RESUMO

Objective@#To explore the application of septoplasty and cheiloplasty in complete unilateral cleft lip repairment, and evaluate the effect on nasal shape correction.@*Methods@#Twenty-four infants with complete unilateral cleft lip were divided into two groups: the correction group and the control group. Both groups underwent cheiloplasty by Mohler′s technique, septoplasty was performed in the correction group during the primary cheiloplasty. Six-month follow-up was taken to evaluate the nasal shape with the three dimensional images. Independent-samples t Test was performed using SPSS 21.0, to compare the nasal morphology between two groups.@*Results@#All patients healed in the first stage, and were followed for 6 months after operations. There were no complication, including nasal septum perforation, occurred in either group. There were statistically significant differences in nasal parameters between the correction group and controls (P<0.05). The columellar width, nostril width on the affected side, and columella deviation of the correction group were smaller, than those of controls. The nose tip height, columellar height and nostril height on the affected side of the correction group were greater than those of the control group.@*Conclusions@#Septoplasty associated with the primary cheiloplasty in complete unilateral cleft lip repairment, shows better results in the elongation of the nasal columella, and the correction of septum deviation, and provides a stable nasal structure.

RESUMO

Objective To assess the effectiveness of the Mulliken's method in bilateral complete cleft lip patients with 3dMD system and anthropometric landmarks. Methods Thirty-one infants with bilateral complete cleft lip received treatment in Hospital of Stomatology,Wuhan University between January 2014 and December 2016.Patients underwent primary cheiloplasty and nasoplasty by the same senior surgeon.Periodic review was taken to measure and record the 7 items in the nasolabial area with three-dimensional (3D) images. Results The labial and nasal deformities were restored after primary surgery. The upper lip and nose were corrected and obtained the symmetric shape. The columella was elongated to acquire favorable nasal tip.The arc of nasal fornix was upward. The ridge of the white lipwas continuous and integrated with full vermilion tubercle.Total length of upper lip (Sn-Sto),thickness of the vermilion tubercle (Ls-Sto),protrusion of the nasal tip (Nh) and columellar height (Ch) were markedly improved and there were no significant difference between the observation group and the control group. The significant differences between two groups occurred in values of the nostril width (Nw),which was greater than control group and white lip height (Sn-Ls) and lower than that of control group. Conclusions Mulliken's method during the primary cheiloplasty of the bilateral complete cleft lip shows better results in correction the nasal deformity and the ideal effects are achieved during follow-up.

RESUMO

Objectives To investigate the effect of different physical activities on the blood pressure in elderly patients with hypertension, and to provide evidence for the prevention and treatment of hypertension. Methods 256 elderly patients with grade 1,2 hypertension and no other complications, and 256 healthy elderly people were selected as the control subjects.According to the blood pressure and the level of physical activity, the patients were divided into hypertension combined with low physical activity group(HL group),hypertension combined with middle physical activity group(HM group),normal blood pressure combined with low physical activity group(NL group)and normal blood pressure combined with middle physical activity group (NM group). The changes of blood pressure were compared in quiet, during and after physical activity. Results The diastolic blood pressure(DBP) and systolic blood pressure(SBP) measured at rest, peak exercise and 5 minutes after exercise were significantly higher in group HL and HM group than in NL group and NM group, and the differences between groups were statistically significant(SBP:F=11.846、18.337、10.976,all P< 0.001 for all;DBP:F=13.182、11.694、11.453,all P<0.05).The DBP and SBP in HL group at rest and 5 minutes after exercise were significantly higher than those in the HM group, and the difference was statistically significant(P<0.05).The change magnitudes of DBP and SBP were significantly lower in HL group than in the HM group, and the differences were statistically significant (P < 0.05).Conclusions Taking part in an appropriate exercise can improve blood pressure level and a recovery capability of post-exercise blood pressure in elderly patients with hypertension.

RESUMO

Objective To discuss the correlation of high homocysteine (Hcy) hypertension (Htype hypertension) with serum levels of uric acid (UA) and fibrinogen (FIB) in the elderly. Methods A total of 188 elderly in-and out-patients with essential hypertension were selected in our heart medicine outpatient and ward from April 2013 to April 2014.They were divided into H type hypertension group (n =136,Hey value≥ 10μmol/L) and primary hypertension group (n =52,Hcy value<10μmol/L).Another 160 elderly individuals without hypertension taking physical examination in our hospital during same period served as controls.General information were colleted, and the serum Hcy,UA and FIB were detected in all the patients to compare the differences among different groups.Logistic regression analysis was adopted to seek affecting factors of H-type hypertension patients. Results Compared with control group, there were statistical increases in the levels of UA and FIB for primary hypertension group and H-type hypertension group(P<0.01).Compared with primary hypertension group, there were statistical differences in the level of UA and FIB(P<0.01).The influence factors analysis showed that H-type hypertension was positively correlated with the serum level of UA and FIB(P<0.01).Conclusions H-type hypertension is closely associated with UA and FIB.It's good for management of hypertension patients to monitor UA and FIB levels.

RESUMO

Objective To study the diagnostic value of homocysteine (Hcy) in coronary heart disease(CHD) .Methods 102 CHD patients were collected as CHD group [15 cases of acute myocardial infarction (AMI) ,56 cases of unstable angina pectoris (UAP) and 31 cases of table angina pectoris (SAP)] and 100 healthy subjects were collected as control group .The level of Hcy was measured by enzymatic cycling assay and the levels of folic acid (FA) and vitamin B12 (VB12 ) were measured by microparticle chemoluminescence immunoassay .Results The Hcy levels of patients with AMI ,UAP and SAP were significantly higher than con-trol group (P<0 .01) .The levels of AMI group ,UAP group and SAP group were (24 .12 ± 5 .87)μmol/L ,(22 .56 ± 6 .13)μmol/L and (17 .42 ± 5 .23)μmol/L ,respectively .Not only in CHD group ,but also in control group ,the level of Hcy in men was signifi-cantly higher than that in women .Similarly ,the level of Hcy were significantly higher in current smokers compared with non-smok-ers as well as in older group compared with young group .There were significant negative correlations between Hcy and FA (r= -0 .62 ,P<0 .01) and VB12 (r= -0 .47 ,P<0 .01) .Conclusion Hyperhomocysteinemia is significantly associated with CHD .Hcy may be an marker for early warning CHD .

RESUMO

OBJECTIVEThis study was conducted to observe dynamically the serum and stool level of IL-2, IL-6 and IFN-alpha with RV enteritis, and to analyse the relation of them and their relation to clinical manifestations.

METHODSThe double-antibody sandwich ELISA technique was applied.

RESULTSThe positive rate of RV in 119 enteritis cases was 63.0% and the peak month was November. These cytokines had different peak phases in the course of disease. The earliest cytokine which had peak phase was IFN-alpha, the last ones were serum and stool IL-2. There was a negative relation between stool IL-2 and IL-6 (P<0.01). There were positive relation between IL-2 and IFN-alpha, stool IL-6 and IFN-alpha, serum IL-2 and IL-6, stool IL-2 and the recovery time of the enteritis (P<0.01 or P<0.05). Serum and stool level of IL-2 and IFN-alpha had positive relation to the number of lymphocyte plus mononuclear cells (P<0.01 or P<0.05).

CONCLUSIONSIL-2, IL-6 and IFN-alpha play important roles of immunoregulation in RV enteritis, and have close relation to clinical manifestations.
