RESUMEN

PURPOSE: Recent reports have posed doubts about the effect of preradiosurgical embolization in brain arteriovenous malformation (AVM) because it makes the planning of stereotactic radiosurgery (SRS) difficult and has the risk of recanalization out of the target. We investigated whether the performance and quality of embolization may influence the success of SRS based on a retrospective case cohort study. MATERIALS AND METHODS: Seventy-three patients who underwent embolization followed by SRS between 2003 and 2012 in eight institutes with neurointerventionists were considered. They were divided into the following two groups at 3 years of follow up after the final SRS: “successful occlusion group” (S group), with radiologically complete occlusion of AVM; and “non-successful occlusion group” (N group) with persistent remnant nidus or abnormal vascular networks. Patient background, AVM profile, embolization performance grade and complications were compared in each group. The quality of embolization was evaluated with the new grading system: embolization performance grade (E grade), specializing the achievement of nidus embolization. E grade A was defined as sufficient nidus embolization with more than half of the total number of feeders achieving nidus penetration. E grade B was defined as less than half achievement of nidus embolization, and E grade C was defines as failure to perform nidus embolization. RESULTS: Forty-three patients were included in the S group, and 29 patients were included in the N group. The size and Spetzler-Martin grade of AVM and the rate of diffuse type was higher in the N group without statistical significance. The embolization performance level according to E grade indicated a significantly higher rate of successful embolization with more than 50% of nidus penetration in the S group (P<0.001). This difference was also confirmed in the subanalysis for limited cases, excluding smaller AVMs with complete occlusion with SRS alone (P=0.001). CONCLUSION: The cause of the unsuccessful result of post-embolization SRS might be the large, diffuse angioarchitecture, but proper embolization with a high rate of nidus penetration to avoid recanalization is more important. Effective embolization is essential to contribute to and promote the effect of radiosurgery.

OBJECTIVE:To establish the method for simultaneous determination of 5 heavy metals in Vespae Nidus,and to dis-cuss the effects of difterent producing area and part on the contents of heavy metals in Vespae Nidus. METHODS: The contents of arsenic(As),mercury(Hg),lead(Pb),cadmium(Cd)and copper(Cu)were determined by atomic spectrophotometry(hydride generation method,cold vapor method,graphite furnace method and flame method). The difference in the contents of heavy metals in samples were compared among different producing areas by analyzing data. RESULTS:The linear range of As,Hg,Pb,Cu, Cd were 0-40 ng/mL(r=0.9930),0-0.9 ng/mL(r=0.9974),0-80 ng/mL(r=0.9955),0-8 ng/mL(r=0.9927),0-800 ng/mL(r=0.9966),respectively. Limit of quantitation were 4.473,2.35×10 - 3 ,8.380,2.128,54 ng/mL,limit of detection were 1.342,0.701× 10 - 3,2.514,0.632,16 ng/mL. RSDs of precision,stability and reproducibility tests were all lower than 5.0%. The recoveries were 84.26% -102.25%(RSD=7.71% ,n=6),90.90% -115.31%(RSD=9.38% ,n=6),92.16% -105.70%(RSD=6.19% ,n=6), 90.81%-113.99%(RSD=8.86%,n=6),91.63%-102.79%(RSD=5.09%,n=6). The phenomenon that heavy metals of sample from Shandong and Hebei exceed the standard was the most serious. Main heavy metal which exceed the standard was Pb. The con-tent of Pb in Chamber was higher than other parts. CONCLUSIONS: The method is simple,precise,stable and reproducible,and can be used for simultaneous determination of 5 heavy metals in Vespae Nidus. There are different contents of heavy metal in Ves-pae Nidus of different producing areas and parts.

Objective:To establish a method for the content determination of total nitrogen in Nidus Vespae by salicylic acid and sodium hypochlorite spectrophotometry. Methods:The content of total nitrogen in Nidus Vespae from different areas with various batches was detected. Nidus Vespae was treated with sulfuric acid and hydrogen peroxide, and colored by salicylic acid and sodium hypochlo-rite. Using ammonium chloride as the control sample, the content of total nitrogen in the samples was tested at 650 nm. To ensure the method reliability, single factor experiments were performed to study the effects of temperature, reaction time and reagent dosage; or-thogonal experiments were used to screen optimum amounts of phosphate buffer solution, sodium salicylate solution and sodium hypo-chlorite solution. Results:The results showed that the optimum temperature was ambient, and the optimum time was 1. 5h. The opti-mum amount of phosphate buffer solution , sodium salicylate solution and sodium hypochlorite solution was 4 ml,7 ml and 1. 5 ml, re-spectively. The linear range of total nitrogen was 0. 06-0. 78 μg·ml-1(r=0. 9998), and the average recovery was 100. 98% (RSD=2. 10%,n=6). Conclusion:The method can be used to determine the total nitrogen content in Nidus Vespae. The total nitrogen content in Nidus Vespae is various in various areas with different batches.

RESUMEN

The Cerebral Arteriovenous Malformation (CAVM) is a complex neurovascular malformation. It contains different types of complex nidus structures combinations. Each CAVM classification is of different clinical procedure and management of disease. The clinical procedure to measure hemodynamics for each CAVM classification is risky, due to complex vessel geometry may get ruptured. In this paper, we address this problem for different AVM classification and nidus complex structures combinations, by measuring hemodynamics non-invasively. The simulation of model is extended for organs other than Cerebral AVM, such as spine and lungs. The modeling results are validated with mechanical measurements. 15 CAVM patients and 20 simulated scenarios of different AVM types are studied to measure hemodynamic indices non-invasively.

Objective To observe the inhibitory effect of the extracts of Nidus Collocaliae on avian influenza A H5N1 virus in vitro. Methods Nidus Collocaliae water extract, artificial gastric juice digestion products of Nidus Collocaliae water extract, and artifitial intestinal juice digestion products of Nidus Collocaliae water extract were prepared for the experimental study. 293T cells transfection in vitro was carried out. The effects of 3 kinds of Nidus Collocaliae extracts on H5N1 pseudovirus and VSV-G pseudovirus were determined by luciferase detection kit. Blood clotting response to erythrocyte hemagglutinin subtypes H5, H7, H9 antigens and the inhibitory effects of 3 kinds of Nidus Collocaliae extracts were observed. The effects of 3 kinds of Nidus Collocaliae extracts on neuraminidase activity were determined by neuraminidase inhibitor screen kit. Results The 3 kinds of Nidus Collocaliae extracts had inhibitory effects on H5N1 avian influenza pseudovirus, the effects being enhanced with the increase of the concentrations of Nidus Collocaliae extracts. Of the 3 extracts, artificial intestinal digestion products had the strongest inhibitory effect, while Nidus Collocaliae water extract had the weakest effect. However, Nidus Collocaliae extracts had no obvious effect on VSV-G pseudovirus. The concentration of H5, H7 antigen for positive blood clotting response was 1 ∶ 128, and that of H9 antigen was 1 ∶ 256. The 3 kinds of Nidus Collocaliae extracts at certain concentrations could inhibit blood clotting response to H5, H7, H9 antigen, but had no obvious effect on neuraminidase. Conclusion The anti-H5N1 virus effect of Nidus Collocaliae extracts has been achieved probably through resisting hemagglutinin.

Plastic biliary stents are commonly used during endoscopic retrograde cholangiopancreatography. Main indication for biliary stenting is benign or malignant obstruction. Plastic stents can be used as an escape route in patients with large common bile duct stones to provide drainage until definitive treatment. But, stent occlusion is the main disadvantage, limiting their patency to around 3 months, after which replacement is recommended. A biliary stent can act as a nidus for the biliary stone formation leading to stent-stone complex after long-term stent placement. This report was a case of a large stent-stone complex after plastic stent placement for 4 years. The stent-stone complex was successfully removed by mechanical lithotripsy and ballon catheter. In all other cases where plastic stents are placed into the common bile duct we should keep in mind that stents can act as nidus for stone formation, as all foreign bodies do.

AbstractObjective:To report the results of computed tomography (CT)-guided percutaneous resection of the nidus in 18 cases of osteoid osteoma.Materials and Methods:The medical records of 18 cases of osteoid osteoma in children, adolescents and young adults, who underwent CT-guided removal of the nidus between November, 2004 and March, 2009 were reviewed retrospectively for demographic data, lesion site, clinical outcome and complications after procedure.Results:Clinical follow-up was available for all cases at a median of 29 months (range 6–60 months). No persistence of pre-procedural pain was noted on 17 patients. Only one patient experienced recurrence of symptoms 12 months after percutaneous resection, and was successfully retreated by the same technique, resulting in a secondary success rate of 18/18 (100%).Conclusion:CT-guided removal or destruction of the nidus is a safe and effective alternative to surgical resection of the osteoid osteoma nidus.


ResumoObjetivo:Avaliar os resultados da ressecção percutânea do nidus guiada por tomografia computadorizada em 18 casos de osteoma osteoide.Materiais e Métodos:Os prontuários médicos de 18 crianças, adolescentes e adultos jovens diagnosticados com osteoma osteoide e submetidos a ressecção percutânea do nidus guiada por TC, entre novembro de 2004 e março de 2009, foram revisados retrospectivamente para coleta de dados demográficos, local da lesão, evolução clínica e complicações após o procedimento.Resultados:A mediana de tempo de seguimento de todos os pacientes foi 29 meses (intervalo de 6–60 meses). Dezessete pacientes não apresentaram recorrência da dor no pós-operatório. Apenas um paciente teve recorrência dos sintomas 12 meses após a ressecção percutânea e foi retratado com sucesso, pela mesma técnica, resultando numa taxa de sucesso secundário de 18/18 (100%).Conclusão:A ressecção guiada por tomografia computadorizada do nidus é uma alternativa segura e eficaz para a ressecção cirúrgica do nidus em pacientes com osteoma osteoide.

Traditional Chinese medicine Nidus Vespae, also known as hive, is an important and commonly used animal drug. There are related records about its applications in ancient Chinese medicinal literature. It possesses the efficacy of dispelling the wind, relieving pain, counteracting toxic substances and killing insect, and is widely employed as single medicine or an ingredient of compound medicine. The objective of this review is to provide a comprehensive reference for further development and utilization of Nidus Vespae. Thus, we summarize its chemical constituents, clinical applications, and pharmacological mechanism of action.

RESUMEN

Sialoliths are calcified organic matter that forms within the secretory system of the major salivary glands. Salivary gland calculi account for the most common disease of the salivary glands, and may range from tiny particles to several centimeters in length. The majority of sialoliths occur in the submandibular gland or its duct and is a common cause of acute and chronic infections. While the majority of salivary stones are asymptomatic or cause minimal discomfort, larger stones may interfere with the flow of saliva and cause pain and swelling. The prevalence of sialoliths varies by location. Sialolith in the parotid glands is less common when compared with that of submandibular gland. This case report describes a patient presenting with parotid gland sialolith and review of the literature regarding the salivary sialothiasis.

RESUMEN

The authors report a case of recurrent expansile cyst formation after gamma knife radiosurgery(GKRS) for a cerebral arteriovenous malformation where the residual nidus might play an important role. A 28-year-old male patient underwent gamma knife radiosurgery for a left parietal lobe arteriovenous malformation which had manifested with generalized tonic-clonic seizure in November 1992. In March 1999, 76 months after GKRS, the patient presented with headache, dysphasia and right hemiparesis and the computed tomography(CT) and magnetic resonance(MR) image revealed a huge cystic lesion with enhancing nodule at the anterior wall of the cyst. Cerebral angiography showed a small residual nidus. Aspiration and drainage of the cystic fluid via a burr hole was performed and the symptoms improved. One year later, in March 2000, the symptoms recurred, and CT and MR image revealed a new huge cystic lesion anterior to the enhancing nodule. Surgical removal of the residual nidus was performed, and there is no recurrence of the cyst for two years till now.

OBJECTIVES:To isolate and purify a new peptide named as NV-PP-1,from anti-inflammatory protein extracts of Nidus Vespae and to characterize it.METHODS:It was purified by ammonium sulfate precipitation,DEAE-Sephadex A50 ionexchange chromatography,Sephadex G-50 gel filtration and HPLC,respectively.Then it was characterized by HPLC on Protein-PakTM-60 column,IEF-HPCE(isoelectric focusing-high performance capillary electrophoresis),UV spectrum,amino acid analysis,and the N-terminal analysis with DNS-Cl.RESULT:Both HPLC on Protein-PakTM-60 column and IEF-HPCE showed its homogeneity.Its molecular weight was estimated to be 7.079 kD by HPSEC.UV spectrum indicated its maximum absorption at 274nm.Its PI was determined to be 4.08 according to IEF-HPCE.The terminal analysis showed its N-terminal amino acid was blocked.Amino acid analysis of NV-PP-1 indicated that it was rich in Ser,Glu,Gly.CONCLUSION:NV-PP-1 is the first peptide isolated from the anti-inflammatory protein extracts of Nidus vespae.

Objective To study the mechanism of protein extracted from nidus vespae protein(NVP) on anti-leukemia and provide experimental and theoretical evidence for exploring new anti-leukemia drugs. Methods The effect of the NVP on cell growth and morphological changes of K562 cells was observed by using cell culture and transmission electronic microscopy,the effect of NVP on apoptosis-associated signal transduct protein NF-? Bp65,?-catenin and iNOS of K562 cells was detected via immunohistochemical method and image analysis system. Results(1) The saturation destinity of K562 cells in NVP group was decreased which suggested that the proliferation was inhibited.(2) In the NVP group typical apoptosis morphological changes were observed in the K562 cells under transmission electronic microscope.(3) The result of immunohistochemistry showed that the expression of NF-? Bp65,?-catenin and iNOS of K562 cells in the NVP group decreased.The optical density(A) values of NF-?Bp65,?-catenin analysed by the image analysis system were decreased in the NVP group.Conclusion The NVP can inhibit the proliferation of K562 cell prominently,and the mechanism probably is that the apoptosis is induced via regulating the expression of apoptosis-associated signal transducted factors NF-?Bp65,?-catenin and iNOS.
