Objetivo: identificar ocorrência de surto compatível com escorbuto e fatores de exposição associados aos sinais/sintomas típicos de hipovitaminose, em penitenciária masculina, Ceará, Brasil, 2019-2020. Métodos: estudo de caso-controle populacional; foram utilizados registros clínicos e entrevistas com casos compatíveis - sinais/sintomas iniciados no período - e com controles; realizou-se análise multivariável. Resultados: 62 casos; idade média de 40,6 anos (DP = 10,8); principais sinais/sintomas foram edema e dor em membros inferiores (100,0%), dificuldade para deambular (91,9%), hematoma/equimose em membros inferiores (90,3%), febre (88,7%); identificou-se, como fator de exposição, média de idade > 40 anos (ORa = 1,10; IC95% 1,05;1,17; p-valor = 0,001); e como fatores protetores, trabalho (ORa = 0,11; IC95% 0,03;0,36; p-valor < 0,001) e participação em aulas (ORa = 0,21; IC95% 0,08;0,59; p-valor = 0,003) dentro da penitenciária. Conclusão: surto da penitenciária compatível com escorbuto pelos sinais/sintomas característicos, associados aos fatores identificados; recomendou-se oferta regular de dieta rica em vitamina C para todos os internos e acompanhamento clínico dos casos.


Objective: to identify the occurrence of an outbreak compatible with scurvy and exposure factors associated with typical signs/symptoms of hypovitaminosis that occurred in a male penitentiary in Ceará, Brazil between 2019-2020. Methods: this was a population-based case-control study; we used clinical records and interviews with compatible cases - based on sign/symptom onset during the study period - and with controls; we carried out multivariate analysis. Results: out of 62 cases, mean age was 40.6 years (SD = 10.8); main signs/symptoms were edema and pain in the lower limbs (100.0%), difficulty in walking (91.9%), hematoma/ecchymosis in the lower limbs (90.3%) and fever (88.7%); we identified being over 40 years old as an associated factor (aOR = 1.10; 95%CI 1.05;1.17; p-value = 0.001); and as protective factors: working (aOR = 0.11; 95%CI 0.03;0.36; p-value < 0.001) and taking part in classes (aOR = 0.21; 95%CI 0.08;0.59; p-value = 0.003) in the prison. Conclusion: we considered the penitentiary outbreak to be compatible with scurvy due to characteristic signs/symptoms, associated with the identified factors; we recommended regular provision of a diet rich in vitamin C to all male inmates and clinical follow-up of cases.

Métodos: estudio poblacional de casos y controles; se utilizaron historias clínicas y entrevistas con casos compatibles, con inicio de signos/síntomas durante el período de estudio, y controles; se realizó análisis multivariable. Resultados: de los 62 casos, edad media fue 40,6 años (DE = 10,8); principales signos/síntomas fueron edema y dolor en miembros inferiores (100,0%), dificultad para caminar (91,9%), hematoma/equimosis en miembros inferiores (90,3%) y fiebre (88,7%); la edad mayor de 40 años se identificó como factor asociado (ORa = 1,10; IC95% 1,05;1,17; p-valor = 0,001); y como factores protectores: trabajo (ORa = 0,11; IC95% 0,03;0,36; p-valor< 0,001) y participación en clases (ORa = 0,21; IC95% 0,08;0,59; p-valor = 0,003) dentro del centro penitenciario. Conclusión: el brote en centro penitenciario fue considerado compatible con escorbuto debido a los signos/síntomas característicos, asociados a los factores identificados; se recomendó oferta regular de dieta rica en vitamina C a todos los internos y seguimiento clínico de los casos.

El escorbuto es una enfermedad ocasionada por el déficit de vitamina C. La vitamina C, también llamada ácido ascórbico, actúa como cofactor de la propil-lisil hidroxilasa, enzima que interviene en la biosíntesis de colágeno; por ello es de vital importancia en la integridad estructural de piel, mucosas, anexos, vasos sanguíneos, huesos y dientes. Las manifestaciones clínicas incluyen síntomas constitucionales severos como debilidad y fatiga. Los hallazgos cutáneos iniciales son pápulas hiperqueratósicas foliculares y púrpura palpable perifolicular. Con el tiempo aparecen áreas extensas de equimosis, edema en miembros inferiores, hemorragias en astillas en uñas y alopecia difusa. Por lo general se describe esta enfermedad como asociada a tiempos pasados y extremadamente infrecuente en países desarrollados. Sin embargo, actualmente se ha detectado un aumento en el número de casos, asociados con malnutrición severa. Desórdenes psiquiátricos como la anorexia nerviosa, alcoholismo y deficiencias nutricionales por dietas excesivas y mal balanceadas o escasez de recursos económicos constituyen los principales factores de riesgo hoy en día. El tratamiento consiste en la corrección del déficit mediante la mediante suplementación con vitamina C vía oral. Presentamos una mujer de 24 años con escorbuto asociado a anorexia nerviosa y a dieta pobre en vegetales y fruta.


Scurvy is a disease caused by a deficit of vitamin C. Vitamin C, also called ascorbic acid, acts as a cofactor for propyl-lysyl hydroxylase, an enzyme that is involved in collagen biosynthesis, vitally important in the structural integrity of skin, mucous membranes, annexes, blood vessels, bones and teeth. Clinical manifestations include severe constitutional symptoms such as weakness and fatigue. The initial cutaneous findings are follicular hyperkeratotic papules and perifollicular palpable purpura. Over time, large areas of ecchymosis, edema of the lower limbs, splinter hemorrhages in nails and diffuse alopecia appear. Usually this disease is described as associated with past times and extremely uncommon in developed countries. However, the number of cases have been increasing, especially those associated with severe malnutrition. Psychiatric disorders such as anorexia nervosa, alcoholism and nutritional deficiencies due to excessive and poorly balanced diets or shortage of economic resources are the main risk factors nowadays. Correcting the deficit with oral vitamin C supplementation is the treatment of the disease. We present a 24-year-old woman with scurvy associated with anorexia nervosa and a diet low in vegetables and fruit.

OBJECTIVES: Reduced bone quality caused by vitamin C deficiency in older persons may lead to incidental fragility fractures during bisphosphonate treatment, although bisphosphonate increases bone mineral density (BMD). This study aimed to evaluate the effects of minodronate and ascorbic acid (Aa) on BMD, bone quality, and bone strength in Aa(-)deficient osteogenic disorder Shionogi (ODS) rats. METHODS: Six-month-old ODS rats were divided into four groups (n = 20 per group): (1) Aa supplementation (Aa(+)); (2) Aa(-)deficient (Aa(-)); (3) Aa supplementation and minodronate administration (Aa(+) + Mino); and (4) Aa(-)deficient and minodronate administration (Aa(-) + Mino). BMD, bone strength, bone histomorphometry, and bone quality determined using Fourier transform infrared spectroscopy imaging (FTIRI) were evaluated after 4 and 8 weeks. RESULTS: BMD was significantly higher in the Aa(+) + Mino group than in the Aa(-) group (p < 0.05). Bone strength was significantly higher in the Aa(+) and Aa(+) + Mino groups than in the Aa(-) group (p < 0.05). Furthermore, bone strength was significantly higher in the Aa(+) + Mino group than in the Aa(-) + Mino group (p < 0.05). Minodronate treatment irrespective of Aa supplementation significantly decreased bone resorption compared with the Aa(+) and Aa(-) groups (p < 0.05). No significant differences in the parameters evaluated by FTIRI were observed between the groups. CONCLUSIONS: Aa supplementation improved bone strength in ODS rats. Combined treatment with minodronate and Aa, but not minodronate alone, improved bone strength and increased BMD. Aa is required for bone health because it is essential for osteoblast differentiation.

Vitamins are essential micronutrients for maintenance of tissue functions. Vitamin deficiency is one of the most serious and common health problems among both chronic alcoholics and the homeless. However, the vitamin-level statuses of such people have been little studied. We evaluated the actual vitamin statuses of alcoholic homeless patients who visited an emergency department (ED). In this study the blood levels of vitamins B1, B12, B6, and C of 217 alcoholic homeless patients were evaluated retrospectively in a single urban teaching hospital ED. Vitamin C deficiency was observed in 84.3% of the patients. The vitamin B1, B12, and B6 deficiency rates, meanwhile, were 2.3%, 2.3%, and 23.5%, respectively. Comparing the admitted patients with those who were discharged, only the vitamin C level was lower. (P=0.003) In fact, the patients' vitamin C levels were markedly diminished, vitamin C replacement therapy for homeless patients should be considered in EDs.

Scorbutic Gingivitis was a condition of great prevalence among sailors of the yesteryears. With added advancement and diet consciousness the condition is a rarity now. Although scurvy is prevalent in population below poverty line, in this patient having scorbutic gingivitis, it was ironic that the patient starved due to religious belief and not due to circumstances. Vitamin C is the vitamin with the second largest daily requirement and plays a vital role in a host of metabolic reactions. Its lack produces a panorama of signs and symptoms, many of which present primarily or originally in the mouth. This case report of a rare case of Scorbutic gingivitis was diagnosed and treated in Department of Periodontology and reviews the case itself and the recent literature on this potentially fatal condition.

PURPOSE: Serum vitamin C is one of the indicators for antioxidant levels in the body and it is lower in cancer patients compared with the healthy population. However, there have been few studies on the levels of serum vitamin C in terminally ill cancer patients and related factors. METHODS: We followed 65 terminal cancer patients who were hospitalized in two palliative care units. We collected data of age, sex, cancer type, functional status, clinical symptoms, history of cancer therapy, and various laboratory findings including serum vitamin C level. Patients were categorized into two groups according to the quartile of serum vitamin C level (Q1-3 vs. Q4), which were compared each other. Stepwise multiple logistic regression analysis was used to identify factors related to serum vitamin C levels. RESULTS: The mean serum vitamin C level was 0.44 microg/mL, and all patients fell into the category of vitamin C deficiency. Univariate analysis showed that The serum vitamin C level was lower in non-lung cancer patients (P=0.041) and febrile patients (P=0.034). Multivariate analysis adjusted for potential confounders such as lung cancer, fever, dysphagia, dyspnea, C reactive protein, and history of chemotherapy demonstrated that odds for low serum vitamin C level was 3.7 for patients receiving chemotherapy (P=0.046) and 7.22 for febrile patients (P=0.02). CONCLUSION: Vitamin C deficiency was very severe in terminally ill cancer patients, and it was associated with history of chemotherapy and fever.

OBJETIVOS: Verificar adequação do consumo de vitamina C em crianças e adolescentes infectados pelo vírus da imunodeficiência humana (HIV), avaliar níveis séricos da vitamina e indicadores de estresse oxidativo, comparar ao grupo não infectado, correlacionar a vitamina sérica ao estresse oxidativo e associá-los segundo os valores de referência. MÉTODOS: Estudo seccional transversal comparativo. Dois grupos com 27 crianças e adolescentes cada, de 3 a 19 anos, sendo G1 infectados pelo HIV por transmissão vertical atendidos em ambulatório regional e G2 indivíduos convidados sem histórico de infecção pelo HIV. Foram pareados por sexo, idade e condição socioeconômica. Avaliou-se: antropometria (índice de massa corporal para idade), ingestão e consumo do micronutriente, análise sérica da vitamina C, proteína C reativa e albumina. RESULTADOS: Idade média encontrada de 12 anos, prevalência do sexo feminino (17 - 63%) e da classe econômica C (27 - 50%). Diagnóstico nutricional prevalente foi eutrofia em 20 (74,1%) em G1 e 21 (77,8%) em G2. O consumo de vitamina C foi significativamente maior em G1 (p = 0,006; t = 2,987) pelo método recordatório 24 horas. Houve diferença significativa da dosagem sérica de vitamina C, sendo menor em G1 (p = 0,000; t = -7,309). Em relação ao estresse oxidativo, os valores da proteína C reativa em G1 foram significativamente maiores (p = 0,007; t = 2,958). Não houve relação entre deficiência da vitamina, proteína C reativa e albumina. CONCLUSÃO: Os resultados encontrados mostram que indivíduos infectados pelo HIV apresentam baixos valores da vitamina C, não relacionados à alimentação, uma vez que o consumo desse micronutriente foi superior ao grupo comparativo, que não apresentou essa depleção. Existem peculiaridades nos indivíduos infectados que aumentam o estresse oxidativo, evidenciado pelo aumento da proteína C reativa.


OBJECTIVES: To assess adequacy of vitamin C intake in HIV-infected children and adolescents; to evaluate serum levels of vitamin C and indicators of oxidative stress; to compare with the uninfected group; to correlate serum vitamin C with oxidative stress and associate them according to the reference values. METHODS: Comparative cross-sectional study. Two groups of 27 children and adolescents each, aged between 3 to 19 years. Group 1 (G1) comprised individuals vertically infected with HIV seen at a regional outpatient clinic. Group 2 (G2) comprised invited individuals without history of HIV infection. The groups were matched for age, sex, and socioeconomic status. The following variables were analyzed: body mass index for age; micronutrient intake and consumption; and serum vitamin C, C-reactive protein (CRP), and albumin. RESULTS: The mean age was 12 years old. Most subjects were female (17, 63%), and there was prevalence of the economic class C (27, 50%). The most prevalent nutritional status was normal weight in 20 individuals (74.1%) in G1 and 21 (77.8%) in G2. The intake of vitamin C was significantly higher in G1 (p = 0.006; t = 2.987) according to the 24-hour dietary recall method. There were significant differences in serum vitamin C concentration between the groups, with a lower level in G1 (p = 0.000; t = -7.309). In relation to oxidative stress, values of CRP in G1 were significantly higher (p = 0.007; t = 2.958). There was no association between deficiency of vitamin, CRP, and albumin. CONCLUSION: Our findings show that HIV-infected individuals have low levels of vitamin C; however, this deficiency is not related to eating habits, since the intake of this nutrient was higher in this group than in the control group. HIV-infected individuals have specific characteristics that increase their oxidative stress, which is evidenced by increased CRP.

La vitamina C es una de las principales vitaminas antioxidantes. El hombre, es uno de los poco mamíferos que no puede sintetizarla orgánicamente. La vitamina C desarrolla funciones bioquímicas que incluyen su participación en la síntesis de colágeno y carnitina; funciones fisiológicas que tienen que ver con el fortalecimiento del sistema inmunológico y la regulación de la respuesta frente a reacciones alérgicas. La vitamina C está ampliamente distribuida en la naturaleza, pero su consumo puede ser sobrestimado porque las tablas de composición de alimentos no consideran las pérdidas de la vitamina producidas por la cocción.

RESUMEN

Scurvy is a vitamin C deficiency disease which mainly occurs in teenagers. Vitamin C plays a vital role in the synthesis of collagen, the deficiency of which leads to a widespread pathology of supporting tissues of blood vessels, bone and teeth. There is an increased permeability of the capillaries to red blood cells causing hemorrhage. The bone becomes friable at the growing end. There is interference with timely collagen formation which causes delayed wound healing. In addition to other symptoms, there are bleeding gums, loose teeth plus swollen and stiff joints. A case report of a 13 years old girl is presented with gingival swelling and bleeding since last 4 years. The case was thoroughly investigated, oral prophylaxis along with vitamin C was prescribed and the patient became normal within one week time

A 17-year-old girl presented with multiple perifollicular hemorrhagic lesions on legs for 2 weeks. There was history of intermittent gum bleeding for last 2 months. Clinical examination revealed follicular keratosis and broken twisted hair [corkscrew hair], in addition to hemorrhagic lesions. Perifollicular hemorrhagic lesions and corkscrew hair are virtually pathognomonic for scurvy. When specifically asked, mother revealed that the girl was on self-imposed diet with almost no citrus fruits. Based on history and clinical findings, diagnosis of scurvy was made. The response to vitamin C supplementation was very good and all the lesions disappeared within one month. Scurvy has been reported in increasing numbers in recent literature; hence, recognition of scurvy is essential to avoid many serious complications including internal bleeding and even, death

Hyperoxic ventilation induces detrimental effects on the respiratory system, and ambient oxygen may be harmful unless compensated by physiological anti-oxidants, such as vitamin C. Here we investigate the changes in electrolyte transport of airway epithelium in mice exposed to normobaric hyperoxia and in gulonolacton oxidase knock-out (gulo[-/-]) mice without vitamin C (Vit-C) supplementation. Short-circuit current (Isc) of tracheal epithelium was measured using Ussing chamber technique. After confirming amiloride-sensitive Na+ absorption (DeltaIsc,amil), cAMP-dependent Cl- secretion (DeltaIsc,forsk) was induced by forskolin. To evaluate Ca2+-dependent Cl- secretion, ATP was applied to the luminal side (DeltaIsc,ATP). In mice exposed to 98% PO2 for 36 hr, DeltaIsc,forsk decreased, DeltaIsc,amil and DeltaIsc,ATP was not affected. In gulo(-/-) mice, both DeltaIsc,forsk and DeltaIsc,ATP decreased from three weeks after Vit-C deprivation, while both were unchanged with Vit-C supplementation. At the fourth week, tissue resistance and all electrolyte transport activities were decreased. An immunofluorescence study showed that the expression of cystic fibrosis conductance regulator (CFTR) was decreased in gulo(-/-) mice, whereas the expression of KCNQ1 K+ channel was preserved. Taken together, the CFTR-mediated Cl- secretion of airway epithelium is susceptible to oxidative stress, which suggests that supplementation of the antioxidant might be beneficial for the maintenance of airway surface liquid.

Objetivo. Investigar si componentes de la inmunidad innata están involucrados en la iniciación/perpetuación de las anormalidades estructurales observadas en la capa de Bowman y el estroma superficial de la córnea de pacientes con queratopatía climática esferoidea (QCE). Materiales y métodos. En el estudio participaron 8 pacientes con QCE y 12 individuos sanos del Departamento El Cuy, Provincia de Río Negro, y 10 individuos sanos de la ciudad de Córdoba. Todos ellos, luego de firmar el consentimiento informado, recibieron un examen oftalmológico completo y se recolectaron muestras de lágrima para estudiar las concentraciones de diferentes citocinas, niveles y formas de metaloproteinasas de matriz (MMPs), y el inhibidor natural de MMPs (TIMP-1). Se realizó microscopía confocal in vivo (MCF) en algunos pacientes y controles. Biopsias de córneas provenientes de pacientes que fueron tratados con queratoplastia penetrante también fueron estudiadas mediante inmunohistoquímica (IHQ). Resultados. Los resultados de MCF indicaron claramente una progresión en la cantidad de depósitos a nivel subepitelial, a medida que la enfermedad avanza. El daño progresivo de las fibras nerviosas sub basales y estromales en los estadios 2 y 3 se correlaciona con pérdida de la sensibilidad corneal. Además de estas alteraciones, observamos que el número de células dendríticas (CD) en el limbo corneal aumentó significativamente a medida que la QCE progresa. En lágrimas de pacientes con QCE se detectaron concentraciones significativamente superiores de citocinas proinflamatorias (IL1ß e IL-8) que en individuos controles (p<0,005). No se halló IL-2, IL-17, IL-4, IL-13 ni IL-10 en pacientes y ni controles. Las actividades de gelatinasas (MMP-9 y -2) fueron significativamente mayores en QCE que en los controles (p<0,001), mientras que los niveles de TIMP-1 fueron significativamente menores en los pacientes (p<0,05). La concentración de MMP-8 fue mayor en controles pero los niveles de esta colagenasa-2 fueron 30 veces superiores, tanto en QCE como controles, con respecto a los valores de los individuos de un centro urbano. Mediante IHC observamos reactividad para MMP-9 en la mayoría de las células epiteliales, solamente en córneas con QCE. Conclusión. Demostramos un rol protagónico del eje citocinas proinflamatorias - gela-tinasas en el desarrollo de la QCE. Los altos niveles de IL-1ß e IL-8 en lágrimas de pacientes facilitan la producción de MMP-8 y gelatinasas, y los efectos de las mismas se exacerban, ya que los pacientes tienen bajos niveles de sus inhibidores naturales (TIMP-1). La MMP-9, además de degradar componentes de la matriz extracelular, cataliza la activación postranscripcional de IL-1ß, potenciando el proceso inflamatorio. Estos resultados son los primeros en explicar mecanismos inmunológicos involucrados en la etiopatogénesis de la QCE y aportan nuevas alternativas para el desarrollo de terapias preventivas utilizando inhibidores de IL-1ß y/o gelatinasas(AU)


Objective. To investigate whether components of innate immunity are involved in the initiation / perpetuation of the structural abnormalities observed in Bowman's layer and superficial stroma of the córnea of patients with Climatic droplet keratopathy (CDK). Materials and Methods. The study included 8 CDK patients and 12 healthy individuals from Department El Cuy, Province of Río Negro, and 10 healthy subjects from the city of Córdoba. All of them, after signing informed consent, received a thorough eye exam and tear samples were collected to study the concentrations of different cytokines, and levels and forms of matrix metalloproteinases (MMPs) and their natural inhibitor (TIMP-1). In vivo confocal microscopy (CFM) was performed in some patients and controls. Corneal biopsies from CDK patients treated with penetrating keratoplasty were also studied by immunohistochemistry (IHC). Results. CFM results clearly indicated a progression in the amount of deposits at corneal sub epithelial level as the disease progresses. The progressive damage in the nerve plexus in stages 2 and 3 correlated with a loss of corneal sensitivity. In addition to these alterations, we observed that the number of dendritic cells (DC) in the limbus increased significantly as the disease progresses.In tears of patients with CDK we detected significantly higher concentrations of pro-inflammatory cytokines (IL-1ß and IL-8) than in control subjects (p < 0.005). We found no IL-2, IL-17, IL-4, IL-13 and IL-10 in patients and controls. The activities of gelatinases (MMP-9 and -2) were significantly higher in CDK than in controls (p < 0.001), while TIMP-1 levels were significantly lower in patients (p < 0.05). The concentration of MMP-8 was higher in controls, but levels of this collagenase-2 were 30 times higher, both in CDK and controls, with respect to MMP-8 values of individuals inhabiting an urban area. By IHC we observed reactivity for MMP-9 in most epithelial cells only in CDK corneas. Conclussion. We demonstrated a key role of the axis pro-inflammatory cytokines ­ gelatinases in the development of CDK. High levels of IL-1ß and IL-8 in tears of patients facilitate the production of MMP-8 and gelatinases, and the effects of these molecules are exacerbated because patients have low levels of their natural inhibitors (TIMP-1). Since MMP-9 besides degrading extracellular matrix components, catalyzes the post translational activation of IL-1ß, the inflammatory process is fuelled. These results are the first to explain immunological mechanisms involved in the pathogenesis of the QCE and provide new alternatives for the development of preventive therapies using inhibitors of IL-1ß and / or gelatinases.(AU)

RESUMEN

Apesar de considerado raro, o escorbuto, doença clínica causada pela deficiência de vitamina C, ainda ocorre nos dias de hoje, inclusive em países desenvolvidos. É caracterizada por acometer pacientes com história de alcoolismo, baixo nível socioeconômico, alergia alimentar e doenças psiquiátricas. A causa principal é a insuficiente ingestão de frutas e vegetais frescos. A apresentação clínica consiste de sinais e sintomas inespecíficos. Essas manifestações podem estar associadas ao acometimento de diversos órgãos, como a pele e os sistemas hematológico, articular e ósseo, pois o ácido ascórbico constitui elemento fundamental na formação do componente essencial desses tecidos: o colágeno. O diagnóstico é essencialmente clínico, baseando-se na sintomatologia e história dietética do paciente. O tratamento é feito com reposição de vitamina C em doses que variam de 500mg a 1g por dia, com resultados satisfatórios em curto espaço de tempo. Descrevemos o caso de uma paciente com 24 anos, com manifestações típicas de deficiência de vitamina C, retratando uma apresentação clássica de escorbuto nos dias atuais


Although considered rare, scurvy, a clinical disease caused by deficiency of vitamin C, still occurs today, even in developed countries. It usually affects patients with a history of alcohol abuse, low socio-economic status, food allergy and psychiatric disorders. The main cause is insufficient intake of fresh fruit and vegetables. The clinical presentation consists of nonspecific signs and symptoms. These symptoms may be associated with the involvement of various organs such as the skin and the hematological, joint and bone systems, because ascorbic acid is a crucial element in the formation of collagen, the essential component of these tissues. The diagnosis is essentially clinical, based on symptoms and dietary history of the patient. Treatment is with supplements of vitamin C in doses ranging from 500mg to 1g per day, with satisfactory results in a short time. We report the case of a 24-year female patient with typical manifestations of vitamin C deficiency, featuring a typical presentation of scurvy today

This work aimed at an evaluation of the classical iodine method for quantification of vitamin C (L-ascorbic acid) in fruit juices, as well as at a search into the stability of this so popular vitamin under different conditions of pH, temperature and light exposition, in addition to a proposal of a new quantification method. Our results point to the persistent reversibility of the blue color of the starch-triiodide complex at the end point when using the classical iodine titration, and the overestimation of the true vitamin concentration in fruit juices. A new quantification method is proposed in order to overcome this problem. Surprising conclusions were obtained regarding the controversial stability of L-ascorbic acid toward atmospheric oxygen, at low pH, even in fruit juice and at room temperature, showing that the major problem concerned with aging of fruit juices is proliferation of microorganisms rather than expontaneous oxidation of L-ascorbic acid.


Este trabalho teve como objetivo uma avaliação do método clássico do iodo para a quantificação da vitamina C (ácido L-ascórbico) em sucos de frutas, assim como uma pesquisa da estabilidade desta vitamina tão popular sob diferentes condições de pH, temperatura e exposição à luz, além de uma proposta de novo método de quantificação. Nossos resultados indicam uma persistente reversibilidade da cor azul do complexo amido-triiodeto no ponto final, quando usamos a clássica titulação com iodo, e uma super-estimação da verdadeira concentração da vitamina em sucos de frutas. Um novo método de dosagem é proposto a fim de superar este problema. Conclusões surpreendentes foram obtidas em relação à controvertida estabilidade do ácido L-ascórbico frente ao oxigênio atmosférico, em pH baixo, mesmo no suco da fruta e em temperatura ambiente, mostrando que o maior problema em relação ao envelhecimento de sucos de frutas é a proliferação de microorganismos e não a oxidação espontânea do ácido L-ascórbico.

Scurvy has become rare in modern societies, but should be considered in malnourished persons, alcoholics, and in infants on unsupplemented milk diets who present with musculoskeletal pain or a bleeding tendency. The diagnosis of scurvy can be challenging because of the rare incidence and vague and nonspecific early symptoms. We report here a case of scurvy in a young boy who presented with an inability to walk and severe musculoskeletal pain. The diagnosis was established based on his clinical picture, radiological appearance, and low serum level of vitamin C. The patient responded well to vitamin C supplementation with full resolution of his symptoms. He regained his ability to walk and his family was happy and satisfied with the outcome of treatment. Although the incidence of scurvy has become low in Saudi Arabia, it can still occur and early recognition is important because of the excellent prognosis

RESUMEN

Hypervitaminosis and hypovitaminosis occur in children in our country mainly due to socioeconomic reasons. Most common being the rickets due to vitamin D deficiency, however scurvy is also not uncommon and radiological changes suggestive of scurvy are occasionally seen. Three cases of scurvy are presented here, between one and three years of age, showing typical radiological changes especially in knees. Good history, physical examination and quality X-rays are essential for making diagnosis of scurvy
