RESUMO

Objective:To evaluate the effect of melatonin preconditioning on hepatic ischemia-reperfusion (I/R) injury in rats with non-alcoholic fatty liver disease (NAFLD).Methods:Forty-eight SPF male Sprague-Dawley rats, aged 10-12 weeks, weighing 200-230 g, were divided into 4 groups ( n=12 each) using a random number table method: control group (Con group), NAFLD group, NAFLD + hepatic I/R group (NAFLD+ HIR group), and NAFLD + hepatic I/R + melatonin treatment group (NAFLD+ HIR+ MT group). The NAFLD model was developed by a high-fat and high-glucose diet (10% glucose, 10% fat) for 8 consecutive weeks in NAFLD, NAFLD+ HIR and NAFLD+ HIR+ M groups, and rats were fed with common chow and freely drank water in the other groups.Melatonin 10 mg/kg was given intragastrically daily for 2 consecutive weeks before developing the model in group NAFLD+ HIR+ MT.The model of liver I/R injury was developed by clipping the hepatic artery and portal vein for 20 min, opening for 5 min, and re-clamping for 20 min followed by restoration of perfusion.Blood samples from inferior vena cava were collected and liver tissues were obtained at 6 h of reperfusion to detect serum levels of insulin, blood glucose, free fatty acid (FFA), triglyceride (TG), alanine aminotransferase (ALT), aspartate amino transferase (AST) and ferritin, and insulin resistance index was calculated.The levels of superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), reactive oxygen species (ROS) and Fe 2+ in liver tissues were detected by enzyme-linked immunosorbent assay, the pathological changes of liver tissues were examined with a light microscope after hematoxylin-eosin staining.The expression of nuclear factor E2-related factor 2 (Nrf2), lysophosphatidylcholine acyltransferase 3 (LPCAT3), long-chain fatty acyl-CoA synthase 4 (ACSL4) and glutathione peptide peroxidase 4 (GPX4) was detected by Western blot. Results:Compared with Con group, the levels of serum FFA, TG, ALT, AST and ferritin and insulin resistance index were significantly increased, the levels of ROS and Fe 2+ in liver tissues were increased, the levels of GSH-Px and SOD were decreased, the expression of ACSL4 and LPCAT3 was up-regulated, and the expression of Nrf2 and GPX4 was down-regulated in NAFLD group ( P<0.05). Compared with NAFLD group, the serum levels of FFA, TG, ALT, AST and ferritin and insulin resistance index were significantly increased, the levels of ROS and Fe 2+ were decreased, the levels of GSH-Px and SOD were increased, the expression of ACSL4 and LPCAT3 was up-regulated, and the expression of Nrf2 and GPX4 was down-regulated in NAFLD+ HIR group ( P<0.05). Compared with NAFLD+ HIR group, the serum levels of FFA, TG, ALT, AST and ferritin and insulin resistance index were significantly increased, the levels of ROS and Fe 2+ were decreased, the levels of GSH-Px and SOD were increased, the expression of ACSL4 and LPCAT3 was down-regulated, and the expression of Nrf2 and GPX4 was up-regulated in NAFLD+ HIR+ MT group ( P<0.05). Conclusions:Melatonin preconditioning can alleviate hepatic I/R injury in rats with NAFLD, and the mechanism may be related to activation of Nrf2 signaling pathway, reduction of lipid peroxidation and inhibition of ferroptosis.

Objective:To evaluate the differences in organ dysfunction between gram-positive and gram-negative bacteria-induced sepsis in rats.Methods:Fifty-two adult male Sprague-Dawley rats, weighing 340-380 g, aged 15-18 weeks, were divided into 3 groups using a random number table method: control group (C group, n=12), G - bacteria group ( n=20), and G + bacteria group ( n=20). The G + and G - septic models were developed by intraperitoneal injection of inactivated Staphylococcus aureus and Escherichia coli suspension in G + and G - bacteria groups, respectively.The survival status within 36 h after injection of inactivated bacteria was recorded.Mean arterial pressure (MAP), platelet count (Plt), oxygenation index (OI) and serum concentrations of tumor necrosis factor α (TNF-α), cardiac troponin T (cTnT), total bilirubin (TBIL), creatinine (Cr), neuron-specific enolase (NSE) and lactic acid (Lac) were measured at 6, 12, 24 and 36 h after injection of inactivated bacteria.Fear conditioning test and open field test were performed at 12 and 36 h after injection of inactivated bacteria.The apoptosis rate of neurons in hippocampal CA1 area was measured by TUNEL, and the permeability of blood-brain barrier was measured by Evans blue method.The histopathological changes of lung, heart, kidney and brain tissues were examined at 36 h after injection of inactivated bacteria. Results:Compared with C group, the number of crossing grids and percentage of time spent freezing were significantly decreased, the apoptosis rate of neurons and permeability of blood-brain barrier were increased, and the survival rate was decreased in G - bacteria group and G + bacteria group ( P<0.05); the serum TNF-α concentration was significantly increased in G + bacteria group, and the serum cocentration of OI was significantly decreased, and the serum concentrations of cTnT, Cr, TNF-α and Lac were increased in G - bacteria group at 6 h after injection of inactivated bacteria ( P<0.05); the serum concentrations of cTnT, Cr, NSE, TNF-α and Lac in G + bacteria group and serum concentrations of cTnT, Cr, TNF-α and Lac in G - bacteria group were increased, and the OI, MAP and Plt were decreased at 12 h after injection of inactivated bacteria in both groups ( P<0.05); the serum concentrations of cTnT, TBIL, Cr, NSE, TNF-α and Lac were increased, and OI, MAP and Plt were decreased at 24 and 36 h after injection of inactivated bacteria in G + bacteria group and G - bacteria group ( P<0.05). Compared with G + bacteria group, the serum concentrations of TNF-α and cTnT and apoptosis rate of neurons were significantly decreased at 12 h after injection ( P<0.05); the serum concentrations of TNF-α and Lac were significantly increased, the serum concentrations of cTnT, OI, MAP and Plt were decreased at 24 h after injection of inactivated bacteria ( P<0.05); the serum concentrations of Cr, NSE, TNF-α and Lac were significantly increased, OI, MAP and Plt were decreased, and the percentage of time spent freezing, apoptosis rate of neurons and permeability of blood-brain barrier were increased at 36 h after injection of inactivated bacteria in G - bacteria group ( P<0.05). Conclusions:Early respiratory dysfunction and multiple organ failure often occur in G - bacteria sepsis, while G + bacteria sepsis is more likely to cause early circulatory and neurological dysfunction, and G - sepsis presents with more serious organ damage and high fatality rate as the disease progresses.

Objective:To explore the safety and feasibility of the concept of enhanced recovery after surgery in the removal of catheters at different stages after laparoscopic gastric cancer surgery.Methods:A total of 219 patients who underwent laparoscopic radical gastrectomy in Harbin Medical University Cancer Hospital from December 2017 to January 2019 were selected as the research subjects. All patients were divided into groups A, B, and C according to the random number table with 73 cases each. One case in each group fell off, and 72 cases in each group finished the study. In group A, the ureter was removed 12 hours after operation; in group B, the ureter was removed 24 hours after operation; in group C, the ureter was removed 48 hours after operation. The urination-related indicators and rehabilitation indicators among the three groups of patients were compared.Results:There was no significant difference in the number of cases of urinary urination, the number of urinary retention and reinsertion cases, the number of urinary tract irritation, the number of urinary routine abnormalities, and the urination pain score after the first urinary extubation ( P> 0.05). The patients in group A got out of bed activity time, first ventilation time, postoperative hospital stays, total hospital stays, total hospitalization costs were (10.26 ± 4.51) h, (28.74 ± 8.04) h, (4.94 ± 1.73) d, (6.68 ± 1.93) d, (7.19 ± 0.31) ten thousand yuan, group B were (16.37 ± 5.13) h, (39.16 ± 11.52) h, (5.27 ± 1.97) d, (7.83 ± 1.88) d, (7.51 ± 0.36) ten thousand yuan, and group C were (24.19 ± 5.77) h, (54.37 ± 17.49) h, (6.48 ± 1.73) d, (8.16 ± 1.81) d, (7.98 ±0.42) ten thousand yuan. There were statistically significant differences among the three groups ( F values were 12.376 - 131.721, all P <0.01). Conclusions:It is safe and feasible to remove the catheter early after laparoscopic gastric cancer surgery under the concept of enhanced recovery after surgery. It can promote the early recovery of patients, reduce hospitalization time and reduce the overall economic burden of patients.

Objective To test the effect of the cluster management strategy in the hospital of ICU patients, in order to ensure the safety of the transhipment of the ICU patients. Methods From February 2016 to November 2017, 351 cases of ICU patients in a three class first class cancer hospital were selected as the subjects, including 182 cases in the control group from February 2016 to December 2016 and 169 cases in the experimental group from January 2017 to November 2017. The patients in the control group carried out conventional traditional transport management measures, and the patients in the experimental group were transported by cluster management strategy. The incidence of adverse events in the hospital transhipment was observed and compared. Results The test group received the cluster management strategy intervention ICU hospital transport of patients with adverse events (associated with disease, correlation, and transfer of personnel information, poor communication pipeline displacement, arteriovenous catheter prolapse, monitor lead loose, monitor micro pump power supply, and related equipment) rates were 15.38%(26/169), 9.47%(16/169), 3.55%(6/169), 2.96%(5/169), 2.37%(4/169), 2.37%(4/169), 2.96%(5/169), 5.92%(10/169), significantly lower than the control group were 29.12%(53/182), 26.92%(49/182), 10.44%(19/182), 7.69%(14/182), 7.14%(13/182), 7.69% (14/182), 11.54% (21/182), 20.33% (37/182), the difference was statistically significant (χ2=3.835-17.695, P<0.01 or 0.05). Conclusion Cluster management strategy can improve the safety of ICU patients in hospital transport, and suggest extensive clinical promotion.

RESUMO

Pterostilbene (3,5-dimethoxy-4'-hydroxystilbene) is a polyphenolic compound primarily found in blueberries, grapes, and a tree wood, pterocarpus marsupium. Studies demonstrate that pterostilbene inhibits a variety of cancers, such as lung, breast, stomach, colon, etc. The anti-cancer activities are related to the regulation of several hallmarks of cancer. Moreover, pterostilbene exhibits much greater bioavailability and bioactivity than resveratrol which warrants further investigation in the anti-cancer functions and mechanisms.

AIM:To investigate the effect of Shenci capsule combined with cisplatin ( DDP) in reversing DDP resistance by PI3K/AKT/mTOR signaling pathway in nude mice bearing A 549/DDP tumor.METHODS:The patient-de-rived lung adenocarcinoma A 549/DDP cell xenograft model was established .The tumor-bearing nude mice were randomly divided into control group , Shenci capsule group , DDP group and Shenci capsule combined with DDP group .The mice in control group was treated with normal saline , while the mice in other groups were treated with different drugs for 21 d.After treatment, the mice were killed and lung cancer tissues were collected .Flow cytometry was used to analyze the cell cycle and apoptosis.FQ-PCR was used to determined the mRNA levels of PTEN , P-glycoprotein, PI3K, AKT and mTOR in A549/DDP lung tumor .RESULTS:Compared with control group , the cell proliferation in all the drug treatment groups was inhibited .Compared with other drug treatment groups , Shenci capsule combined with DDP blocked the cell cycle of A 549/DDP cells at G2/M phase, promoted the apoptosis rate , increased the mRNA expression of PTEN and inhibited the mRNA expression of P-glycoprotein, PI3K, AKT and mTOR.CONCLUSION:Shenci capsule increases the sensitivity of A 549/DDP resistant cells in nude mice to DDP by blocking PI 3K/AKT/mTOR signaling pathway , increasing the expression of PTEN or inhibiting P-glycoprotein-mediated resistance pathway .

Discoidin domain receptor 2 (DDR2), a tyrosine kinase receptor belonging to DDRs family, is associated with progression of various cancers. Studies have indicated that DDR2 expression is upregulated in various cancer tissues, which could promote tumors proliferation, invasion, and metastases. Furthermore, patients with high DDR2 expression showed poor 5-year overall survival rate. In addition, DDR2 point mutation can enhance the sensitivity of tumor cells to small molecule tyrosine kinase inhibitor dasatinib, which suggests that DDR2 mutation may be a promising target for cancer therapy.

Objective To observe Shenci capsule's effect on immunologic function and hematological tox-icity of patients with advanced non-small cell lung cancer.Methods 60 cases of patients with advanced non-small cell lung cancer were chosen and divided into the therapy group and the control group randomly with 30 cases in each group respectively. While patients in the control group were administered simple GP chemotherapy regime, patients in the therapy group were administered Shenci capsule treatment orally in addition to the therapy in the control group.Comparisons were made of the variations in the subgroup of T lymphocyte(including the absolute val-ue of CD3+cell,the absolute value of CD4+cell,the absolute value of CD8+cell and CD4+/CD8+ratio)of hematology before and after chemotherapy between the two groups of patients. Results No significant discrepancy was found in the comparison of the absolute value of CD3+cell,the absolute value of CD4+cell,the absolute value of CD8+cell and CD4+/CD8+ratio before the chemotherapy between the two groups(P > 0.05)and the absolute value of CD3+cell,and the absolute value of CD4+cell of both groups decreased significantly after the chemotherapy(P<0.05), especially in the control group(P < 0.05). The hematological toxicity of the therapy group was lower than that in the control group(P < 0.05)after the chemotherapy. Conclusion Shenci capsule was capable of enhancing the body immunity in advanced non-small cell lung cancer and alleviating hematological toxicity.

Objective:To analyze survival in patients with advanced oral cancer from prospective clinical trials. Methods:From 2008 to 2010, 256 patients with oral cancer at clinical stage III/IVA were randomly categorized into two groups. Patients in the experi-mental group received neo-adjuvant chemotherapy, surgery, and post-operative radiation, and patients in the control group underwent surgery and post-operative radiation. All patients were routinely followed-up after treatments. Survival was analyzed using Kaplan–Meier method and log-rank test, and differences were considered statistically significant at P value lower than 0.05. Results: Each group was composed of 128 patients. With the median follow-up period of 60 months, the 5-year overall survival rate was 61.7%and the disease-free survival rate was 53.9%. The overall survival rate (P=0.350) and the disease-free survival rate (P=0.160) were not sig-nificantly different between the experimental and control groups. Patients with positive pathological response to neo-adjuvant chemo-therapy exhibited significantly improved overall survival (P<0.05). Conclusion:Radical surgery should be emphasized to improve the prognosis of oral cancer. Functional reconstruction could also improve the quality of life and survival of patients. Despite that neo-adju-vant chemotherapy could not improve the survival of patients with advanced oral cancer in entirety, it could benefit patients exhibiting positive treatment responses.

Objective:To analyze the clinical manifestations, radiographic features, and pathological classification of the juvenile ossifying fibroma (JOF) of the jaws and discuss its clinical management and prognosis. Methods: From January 2005 to December 2014, 15 patients with JOF who underwent surgery were retrospectively investigated with regard to clinical and radiologic data. On the basis of the standards of the World Health Organization in 2005, JOF was divided into juvenile psammomatoid ossifying fibroma (JPOF) and juvenile trabecular ossifying fibroma (JTOF). Results:Among the 15 patients, 10 were female and 5 were male. Patient age ranged from 7 years old to 18 years old with a mean of 10.93 years old. Nine cases were located in the mandible and 6 in the maxilla. The clinical manifestation was painless swelling of the jaw, but 20%of the cases showed jaw swelling with pain. Various JOF radiolog-ic appearances, such as radiolucent, mixed radiopaque-radiolucent, or ground-glass pattern, were observed. Ten of the 15 patients were JTOF and 5 were JPOF With regard to treatment, 4 patients underwent conservative surgery, 3 patients lived with tumors, and 11 pa-tients underwent radical surgery during the follow-up period; no lesion recurrence occurred. Nine patients underwent reconstruction, that is, 5 cases with fibula flap graft, 3 cases with free iliac graft, and 1 case with costal cartilage graft. Conclusion:JOF is a rare form of benign fibro-osseous lesions and occurs in adolescents. Mandible and maxilla are two of the most common locations. Early diagnosis and treatment and strict clinical and radiological follow-up is important in the clinic because of the aggressiveness and high recurrence rate of JOF. Operation time and treatment options should be selected according to the patients' specific situation.

Many college students’ English is not good enough to meet the need of the society. Pairing general English with ESP is proposed so as to solve the problem. Taking college English teaching in schools of traditional Chinese medicine as a case, this paper analyzes the key factors such as teaching materials, language teachers, students, teaching objectives, methods and environment in the teaching process in order to prove that pairing general English with ESP is feasible.

OBJECTIVE To investigation central venous catheter-related infections(CRI) in critical patients. METHODS Fifty-nine cases from Jul 2006 to Jun 2007 in intensive care unit (ICU) of our hospital were analysed retrospectively. RESULTS Among 59 cases, 27 cases were with positive results of fungi (51.85%) and bacteria(48.15%) in which G+(25.93%) and G-(22.22%). CONCLUSIONS CRI is related to the indwelling time, fungi are the major pathogens. It is important to prevent CRI through careful nursing. We must pull out the catheters if recessary and antibiotics should be used reasonablly.

RESUMO

5cm hepatic cancer, the effective rate (CR+PR) was 60 8% (62/102), no change (NC) rate 39 2% (40/102), and the 1-, 2-, 3-year survival rates 67 6% (69/102), 49 1% (50/102) and 39 2% (40/102), respectively. The effective rates (CR+PR) in patients with bone tumor and lung cancer were 100%(12/12) and 60%(3/5), respectively. There were no other severe complications found. Conclusions Ultrasound-guided microwave intervention is a safe, effective and feasible method for hepatic cancer, lung cancer and bone tumor.

AIM:To observe the effect of TNF-? and Yigu capsule drug-containing sera on osteoblasts apoptosis in osteoblasts-osteoclasts coculture system.METHODS:(1)Twenty female Sprague-Dawley rats,10 months old,were randomly assigned into 2 groups,NS and Yigu capsule groups,to prepare blank sera and drug-containing sera.(2)The DNA gel electrophoresis method was used to detect apoptosis of osteoblasts treated with different concentration of TNF-? in order to determine the best dosage in the co-culture system.(3)The cells were divided into four groups,TNF-? group,normal group,TNF-? + blank serum group and TNF-? + drug-containing serum group.DNA gel electrophoresis,acridine orange staining and flow cytometry were used to observe osteoblast apoptosis in these groups.RESULTS:(1)After induced by TNF-? for 72 h at a concentration of 60 ?g/L,relatively typical DNA ladder appeared in TNF-? group.(2)Only two DNA brands appeared and most of cells were well-proportioned stained in TNF-? + drug-containing serum group,the rate of osteoblasts apoptosis in TNF-? + drug-containing serum group(9.60%?0.26%)was obviously lower than that in the TNF-? group(26.90%?0.06%)and TNF-? + blank serum group(18.10%?0.06%).CONCLUSION:TNF-? induces osteoblast apoptosis in the co-culture system,and Yigu capsule drug-containing serum prevents osteoblast apoptosis induced by TNF-?.

AIM: To investigate the effect of yigu capsule drug-containing serum on proliferation, ALP activity and expression of interleukin-11 mRNA in Sprague-Dawley rat osteobalsts in the co-culture system. METHODS: (1) Osteoblasts and osteoclasts were isolated from 1 and 5-day-old Sprague-Dawley rats, respectively. The osteoblasts-osteoclasts co-culture system was built to prevent the two kinds of cells from contact and allow the media to exchange. The experiment included two groups, drug-containing sera group and control group. (2) The osteoblasts proliferation, ALP activity and expression of interleukin-11 mRNA were detected by the MTT, 4-aminoantipyrine spectrometric methods and FQ-PCR,respectively. AIM: To study the effect of Bone-benefiting capsule(BBC)-containing serum on activity and apoptosis of osteoclasts(OC) in osteoblasts(OB) and osteoclasts co-culture system.METHODS:(1) OB were separated from the skull of SD rats aged 1 d and OC from the thighbone and shinbone of SD rats aged 5 d,the plane OB and OC co-culture system was established,in which the cells in each culture system did not intermix,but the culture medium communicated,and BBC-containing serum group and control group were concluded in the experiment.(2) Female SD rats aged 10 months were divided into BBC group and control group to prepare BBC-containing serum and control serum.(3) The TRAP secreted by OC was measured by the diazol method and the absorption of lacuna formed by OC was observed by microscope in OB and OC co-culture system to determine the activity of OC with the effect of BBC-containing serum.(4) The effect of BBC-containing serum on OC apoptosis in OB and OC co-culture system was examined by optical microscopes and fluorescence microscopes.RESULTS: In OB and OC co-culture system,the activity of TRAP secreted by OC of BBC-containing serum group were obviously lower than that of control group in 48 h,72 h and 96 h,the OC surviving numbers were obviously lower than that of control group and the rate of OC apoptosis was markedly higher than that of control group which showed an obvious time-efficiency relationship;the amount of the absorption of lacuna formed by OC in BBC-containing serum group was evidently lower than that of control group(P
