ABSTRACT

A total of 23 samples of 21 patients designated Langerhans cell Histiocytosis [LCH] or eosinophilic granuloma were collected. Light microscopy showed that the picture of LCHs was a mixture of Langerhans [LCs] and eosinophils in varying proportions along with occasional giant cells of foreign body and Langhans giant cells, lymphocytes and neutrophils. Immunohistochemical study of the 21 cases [23 specimens] of LCHs showed mostly negative staining for alph alpha-1- antitrypsin. Out of the 23 specimens, 19 specimens [83%] were positive for S 100. CD Ta was performed on all cases with 100% positivity. The macrophages were negative for CD Ia and S100 but were positive for alpha-1- antitrypsin. The staining for NSE was positive for 19 specimens but it was non-specific as the histiocytes showed positive staining also. All the 21 cases [23 specimens] studied by electron microscopy had the characteristic features of Langerhans' cell Histiocytosis in the form of Birbeck granules along with the deeply cleaved or pseudo-multisegmented nuclei with finely dispersed, thinly marginated chromatin and an inconspicuous nucleoli. It was concluded that the diagnosis of LCH should be based on the presence of at least two out of four which are :-1- The clinical findings including radiological investigations 2- the presence of the typical Langerhans cells and eosinophils by light microscopy 3- positivity for CD Ta [OKT-6] by immunohistochemistry 4- The presence of Birbeck granules by electron microscopy

ABSTRACT

La histiocitosis de células de Langerhans con compromiso pulmonar es una enfermedad infrecuente y de causa desconocida, relacionada al hábito tabáquico. Puede presentarse clínicamente con síntomas respiratorios, o bien, ser un hallazgo en la radiografía de tórax. El diagnóstico se basa en estudio pulmonar mediante tomografía axial computada de alta resolución y se confirma por medio de biopsia. Se presenta el caso de un paciente varón, joven y fumador, cuya principal manifestación fue el desarrollo de un neumotórax bilateral espontáneo. Se determinó histológicamente la presencia de una histiocitosis de células de Langerhans. Se considera de interés este caso por su forma de presentación, que motiva el diagnóstico diferencial de los neumotórax espontáneos y las enfermedades difusas pulmonares y su eventual subdiagnóstico.

ABSTRACT

Relato de caso infreqüente de granuloma eosinofílico, a forma localizada da histiocitose de células de Langerhans, em paciente masculino de 24 anos com envolvimento cutâneo e pulmonar. A síndrome histiocítica é um grupo de alteraçöes proliferativas das células histocíticas que expressam o fenótipo de células de Lagerhans. Varia da forma benigna localizada a fatal e disseminada com envolvimento multicêntrico. Duas categorias foram recentemente estabelecidas: histiocitose de células de Langerhans e histiocitose de células näo-Langerhans. Os autores discutem os aspectos clínicos, histológicos, imunohistoquímicos e ultraestruturais desta forma rara de histiocitose, que pode lembrar várias outras condiçöes, como doença de Darier, dermatite seborréica e psoríase

ABSTRACT

The authors present a rare case of Langerhans cell histiocytosis in a 31 year old female patient with vulvar, peri-anal and oral lesions, diabetes insipidus, pulmonary skin and bone infiltrations. Skin biopsy immunohistochemistry presented positive S100 protein and vimentine, but the diagnosis was done with the demonstration of Birbeck granules with eletronic mucroscopy. The treatment was based on systemical chemotherapy although vulvar lesion has a bad response to chemotherapy.

ABSTRACT

FUNDAMENTOS - Foram observados dois pacientes de doenças imunes da pele, cuja extensäo foi delimitada pela lesäo cutânea de hanseníase, sugerindo comportamentos imunológicos diferentes entre área afetada e a pele sadia do mesmo paciente. OBJETIVOS - Verificar experimentalmente, pelo teste epicutâneo com DNCB e intradêrmico com candidina, se na lesäo cutânea da hanseníase TT e BT existem modificaçöes da imunidade celular em relaçäo à pele aparentemente normal do mesmo paciente. MÉTODOS - Foram selecionados 39 voluntários portadores de hanseníase TT e BT. Em 26, foi aplicado DNCB epicutâneo e, em 13, candidina intradérmica. Os resultados desses testes foram avaliados por: resposta clínica, estudo histopatológico e exame imuno-histoquímico dos componentes celulares das reaçöes. RESULTADOS - A resposta clínica à aplicaçäo epicutânea de DNCB na lesäo de hanseníase TT e BT foi maior do que na pele sadia do mesmo paciente. Näo houve diferença com a aplicaçäo intradérmica de candidina nesses locais. As CL apresentaram diminuiçäo de número nos locais de aplicaçäo de DNCB, permanecendo esse número inalterado nos locais testados com candidina. CONCLUSOES - A lesäo cutânea de hanseníase (TT e BT) responde de modo mais intenso ao imunógeno aplicado por via epicutânea do que a pele sadia do mesmo paciente. Sugere-se que a hanseníase (TT e BT) interfira na reatividade da pele ao nível da lesäo cutânea e que as células de Langerhans näo participem dessa reatividade

ABSTRACT

FUNDAMENTO - O condiloma acuminado ou verruga genital é doença de etiologia viral, causado pelo HPV, e tem sido motivo de inúmeros estudos por seu envolvimento, principalmente, com o câncer ginecológico. As células de Langerhans säo imunomoduladores epidérmicos. OBJETIVO - Observar a morfologia das células de Langerhans e das células indeterminadas, estimando seu número nos casos de condiloma acuminado, por meio da microscopia eletrônica. CASUISTICA e MÉTODO - O estudo das células de Lagerhans foi realizado em seis pacientes do sexo masculino que apresentavam lesöes no sulco balanoprepucial. A idade dos pacientes variou entre 17 e 29 anos; o tempo de evoluçäo da doença, de 15 dias a um ano. RESULTADOS - Foram observados: 1. aumento do número de desmossomas nos ceratinócitos do condiloma acuminado; 2. näo houve uniformindade de aumento e diminuiçäo na quantidade de organelas citoplasmáticas das células de Langerhans e indeterminadas; e 3. näo houve diferenças significativas entre o número de células de Langerhans localizadas na epiderme das áreas com ou sem lesäo de condiloma acuminado. CONCLUSÄO - A quantidade encontrada de células indeterminadas foi maior do que a de células de Langerhans observadas à microscopia eletrônica, fato esse que näo é diferenciado métodos de pesquisa conhecidos para as células de Langerhans

ABSTRACT

The use of electron microscopy to define the cellular changes in vitiligo might prove to be a useful investigative procedure to clarify the pathogenic mechanism of this disease. Examination of vitiligenous skin taken from the margin and center of the depigmented lesions revealed ultrastructural changes in the form of cytoplasmic vacuoles in basal and suprabasal keratinocytes and deposits of extra-cellular granular material in between the keratinocytes in lower epidermis. Melanocytes and melanosomes were completely absent, while Langerhans' cells showed highly vacuolated cytoplasm. There was marked increased in degenerated Langerhans' cells in the center of the vitiligenous lesions. It seems logical to propose that degenerative changes in vitiligo do not only involve the melanocytes, but also keratinocytes and Langerhans' cells i.e. the whole epidermal- melanin unit

ABSTRACT

A direct comparison of skin Langerhans cell (LC) morphologic change following in vivo and in vitro exposure to dengue-2 (DEN-2) virus (16681) was performed in the monkey to investigate any differences in functional activity profiles. Time-lapse study of skin biopsy at the intradermal (id) virus injection sites, and thin skin sheets removed from the monkey with exposure to virus in culture medium, revealed a highly active migration of epidermal LCs in both sets of experimental specimens. The migration led to a relatively higher number of dendritic cells (DC) which appeared in active migrational profiles, in the superficial dermis. Moreover, obvious cytoplasmic structural changes, corresponding to their immunologic function, were observed in these superficial dermal DCs 2 hours after exposure. Despite their similar changes, early and late endosomes with degraded virus-like particles could be seen in the skin sheets owing to lagging in cellular physiological process in vitro, but none in the skin biopsies. Existence of these endosomes, which was extremely difficult to visualize in vivo, highlighted the mode of antigen processing by the endocytic pathway. The present study showed that the epidermal LC was a potent antigen-presenting cell for eliciting the success of id immunization and carried out the immunological activity in vivo or in vitro in the like manner, in respect to the physiological conditions.

ABSTRACT

A study of histopathologic changes, ultrastructure, and expression of the HLA-Dr antigen within the giant papillae of patients with giant papillary conjunctivitis was performed to determine whether cell-mediated immune response is related to this condition. Conjunctival giant papillae from ten patients with giant papillary conjunctivitis were examined by light and electron microscopy and by the indirect immunofluorescent staining method with HLA-Dr antibody. The infiltration of eosinophilic neutrophils and granules was most prominent, with the occasional infiltration of mast cells, as shown by light microscopy. The infiltration of activated fibroblasts and Langerhans cells was also observed. Cells expressing HLA-Dr antigen were also markedly increased, as shown by the immunofluorescent method. These findings suggest that delayed hypersensitivity may, along with the processes of antigen presentation by HLA-Dr-expressing (including Langerhans) cells, contribute to the pathogenesis of giant papillary conjunctivitis.

A resoluçäo näo cirúrgica dos cistos periodontais apicais ainda é questionável, principalmente porque sua comprovaçäo depende de resultados obtidos clínica e radiograficamente, näo fundamentados em observaçöes microscópicas. A partir de 348 casos registrados e organizados nos arquivos do Departamento de Patologia, da Faculdade de Odontologia de Bauru da Universidade de Säo Paulo, utilizando-se das técnicas de coloraçäo de rotina e de imuno-histoquímica, propusemo-nos: 1) a observar microscopicamente as alteraçöes morfológicas do revestimento epitelial, da parede conjuntiva e do lume dos cistos periodontais apicais de dentes tratados ou näo endodonticamente e dos cistos periodontais residuais, para estabelecer-se padröes morfológicos microscópicos próprios; 2) interrrelacionar os dados obtidos nos diversos grupos experimentais, a fim de implicar os aspectos microscópicos que possam sugerir uma regressäo näo cirúrgica dos cistos periodontais apicais de dentes tratados endodonticamente; 3) analisar as distribuiçöes das células B, das células T e das células de Langerhans nas estruturas dos cistos periodontais apicais de dentes tratados ou näo endodonticamente e dos cistos residuais, para estabelecer-se padröes próprios na distribuiçäo das células imunológicas, marcadas imuno-histoquimicamente, em cada grupo experimental já mencionados, e, correlacioná-los com os padröes microscópicos próprios, para aferir uma provável regressäo näo cirúrgica desta periapicopa...

ABSTRACT

Secciones de exocervix humano normal obtenidos de pacientes histerectomizados fueron cortadas por congelación con el criostato y procesadas para el marcado con técnicas inmunoohistológicas. Otras secciones adyacentes fueron fijadas y procesadas para microscopia electrónica. Este procedimiento combinado fue elegido para localizar dos antígenos de superficie (T6 y HLA-DR) en las células de Langerhans y al mismo tiempo identificarlas por su marcador ultraestructural, el gránulo de Birbeck. Un tercer anticuerpo monoclonal (T8) fue empleado para marcar una población de linfocitos migratorios que se encuentran cerca de las células de Langerhans en el epitelio y también en la lámina propia del ectocervix. La microscopia electrónica revela que ambas, las células de Langerhans y los linfocitos T, ocupan los canales intecelulares del epitelio cervical. Se postula que en el tracto genital gemenino las células de Langerhans funcionarían como presentadoras de antígenos para los linfocitos T en condiciones fisiológicas normales

ABSTRACT

ATPase staining and ultrastructural study of skin biopsies from six patients of leprosy (2TT, 4LL) and three normal subjects was carried out to study Langherhans Cells (LC). ATPase staining showed normal counts of LCs in tuberculoid patients, while significant reduction was observed in lepromatous cases. Electron microscopy revealed morphological changes in LL cases in the form of dense matrix and indistinct cristae of mitochondria; decreased number of lysosomes and rough endoplasmic reticulum; and numerous vacuoles in cytoplasm. TT cases showed normal morphology. Possible role of Langerhans Cells in pathogenesis of leprosy is discussed.

ABSTRACT

Se pasa revista a las características que individualizan a las células de Langerhans epidérmicas, señalándose que dichas características están presentes también en ciertas células extraepidérmicas. Entre los conceptos actuales se destacan un origen mesenquimático y una función inmunitaria de las células de Langerhans. Esta última ha sido estudiada en detalle en la dermatitis alérgica por contacto. Se indica la presencia de estas células en la histiocitosis X y en la micosis fungoide. En base a la posible acción inmunológica de las células de Langerhans, se hacen comentarios sobre la importancia e implicaciones de esta función de vigilancia
