ABSTRACT

<p><b>OBJECTIVE</b>To study the long-term clinical results of 34 chronic duodenal ulcer patients treated with high selective vagotomy plus pylorus-preserved mucosal antrectomy (HSV + PPMA).</p><p><b>METHODS</b>Clinical follow-up results of the patients from 8 approximately 14 years were analyzed.</p><p><b>RESULTS</b>Thirty-tow patients (94.1%) followed-up for 8 approximately 14 years after operation achieved Visick grades I-II. No patient died. Gastric acid secretion and infection rate of Helicobacter pylori in the antral mucosa were significantly reduced after operation. No significant difference was found in bile acids, total bacterial counts in gastric juice, and the level of serum gastrin after operation. Gastric emptying was normal. No ulcer recurrence was found by barium meal and endoscopy.</p><p><b>CONCLUSIONS</b>HSV + PPMA is a better operative treatment for duodenal ulcer, which not only can decrease acid secretion and ulcer recurrence rate but also can preserve the function of antrum and pylorus and prevent post-operation bile reflux and intragastric bacterial overgrowth.</p>

ABSTRACT

Objective To introduce telomeres, telomerase and their expression in gastric carcinoma. Methods The related literatures were collected and reviewed.Rsults In summary, telomerase activity could be detected in 85%-90% of gastric cancer. Moreover, the patient with telomerase-positive tumors showed poorer prognosis than those with telomerase-negative tumours, indicating that telomerase-positive gastric cancer might have more malignant potential. Conclusion Knowledge of telomerase activity in gastric cancer may be useful in cancer diagnosis, as well as a prognostic indicator of clinical outcome. Future development of drugs aimed at telomerase inhibition may potentially provide a therapy with relatively less side effects.

ABSTRACT

Objective To investigate the role and clinical significance of proglumide (PGL) in gastrin-stimulating cell proliferation on human colon cell line SW480.Methods Effects of proglumide on the viable cell count (A value), IP3 (CPM value), [Ca2+]i and PKC activity of human colon cell line SW480 were evaluated in vitro by MTT assay, 3H-myo-inositol incorporation, fluorescence measurements and γ-32P-ATP incorporation. Results The results showed that A value of PGL+PG group is lower than that of PG or control group (P<0.01 in both groups); The concentration of IP3 or [Ca2+]i in PGL+PG group is lower than that in PG group (P<0.05). Conclusion These results indicate effects of proglumide may be through the phosphoinositide signalling pathway on gastrin-stimulating cell proliferation in human colon cell line SW480. It will provide an experimental evidence for antisingnal transduction therapy for patients with colon cancer.

ABSTRACT

The association of selective vagotomy and antrectomy for the treatment of duodenal ulcer is considered as the best procedure to prevent ulcer recurrenceut it brings about too many operative sequelae.The authors of this paper used highly selective vagotomy in association with antrum mocosa resection to treat duodenal ulcer and the effects on gastric acid level and serum level of gastrin were compared with those after selective vagotomy and antrectomy.It was found that the reduction of gastric acid and the changes of serum gastrin were similar after the 2 operative procedures.It is believed that the association of highly selective vagotomy and antrum mocosa resection is the better operative treatment for duodenal ulcer than the association of selective vagotomy and antrectomy since the former can both reduce the number of postoperative sequelae and preserve the functions of the pylorus and the antrum.

ABSTRACT

Primary tumors of small intestine are rarely eucountered clinically and account about for 1.5-2% of all the gastrointestinal tumors. It is quite difficult to establish the diagnosis preoperatively. In a 25-year-period from January I960 to December 1984, 45 patients with primary small bowel tumors were admitted to this institute, and their clinical data were analyzed. The rate of wrong diagnosis was 68.8% in this series as compared with 65% to 100% reported in the literature. The causes of misdiagnosis were as follows:1. There is usually no characteristic clinical sign.2. No specific diagnostic procedures are available.3. The clinicians do not have enough vigilance with this disease because of their rarity.It is emphasized that clinicians should be familar with the clinical signs and alert on the possibility of the disease.
