RESUMO

Objective: To investigate the clinical characteristics of inpatients with the indication of cardiac implantable electronic devices (CIED) therapy and combined acute pulmonary thromboembolism (APTE). Methods: We retrospectively screened 8 641 inpatients who admitted with the indication of CIED implantation in Fuwai Hospital from January 2014 to May 2019. The clinical characteristics, management strategies and clinical outcome were analyzed for patients diagnosed as APTE. Results: APTE were identified in 45 (5‰) patients in this cohort, there were 18(40%) male patients, the average age was (73±8) years old and body mass index was (27±10) kg/m2.Thirty-two (70%) patients were at intermediate-risk and 13 (30%) at low-risk. Anti-coagulation therapy was initiated in 38(84%) patients, and 30 patients underwent CIED implantation (27 pacemaker, 2 CRT and 1 ICD). No postoperative bleeding or pocket hematoma were detected in the 23 patients taking anticoagulation medication before implantation. During an average of (30±7) months' follow up, thrombus was dissolved in 20 patients, hemorrhage complications were observed in 2 patients (1 cerebral hemorrhage and 1 hematuria), anticoagulation therapy was discontinued in these 2 patients. Among 15 patients without immediate CIED implantation and treated with anticoagulation therapy during hospitalization, 2 patients developed complete paroxysmal Ⅲ° atrioventricular block, and recovered after therapy during hospitalization. Seven patients were re-hospitalized for CIED implantation due to bradycardia. Five patients died during follow-up (3 sudden cardiac death, 1 APTE combined with cerebral infarction, and 1 pulmonary infection). Conclusion: APTE is not rare in patients with the indication of CIED implantation, CIED implantation and anti-coagulation therapy are safe for these patients, and transient atrioventricular block could be detected in APTE patients.

RESUMO

Objective :To explore influence of lyophilized recombinant human brain natriuretic peptide (rhBNP) combined routine treatment on levels of Chemerin (a new adipocytokine) and Galectin (Gal)-3 in aged AMI patients.Methods :A total of 120 aged AMI patients treated in our hospital were randomly and equally divided into routine treatment group and rhBNP group (received rhBNP based on routine treatment ) ,both groups were treated for 3d.Vascular endothelial func-tion ,levels of Chemerin and Gal-3 before and after treatment and incidence rate of adverse reactions within one year were observed and compared between two groups .Results :Compared with before treatment ,after treatment ,there were signifi-cant rise in fore brachial artery endothelium dependent diastolic-systolic function (FMD) and NO level ,and significant re-ductions in levels of ET-1 ,Chemerin and Gal-3 in two groups ,P=0.001 all.Compared with routine treatment group after treatment ,there were significant rise in FMD [ (9.5 ± 2.5)% vs.(14.1 ± 3.4)%] and NO level [ (37.2 ± 3.1) μmol/L vs.(43.2 ± 5.7) μmol/L] ,and significant reductions in levels of ET-1 [ (72.1 ± 9.9) ng/L vs.(61.8 ± 8.5) ng/L] , Chemerin [ (25.2 ± 3.8) μg/ml vs.(16.9 ± 4.2) μg/ml] and Gal-3 [ (9.3 ± 1.4) ng/ml vs.(6.7 ± 1.2) ng/ml] in rhB-NP group ,and each HRV index of rhBNP group was significantly higher than that of routine treatment group , P=0.001 all.Compared with routine treatment group after one-year follow-up , there were significant reductions in mortality (13.3% vs.1.7%) and incidence rate of cardiac insufficiency (18.3% vs.5.0%) in rhBNP group , P<0.05 both .Con-clusion :Lyophilized rhBNP can significantly improve vascular endothelial function and HRV , reduce serum levels of Chemerin and Gal-3 and improve prognosis in AMI patients .

<p><b>OBJECTIVE</b>To explore the impact of 36-hour sleep deprivation (SD) on the brain electrophysiological indicators of visuo-motor coupling in young soldiers.</p><p><b>METHODS</b>During the 36-hour SD, 10 healthy young soldiers were tested on visuospatial rotation tasks by event-related potentials system before and after SD. The incubation period and amplitude of P500 as well as their error number and reaction time were measured.</p><p><b>RESULTS</b>Compared with subjects in SD 0-hour,subjects in SD 36-hour had significantly increased error rate [(9.7 ± 3.9)% vs. (18.3 ± 4.5)%, P<0.05] and significantly increased reaction time [(632.5 ± 53.6) ms vs. (693.6 ± 65.7) ms, P < 0.05]. Subjects in SD 36-hour showed significantly reduced amplitudes than those in SD 0-hour [(8.7 ± 2.3) ΜV vs. (5.2 ± 1.6) ΜV, P < 0.05]. Additionally, subjects in SD 36-hour showed significantly increased P500 latencies than did those in SD 0-hour [(489.6 ± 42.6) ms vs .(530.2 ± 51.9) ms, P < 0.05]. Compared with subjects in SD 0-hour, the deficit was an absence of a mental rotation function SD 36-hour in subjects.</p><p><b>CONCLUSIONS</b>The 36-hour SD in young soldiers can harm the processing mechanism of visuo-motor coupling in a certain extent. SD can affect the fixed position ability of visual space cognition in young soldiers.</p>

<p><b>OBJECTIVE</b>To explore the carrier state of hepatitis E virus(HEV) in livestock in Xi'an area.</p><p><b>METHODS</b>Bile samples from swine, canine, sheep and cow were collected from a local slaughtering house. Reverse transcriptase nested polymerase chain reaction (RT-nPCR) was employed to amplify the ORF2 region in HEV RNA genome. All positive samples were sequenced and compared with data from GenBank. Homology analysis was conducted based on the outcome of sequencing.</p><p><b>RESULTS</b>194, 178, 79 and 191 bile samples from swine, canine, cow and sheep were collected. Positive rates with RT-nPCR method in these domestic animals were 4.10%, 0%, 0% and 0% respectively. Genetic distance analysis indicated that strains being identified were close to genotype IV of HEV, then genotype I, II and III in nucleic acid. Same outcome was shown by the same analysis on amino acid.</p><p><b>CONCLUSION</b>Swine was the only reservoir of HEV in livestock and genotype IV was the prevalent genotype.</p>
