Thyroid cancer in child [about 7 cases]

We report here a study of 7 cases of thyroid cancer in childhood. This study that we reinforced by literature review aims to point out the different aspects of this affection: clinical features, the complementary examinations and the management to diagnoses it with certainty and lastly, the different therapeutical attitudes. It is a study of 7 children between 8 and 15 years aged, with a female daminance, without pearicular previous medical histories. Clinically, these children present an euthyroidal goitre, cervical adenopathies with conservation of good general health condition. Scintigraphy showed a cold nodule that was proved to be neoplastic at the anatomo-pathological study, it was papillar or papillo-vesicular carcinoma. The surgical management has at the present time a tendancy to conservative surgery combined or not with a ganglionic curage, followed by a substitute hormonal treatment. In general, the prognosis of this affection is good for the differentiated forms whereas it remains has for bad anaplastic non differentiated forms

Intervention de Duhamel dans le traitement de la maladie de Hirschsprung chez l'enfant [a propos de 30 cas]

30 childs with Hirschsprung's disease were treated by Duhamel procedure in the surgical pediatric departement "A" in Rabat. The experience has shown that this operation is more adapted at the old child than Soave technique. Otherwise it's a benign procedure, its execution is easily accompleshed. The immediate complications are unnoticed. Late functional results are satisfying. We have remarked 4 stenosis of the colorectal anastomosis. The results can be neatly improved using some modifications of the original technique

Traitement non operatoire du kyste hydatique du poumon rompu dans les bronches chez l'enfant [a propos de 115 cas]

The authors report 115 cases of the hydatid cyst broken in bronchi treated by non operative procedure combining antibiotherapy, kinesitherapy and bronchoaspiration. This attitude, based on clinical radiological and operative arguments, has given 72% of good results in patients controlled, and so avoid post operative complications of the broken cyst. 27 patients were n't seen again. 24 children may be treated surgically on account of faillure of the non operative management or complications

Psoas abscess in infant About four cases

The studies of the four primitive psoas abcess in the child's cases conclude the insistnce on certain points. 1] Entity of rare observation and etiopathogenic m‚canisms still unknown, conserns in more than 50% of children. 2] The diagnostis of the abcess is showed by the association of fever, of the general state, of a varied pain of the inferior limb. A case of anterolateral congenital diaphragmatic hernia is described in an eight days new born; after an embryologic study, the authors remind the rareness of this type of defect which is different from Morgagni diaphragmatic hernia by its size and its revelation in child. Treatement allows operative repair and prognosis depends on the presence and severity of associated malformations

Anorectal sphincterotomy in constipation management in infant

In the period 1980 and 1991 incluse, 37 children have been treated in surgical pediatric d‚partement "A" for rebellous constipation by ano-rectal sphinctero-myectomy. The analytical study of cases and the review of the literature allows to insist on the diagnostic role of this method precising the prelevement length and the histological sections level to have an intepretable result. As to its therapeutic value, the bad results appear have as a cause a persistant and important colorectal ectasie which is secondary at the oldness of constipation or at its bad medical treatment quality. The provisional colostomy appears permetting at the ectasia colorectum to take back its normal calibre and tonicity, therefore prometting efficient results
