The bone marrow biopsy confirmed the diagnosis of Acute Lymphoblastic Leukemia. The patient was referred to Hematology for treatment and follow up and had a satisfactory clinical outcome. Discussion and conclusions: Low back pain in adults is generally mechanical and responds to outpatient treatment, there being no need of study unless there are red flags. There are few reports in the literature of lumbago as the presenting symptom of Acute Lymphoblastic Leukemia in adults. In these cases, imaging studies are fundamental, particularly so in absence of classic symptoms of leukemia and alterations in the hematological parameters.

ABSTRACT

Lumbago disease is a common disease, which mainly manifested as acute or chronic pain in the lumbar spine or paraspinal region, mainly represented by lumbar disc herniation in clinical practice. The new drug, Yaotongning Capsule with better efficacy for lumbago in clinic, is made of Strychni Semen Pulveratum, Eupolyphaga Steleophaga, Cyathulae Radix, Glycyrrhizae Radix et Rhizoma, Ephedrae Herba, Olibanum and Myrrh (prepared with vinegar), scorpion, Bombyx Batryticatus (bran fried), and Atractylodis Rhizoma (bran fried). This paper analyzes the etiology and pathogenesis of lumbar intervertebral disc herniation through traditional Chinese medicine, and explores the mechanism of treating lumbar intervertebral disc herniation with Yaotongning Capsule from the perspective of prescription theory, so as to provide theoretical basis for clinical application of Yaotongning Capsule.

ABSTRACT

Objective: To systematic evaluate the efficacy and safety of Yaotongning Capsule in the treatment of lumbago and leg pain. Methods: The databases of CNKI, VIP, Wanfang and Chinese science citation were searched by computer, and the randomized controlled trials of Yaotongning Capsule in the treatment of lumbago and leg pain were collected. Two researchers independently selected literatures, entered data, and evaluated the quality of literature methodology. Review Manager 5.3 software was used for Meta analysis. Results: This study included 11 RCTs, 1 200 patients, 599 cases in Yaotongning group and 601 cases in control group. Meta analysis showed that the effective rate of Yaotongning Capsule in the treatment of lumbocrural pain was better than that of the control group (OR = 4.13, 95% CI [2.90, 5.88]), P < 0.000 01; The improvement of VAS pain score of Yaotongning Capsule was better than that of the control group (SMD = -1.22, 95% CI [-1.71, -0.73]), P < 0.000 01, (SMD = -2.09, 95% CI [-2.14, -2.04]). The improvement of JOA function score was better than that of the control group (SMD = 6.78, 95% CI [5.21, 8.26]), P<0.000 01, the difference was statistically significant. Conclusion: Yaotongning Capsule can effectively improve the lumbar activity function and clinical symptoms of patients with lumbocrural pain, and the degree of pain relief is positively correlated with the course of treatment and dose. However, the included sample size and observation indicators may lead to bias. It is expected that more researchers can conduct multi center, large sample, scientific and complete research to evaluate the efficacy and safety of Yaotongning Capsule in the treatment of lumbocrural pain.

ABSTRACT

OBJECTIVE: To dig the knowledge structure of lumbago's symptoms and acupoints in book Zhenjiu Jiayi Jing (A-B Classic of Acupuncture and Moxibustion) and Zhenjiu Dacheng (Compendium of Acupuncture and Moxibustion) in order to provide a possible scientific method for exploring their combination regularities in ancient Chinese medical books about acupuncture-moxibustion, and to investigate the feasibility and scientificity of the "association rule" in trimming the related ancient medical books and records. METHODS: The articles about lumbago's symptoms and employed acupoints were screened from classic me-dical book Zhenjiu Jiayi Jing and Zhenjiu Dacheng, followed by establishing a database. Then, the bidimensional correlation analysis about the lumbago-symptoms and symptoms-acupoints was conducted by using Apriori algorithm of Weka3.9.0 platform. RESULTS: A total of 153 entries about symptoms as spinalgia (31.68%) and cold limbs (25.74%) and the employed acupoints as Weizhong (BL40,17.60%), Shenshu (BL23, 9.60%) and Kunlun (BL60, 6.40%) are found. The top 3 supplemented acupoints are BL40 for concomitant symptoms of spinalgia, BL23 for cold limbs, and BL60 for dizzy symptoms. Regarding the paired acupoints for lumbago, the most frequently used ones are BL23-BL40 (30.40%).. CONCLUSION: In book Zhenjiu Jiayi Jing and Zhenjiu Dacheng, lumbago has chief symptoms of spinalgia and cold limbs that are mainly disorders of the Bladder Meridian of Foot Taiyang and Governor Vessel, and the acupoints of these two meridians are frequently selected in combination with some specific acupoints.

ABSTRACT

OBJECTIVE: To observe the effect of long-snake moxibustion on pain, functional disorder and body constitution in the patients with lumbago of cold-dampness type. METHODS: A total of 90 patients with lumbago of cold-dampness type were randomized into long-snake moxibustion, acupuncture and medication groups, 30 cases in each group. In the long-snake moxibustion group, long-snake moxibustion was exerted on the spinal (between Dazhui[GV14] and Yaoshu[GV2]) with the self-prepared moxa powder formula combined with fresh ginger and moxa wool, once a week, for 4 weeks. Regular acupuncture was given to the acupuncture group, 30 min each time, once a day, 8 times as a treatment course, at the interval of 2 days between the courses. The duration of treatment was 1 month. In the medication group, Diclofenac Sodium was prescribed for oral administration, 25 mg each time, three times a day, and Mecobalamine tablets, 0.5 mg each time, 3 times a day. The duration of medication was 1 month. Before and after treatment, the differences were evaluated in the scores of visual analog scale (VAS), the present pain intensity(PPI), Oswestry disability index (ODI) and the scale of yang-deficiency body constitution among the 3 groups. RESULTS: After the treatment, VAS and PPI score and ODI were significantly reduced in the patients of the 3 groups as compared with those before treatment (all P0.05). The score of the scale of yang-deficiency body constitution in the long-snake moxibustion group was significantly lower than that in the acupuncture group and the medication group (P<0.05). Regarding the clinical effect, the effective rate was 93.33% (28/30) in the long-snake moxibustion group and it was 86.67% (26/30) in the acupuncture group and 73.33% (22/30) in the medication group after the treatment. The effective rate in the long-snake moxibustion group was significantly higher than those in the acupuncture group and the medication group (P<0.05). CONCLUSION: The long-snake moxibustion therapy can achieve significant effect on lumbago of cold-dampness type. This therapy can alleviate pain, relieve the functional disorders and improve the conditions of body constitution in the patients. Hence, it is applicable for the clinical promotion.

ABSTRACT

Introducción: los trastornos músculo-esqueléticos ocupacionales en el sector laboral eléctrico han generado gran interés a nivel mundial desde hace décadas. Objetivo: la presente investigación tuvo como objetivo aportar conocimientos acerca de la frecuencia de afecciones músculo-esqueléticas relacionadas con factores de riesgo ergonómicos, en trabajadores de la Empresa Eléctrica de Riobamba. Metodología: se llevó a cabo un estudio descriptivo y transversal con enfoque cuali-cuantitativo, lo cual permitió analizar el objeto de estudio en su contexto real. La muestra estuvo constituida por 271 trabajadores, con edades de 22 a 58 años. La información obtenida se contrastó con estudios relacionados, encontrados a partir del uso de motores de gestión informativa convencionales y la revisión documental in situ. Resultados: la mayoría de los sujetos estudiados (63%) desarrolló algún trastorno músculo esquelético. Predominó el sexo masculino (65%) y el lumbago representó la afección más frecuente (30%). El segmento anatómico más afectado fue el tronco (17%). Conclusiones: el lumbago resultó ser una condición frecuente que afectó mayormente a trabajadores del sexo masculino y que se desempeñaban como linieros. Por la importancia de los resultados encontrados se sugirió a la Empresa reforzar las intervenciones preventivas y de vigilancia epidemiológica para evitar y detectar tempranamente los trastornos músculo-esqueléticos en sus empleados.

ABSTRACT

Objective To analyze the clinical effects of Shenzhuo Decoction combined with massage manipulation, acupotomy and lumbar traction in treatment of lumbar disc herniation. Methods A total of 495 cases of lumbar disc herniation from January 2015 to December 2018 were divided into the observation group and control group, with 259 and 236 cases respectively in observation and control groups. The patients in control group were only given regular massage manipulation, acupotomy and lumbar traction, and the patients in observation group were given Shenzhuo Decoction based on the treatment of control group. The clinical effects of the two groups was analyzed after 2 weeks of treatment. Results The total effective rate and JOA Score of observation group were significantly better than that in the control group (P < 0.05). Conclusion Shenzhuo Decoction in the treatment of lumbar disc herniation has good curative effect. It has the advantages of quick curative effect and short courses of treatment with the regular massage manipulation, acupotomy and lumbar traction, which can be promoted in clinical practice as a conservative treatment for lumbar disc herniation.

ABSTRACT

We report three patients with acute lumbago who did not respond to nonsteroidal anti-inflammatory drugs (NSAIDs), but were treated successfully with daiobotampito and shimotsuto. Case 1 (86-year-old man) and Case 2 (56-year-old woman) were diagnosed with acute lumbago. Case 3 (69-year-old man) was diagnosed with acute lumbago and mild left sciatica. All three patients were initially administered orally NSAIDs, but this treatment was ineffective. Therefore, daiobotampito and shimotsuto were administered after choeikatsurakuto according to “Manbyokaishun.” Following this treatment, the patients' acute lumbago and sciatica resolved within 1-2 weeks. In conclusion, daiobotampito in combination with shimotsuto seems to be a useful Kampo medicine for treatment of lumbago.

Resumen:Las dolencias de la región lumbar constituyen un problema significativo y de importancia creciente, al punto que se ha considerado como un problema de salud publica en muchos países del mundo. Estadísticas de los Estados Unidos de Norte América estiman que aproximadamente dos terceras partes de la población adulta sufrió o sufre de dolor lumbar en algún momento de su vida, lo que constituye a la lumbalgía como una de las causas más comunes de la consulta externa de medicina general y especializada. Datos aportados por Instituto Nacional de Seguros, indican que tan solo en el año 2015 se atendieron 142863 casos por dolor lumbar, lo que significó 7.2 días en promedio de incapacidad temporal por persona, lo que asciende a la suma de 1028613 días (un millón veintiocho mil seiscientos trece), de incapacidad. Además de estos 142863 afectados, 1297 requirieron no sólo de incapacidad temporal, sino también de incapacidad permanente llegando a la cifra de 4.72% en promedio por persona. Con base en estos datos se decidió tomar los casos Valorados por Riesgos del Trabajo en la Sección de Médica del Trabajo de Departamento de Medicina Legal del Organismo de Investigación Judicial, cuya dolencia fuese la Lumbalgía y analizar cuales fueron los mecanismos de trauma implicados en el génesis de su dolencia así como las repercusiones que esta tuvo en cada uno de los evaluados.


Abstract:Diseases of the lumbar region are a significant and growing problem, to the extent that it has been considered a public health problem in many countries of the world. Statistics from the United States of America estimate that approximately two-thirds of the adult population suffered or suffer from low back pain at some point in their lives, which constitutes back pain as one of the most common causes of outpatient medical practice General and specialized. Data provided by the National Institute of Insurance indicate that only in 142863 cases of lumbar pain were treated in 2015, which meant 7.2 days in average of temporary incapacity per person, which amounts to 1028613 days (one million twenty-eight One thousand six hundred thirteen), of incapacity. In addition to these 142863 affected, 1,297 required not only temporary incapacity, but also permanent disability reaching the figure of 4.72% on average per person. Based on these data it was decided to take the cases Risks of Work in the Department of Occupational Medicine of the Department of Legal Medicine of the Judicial Investigation Agency, whose condition was Lumbalgia and to analyze the mechanisms of trauma involved in the genesis Of its ailment as well as the repercussions that this one had in each one of those evaluated.

The treatment group received She medicine therapy and the control group, oral administration of conventional drugs. The excellent and good rate was observed in the two groups after treatment. Post-treatment active straight leg raising times and analgesic dosages were compared between the two groups.Results Active straight leg raising time was significantly advanced (P<0.01) and the number of potent analgesic uses was significantly decreased (P<0.01) in the treatment group after endoscopic surgery for lumbar intervertebral disc herniation. The excellent and good rate was 93.3% in the treatment group and 80.0% in the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusion She medicine therapy can help rapid rehabilitation after endoscopic surgery for lumbar intervertebral disc herniation.

ABSTRACT

Objective To observe the clinical efficacy of acupuncture plus activator method chiropractic technique (AMCT) in treating lumbar facet joint disorders.Method A total of 40 patients with lumbar facet joint disorders were randomized into a treatment group and a control group, 20 cases in each group. The treatment group was intervened by acupuncture plus AMCT, while the control group was treated with AMCT alone. The Visual Analogue Scale (VAS) and Japanese Orthopaedic Association (JOA) were observed in the two groups before and after 3 treatment sessions, and the clinical efficacies were compared between the two groups.Result The VAS score and JOA score were significantly changed right after the 1st treatment and after 3 treatment sessions in the two groups (P<0.05). Right after the 1st treatment and after 3 treatment sessions, there were statistically significant differences in the VAS score and JOA score between the two groups (P<0.05). The total effective rate was 90.0% in the treatment group versus 60.0% in the control group, and the difference was statistically significant (P<0.05). The relapse rate was 22.2% in the treatment group versus 50.0% in the control group 3 months after the treatment, and the difference was statistically significant (P<0.05).Conclusion Acupuncture plus AMCT is an effective approach in treating lumbar facet joint disorders.

ABSTRACT

Objective To observe the clinical efficacy of navel acupuncture in treating lumbar spinal stenosis. Method A total of 120 patients with lumbar spinal stenosis were randomized into a treatment group and a control group, 60 cases each. The treatment group was intervened by navel acupuncture based on syndrome differentiation of traditional Chinese medicine, while the control group was intervened by oral administration of Bulleyaconitine A tablets, 10 d as a treatment course, for successive 2 courses. The Visual Analogue Scale (VAS) was observed before and after the intervention, and the clinical efficacies were compared.Result The total effective rate was 98.3% in the treatment group versus 76.6% in the control group, and the between-group difference was statistically significant (P<0.01). The VAS scores were significantly changed after the intervention in both groups (P<0.05); the VAS score in the treatment group was significantly different from that in the control group after the treatment (P<0.05).Conclusion Navel acupuncture is an effective approach in treating lumbar spinal stenosis.

ABSTRACT

As a clinical diagnosis and treatment of low back pain,straight leg raising test has been used most commonly more than 100 years.It is simple and practical,and has a high clinical value.As a very mature orthopedic special examination,straight leg raising test progress in non-orthopedic applications more often than orthopedic aspects.Of course,the most important thing is the diagnosis of low back pain,followed by the treatment of low back pain,and then can be used as a respiratory and circulatory aspect of diagnosis and treatment options.This article elaborated the principle of straight leg raising test and the clinical research progress in order to give the clinician some guidance.

ABSTRACT

Objective To investigate the clinical efficacy of exercise cupping plus acupuncture in treating acute lumbar sprain. Method Sixty-four patients with acute lumbar sprain were randomly allocated to treatment and control groups, 32 cases each. The treatment group received exercise cupping plus acupuncture and the control group, acupuncture alone. After one treatment and one course of treatment, the therapeutic effects were evaluated using the VAS for pain, lumbar joint activity and the clinical effect assessment. Result After one course of treatment, the total efficacy rate was 96.9% in the treatment group, which was significantly higher than 84.4% in the control group (P<0.05). After one treatment and one course of treatment, the VAS score was significantly lower in the treatment group than in the control group (P<0.05) and lumbar joint activity improved significantly in the treatment group compared with the control group (P<0.05). Conclusion Exercise cupping plus acupuncture is more effective than acupuncture alone in treating acute lumbar sprain.

ABSTRACT

Este trabajo tiene por objetivo determinar el papel de la acupuntura en el tratamiento del dolor lumbar inespecífico, identificando la mejor evidencia disponible que realice comparaciones con otro tipo de intervenciones, para establecer de manera objetiva su aplicabilidad clínica. Se realizó una revisión sistemática no estructurada como parte de la metodología con el fin de contestar las preguntas propuestas. Se identificaron en la búsqueda inicial 1155 referencias de las cuales finalmente cumplieron los criterios de inclusión 98, de estas referencias, fueron incluidas 19 revisiones sistemáticas y una vez realizado el análisis metodológico se encontraron 12 de alta calidad metodológica. Fueron adicionalmente incluidos cuatro artículos referentes a la acupuntura auricular. La adición de la acupuntura al tratamiento convencional es más efectiva en el manejo del dolor lumbar inespecífico tanto a corto como a largo plazo que la acupuntura o el tratamiento convencional como monoterapia, la electroacupuntura asociada con el tratamiento convencional es más efectiva para mejorar la función de la columna, alterada como consecuencia del dolor lumbar inespecífico y La auriculoterpia asociada con otras terapias es efectiva para el manejo del dolor lumbar inespecífico logrando un control adecuado del dolor.

ABSTRACT

ObjectiveTo investigate the clinical efficacy of abdominal catgut embedding therapy for chronic lumbago.Method Sixty patients with chronic lumbago were randomly allocated to abdominal catgut embedding (treatment) and abdominal acupuncture (control) groups, 30 cases each. An evaluation was made using the JOAscore in the two groupsbefore and after one month of treatment. The therapeutic effects were compared between the two groups.ResultThere was a statistically significant difference in the JOA scale score between before and after one month of treatment in both groups (P<0.01,P<0.05) and between the treatment and control groups after one month of treatment (P<0.05); the total effective rate and recovery rate of the treatment group were significantly higher than that of the control group (P<0.01).ConclusionAbdominal catgut embedding therapy is more effective than acupuncture therapy for chronic lumbago.
