RESUMO

For successful restoration of maxillary anterior implants, both pink esthetics and white esthetics must be satisfied. For pink esthetic part, the role of appropriate provisional prosthesis restoration is important, and for white esthetic part, the color and shape of the definitive prosthesis is important. Multilayer zirconia can be used for natural tooth appearance due to the higher transparency of the incisal area compared to the conventional monolithic zirconia. Therefore, in this case, white esthetic part was achieved through multilayer zirconia after recovering function and esthetics through appropriate provisional prosthesis in a patient who lost the maxillary anterior teeth.

Objectives@#A cross-sectional study was conducted to investigate the associations between food groups and hearing loss. @*Methods@#Data of 1,312 individuals were used from the Korea National Health and Nutrition Examination Survey 2013. Hearing loss was determined with a pure tone average (PTA) of greater than 25 dB in either ear. The PTA was measured as the average hearing threshold at speech frequencies of 0.5, 1, 2, and 4 kHz. The dietary intake was examined with a food frequency questionnaire with 112 food items. The food items were classified into 25 food groups. A weighted logistic regression was used to investigate the association. @*Results@#Individuals in the highest tertile of vegetables and nuts food groups were less likely to have hearing loss than those in the lowest tertile [Odds Ratio (OR) = 0.58 (95% Confidence interval (CI) 0.38-0.91), P = 0.019; OR = 0.59 (95% CI 0.39-0.90), P = 0.020, respectively], after adjusting for confounding variables of age, sex, body mass index, drinking, smoking, diabetes, hypertension, and physical activity. @*Conclusions@#In this cross-sectional study, we observed that high intake of vegetables and nuts food groups revealed significant inverse associations with hearing loss, after adjusting for confounding variables among 1,312 participants.

Progressive familial intrahepatic cholestasis (PFIC) is a group of severe genetic disorders, inherited in an autosomal recessive manner, causing cholestasis of hepatocellular origin, later progressing to biliary cirrhosis and liver failure. This is the first report of PFIC type 1 with novel compound heterozygous mutations in Korea. The patient was presented with intrahepatic cholestasis, a normal level of serum γ-glutamyl transferase, steatorrhea, and growth failure. Genetic testing of this patient revealed novel compound heterozygous mutations (p.Glu585Ter and p.Leu749Pro) in the ATP8B1 gene. After a liver transplantation at age 19 months, the patient developed severe post-transplant steatohepatitis.

PURPOSE: To evaluate the outcomes of a hybrid prophylactic strategy to prevent cytomegalovirus (CMV) disease in pediatric liver transplantation (LT) patients. METHODS: CMV DNAemia was regularly monitored by quantitative nucleic acid amplification test (QNAT) and was quantified in all children. CMV infection and disease were defined according to the International Consensus Guidelines. The hybrid strategy against CMV infection consisted of universal 3-week prophylaxis and preemptive treatment of intravenous ganciclovir regardless of the recipient's serostatus. RESULTS: A total of 143 children who underwent living donor LT were managed using the hybrid strategy. The overall incidence of CMV infection by QNAT was 48.3% (n=69/143). The highest CMV DNAemia positivity was observed in 49.2% (n=60/122) of children in the D+/R+ group, followed by 46.7% (n=7/15) in the D+/R− group. CMV disease was noted in 26.1% (n=18/69) patients. Forty-three (62.3%) children had undergone preemptive therapy consisting of intravenous ganciclovir. No symptomatic patients developed tissue-invasive disease, resulting in no CMV-associated mortality. CONCLUSION: The incidence of CMV infection was high in pediatric LT patients despite the hybrid strategy. However, tissue-invasive disease in pediatric LT did not occur.

PURPOSE: Studies on the efficacy of infliximab (IFX) in a large population of pediatric patients with Crohn's disease (CD) are limited, and prognostic factors are not well-known. The aim of this study was to evaluate outcomes of IFX in pediatric patients with CD and to identify factors associated with poor prognosis. METHODS: We retrospectively analyzed medical data of 594 pediatric patients with CD between 1987 and 2013 in a tertiary center. Of these, 156 children treated with IFX were enrolled and were followed up for at least a year with intact data. Outcomes of induction and maintenance, classified as failure or clinical response, were evaluated on the tenth and 54th week of IFX therapy. RESULTS: We treated 156 pediatric patients with CD with IFX, and the median duration of IFX therapy was 47 months. For IFX induction therapy, 134 (85.9%) patients experienced clinical response on the 10th week. Among the 134 patients who showed response to induction, 111 (82.8%) patients maintained the clinical response on the 54th week. In multivariate analysis, low hematocrit (p=0.046) at the time of IFX initiation was associated with the failure of IFX induction. For IFX maintenance therapy, longer duration from the initial diagnosis to IFX therapy (p=0.017) was associated with maintenance failure on the 54th week. CONCLUSION: We have shown the acceptable outcomes of IFX in a large cohort of pediatric CD patients in Korea. Hematocrit and early introduction of IFX may be prognostic factors for the outcomes of IFX.

OBJECTIVES: The study aimed to examine whether healthy diet score was associated with stress and social support among 472 Chinese college students in Korea. METHODS: The study participants were 472 (187 male, 285 female) Chinese college students in Gyeong-gi area. From April 2013 to Oct 2013, participants were asked to fill out questionnaires on healthy diet score (20 questions), stress (20 questions), and social support (20 questions). Each question was scored by a 5-point Likert scale (total scores of each questionnaire were ranged from 20 to 100). Questions on healthy diet were sub-categorized as 'Healthy food eating (HFE)', 'Healthy eating habits (HEH)', and 'Avoidance of unhealthy food (AUF)'. Reliability test was conducted with Cronbach's alphalpha (alpha=0.79). RESULTS: Healthy diet score was higher in participants who stayed longer in Korea, who spoke Korean language fluently, and who assessed his or her own health status as very good. Adjusted means of healthy diet scores were estimated after adjusting for age, gender, body mass index, duration of staying, and Korean language fluency. According to tertile categories, participants with low tertile stress but high tertile social support showed the highest score of healthy diet (72.59+/-1.45), whereas participants with high tertile of stress but low tertile of social support had the lowest score of healthy diet (59.22+/-1.54). As for the three sub-categories of healthy diet score, the score of HFE increased as the score of social support increased. CONCLUSIONS: Our findings suggested that social support system is beneficial to alleviate stress and to improve healthy diet score.

OBJECTIVES: While recent studies showed that coffee consumption reduced the risk of all-cause mortality, no study has examined the effect of coffee consumption on all-cause mortality related to sleep disorders. We aimed to examine whether sleep-related disorders would differently affect the association between coffee consumption and the risk of all-cause mortality among 8,075 adults aged 40 to 69 years. METHODS: In a prospective cohort study, the study participants were biennially followed up for 12 years from 2001 to 2012. On each follow-up visit, the participants underwent comprehensive tests including anthropometric examinations, interviewer-administered questionnaires, and biochemical tests. Coffee consumption frequency and the amount were measured using a semi-quantitative food frequency questionnaire. Using death certificate data from Korean National Statistical Office, the vital status of each study participant was identified. Sleep-related disorders were examined with interviewer-administered questionnaires. We estimated Hazard ratios and the corresponding 95% confidence intervals from Cox Proportional Hazard models. Multivariable models were established after adjusting for center, total caloric intake, age, gender, body mass index, physical activity, education, smoking, drinking, hypertension, diabetes, total cholesterol, c-reactive protein, energy-adjusted food groups of refined grains, vegetables, fruits, meat, fish, and dairy. RESULTS: Compared with those who had no coffee consumption, participants who had about three cups of coffee per day showed a reduced risk of all-cause mortality, after adjusting for covariates. Those who had a sleep-related disorder showed no significant effect of coffee consumption on the risk of all-cause mortality, whereas those who had no sleep-related disorders showed significantly reduced risk of all-cause mortality. CONCLUSIONS: Our findings suggested that approximately three cups of coffee per day would be beneficial to reduce the risk of all-cause mortality only among adults with no sleep-related disorders. Coffee consumption should be prudent for those with sleep-related symptoms.

OBJECTIVES: Number of previous studies reported poor treatment outcome of longer duration of untreated psychosis (DUP). The purpose of this study was to determine whether DUP was associated with clinical and cognitive variables in patients with schizophrenia in Korea. METHODS: The data were collected from forty-five patients with DSM-IV schizophrenia who were hospitalizaed. Demographic data, psychiatric family history, insight, duration of first hospitalization, age of onset, DUP, cognitive function, and premorbid functioning were assessed. RESULTS: The mean DUP was 23.7 months. Female subjects showed longer DUP (p<0.007) independent of age of onset. No significant correlations were observed between DUP and clinical and cognitive variables except the highest Global Assessment Functioning score in past year (r=-0.35, p=0.02). CONCLUSION: These findings do not support the hypothesis of a psychotoxic effect of prolonged period of untreated psychosis.
