ABSTRACT

Objective To analyze the immunological effect and its influencing factors of HAART on HIV/AIDS with a baseline CD4 of less than 350 cells/mm3 in Hubei Province during 2011-2020. Methods The treatment information of people living with HIV/AIDS (PLWH) was collected through China Disease Control and Prevention Information System, and the change of CD4 mean value was analyzed by Wilcoxon sign rank sum test. Chi square test and multivariate logistic regression were used to analyze the factors influencing the immunological effect. Results The mean value of CD4 cell count at baseline was (172.78±107.65) cells/mm3 before treatment and (398.85±206.71) cells/mm3 in the fifth year after treatment. Patients with baseline CD4<50 /mm3 had the largest increase in CD4 mean value after treatment (P<0.05). Multivariate logistic regression analysis showed that Age ≥35 years, heterosexual transmission, time interval from diagnosis to treatment≥1 month, baseline CD4<50 /mm3, treatment time 7-<18 months, and drug failure were the risk factors for the success of antiviral therapy(P<0.05). Conclusion The immunological effect of HIV/AIDS antiviral therapy in Hubei Province is affected by many factors. It is necessary to strengthen the implementation of early detection and early treatment strategy and promote treatment compliance education for patients aged ≥35 years.

ABSTRACT

OBJECTIVE@#To investigate the protective effect against intestinal mucosal injury in rats following traumatic brain injury (TBI) and explore the underlying mechanism.@*METHODS@#SD rat models of TBI were established by fluid percussion injury (FPI), and the specimens were collected at 12, 24, 48, and 72 h after TBI. Another 15 rats were randomly divided into shamoperated group (n=5), TBI with saline treatment (TBI+NS) group (n=5), and TBI with PD treatment (TBI+PD) group (treated with 30 mg/kg PD after TBI; n=5). Body weight gain and fecal water content of the rats were recorded, and after the treatments, the histopathology of the jejunum was observed, and the levels of D-lactic acid (D-LAC), diamine oxidase (DAO), ZO-1, claudin-5, and reactive oxygen species (ROS) were detected. Lipid peroxide (LPO) and superoxide dismutase (SOD) 2 content, jejunal pro-inflammatory factors (IL-6, IL-1β, and TNF- α), Sirt1 activity, SOD2 and HMGB1 acetylation level were also determined after the treatments.@*RESULTS@#The rats showed significantly decreased body weight and fecal water content and progressively increased serum levels of D-LAC and DAO after TBI (P < 0.05) with obvious jejunal injury, significantly decreased expression levels of ZO-1 and claudin-5, lowered SOD2 and Sirt1 activity (P < 0.05), increased expression levels of LPO, ROS, and pro-inflammatory cytokines, and enhanced SOD2 and HMGB1 acetylation levels (P < 0.05). Compared with TBI+NS group, the rats in TBI+PD group showed obvious body weight regain, increased fecal water content, reduced jejunal pathologies, decreased D-LAC and DAO levels (P < 0.05), increased ZO-1, claudin-5, SOD2 expression levels and Sirt1 activity, and significantly decreased ROS, LPO, pro-inflammatory cytokines, and acetylation levels of SOD2 and HMGB1 (P < 0.05).@*CONCLUSION@#PD alleviates oxidative stress and inflammatory response by activating Sirt1-mediated deacetylation of SOD2 and HMGB1 to improve intestinal mucosal injury in TBI rats.

ABSTRACT

<p><b>BACKGROUND</b>We investigated the impact of eliminating the impingement between extensor mechanism and tibial insert on patellar tracking and patellar ligament tension in high knee flexion.</p><p><b>METHODS</b>Six cadaveric specimens were tested on an Oxford-type testing rig. The Genesis II knee system was implanted into each specimen knee with the traditional tibial insert and high-flex insert successively. Compared to traditional insert, the high-flex insert was characterized with a chambered anterior post and a chambered anterior lip which eliminates patella-post and patellar ligament-anterior lip impingements. The patella was tracked with an NDI Optotrak Certus system. The patellar ligament tension was measured using a NKB S-type tension transducer.</p><p><b>RESULTS</b>There was a decrease of resultant patellar translation relative to the femur with statistically significant (P<0.05) at 90 degrees to 150 degrees of knee flexion and a decrease of patellar ligament tension with statistical significance (P<0.05) at 100 degrees, 120 degrees, 130 degrees, and 140 degrees of flexion using high-flex insert compared to traditional insert.</p><p><b>CONCLUSIONS</b>Eliminating the impingement between extensor mechanism and implant in high knee flexion altered patellar tracking and reduced patellar ligament tension, which would facilitate high knee flexion.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the role of cell adhesion molecule CD44 in the lung on airway inflammatory response in rats with asthma.</p><p><b>METHODS</b>Thirty-two Sprague-Dawley rats were randomly divided into normal control and asthma groups. Asthma was induced by repeated inhalation of ovalbulium. CD44 expression in the lung was detected by semi-quantitatively reverse transcription-polymerase chain reaction (RT-PCR) and immuno-histochemical staining 1 week and 2 weeks after ovalbulium challenge. Differential leukocytes (mononuclear phagocytes, neutrophils, eosinophils, and lymphocytes) in bronchoalveolar lavage fluid (BALF) were counted.</p><p><b>RESULTS</b>CD44 expression in the lung increased 1 week after ovalbulium challenge (<0.05) and increased more significantly 2 weeks after ovalbulium challenge (<0.01) compared with that in the control group. The percentages of lymphocytes and eosinophils in BALF increased, while the percentage of BALF mononuclear phagocytes decreased significantly 1 week and 2 weeks after ovalbulium challenge in the asthma group compared with those in the control group (<0.05). An increased percentage of neutrophils was found 1 week after ovalbulium challenge in the asthma group compared with the control group (<0.05). CD44 expression in the lung was positively correlated with the percentages of lymphocytes and eosinophils in BALF in the asthma group 1 week and 2 weeks after ovalbulium challenge, in contrast, lung CD44 expression was negatively correlated with the percentage of mononuclear phagocytes in the asthma group 1 week after ovalbulium challenge.</p><p><b>CONCLUSIONS</b>CD44 was over-expressed in the lung and closely related to inflammatory infiltration in rats with asthma. CD44 may play an important role in the development of airway inflammatory in asthma.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the failure mechanisms of revision hip arthroplasties and evaluate the effects of surgical technique, prosthesis design and patient-related risk factors on different failure mechanisms.</p><p><b>METHODS</b>A review of all revision hip arthroplasties from November 1995 to June 2008 identified 30 patients who underwent 30 revisions with 18 males and 12 females. The overall mean age for primary arthroplasties was 49 years (range 25-68 years) and 53 years (range 27-72 years) for index revision arthroplasties and the average interval between these two operations was 43.8 months (0-156 months). The failure mechanisms of index revision arthroplasties and primary arthroplasties were assessed and compared. Direct comparisons were made of data for the different age categories in terms of time to failures and reasons for failures.</p><p><b>RESULTS</b>Regarding revision or prosthesis removal as the end point of the study, the reasons for 30 revision arthroplasties were aseptic loosening in 22 hips (73.3%), infection in 4 hips (13.3%), periprosthetic fracture in 3 hips and instability in 1 hip (3.3%). The overall mean age for last arthroplasties or prosthesis removal was 58 years (range 38-77 years) with an average interval of 78.8 months (range 1-216 months) from previous revision arthroplasties. The mean time to failure for patients above 60 years of age was significantly shorter than patients below 60 years of age (P < 0.01).</p><p><b>CONCLUSION</b>The majority of failure mechanisms of revision hip arthroplasties are ineffective fixation of revisional implants and recurrence of local infection, which reveals the limitations to joint reconstruction philosophy and surgical technique.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the clinical results and complications of total knee arthroplasty (TKA) performed for patients with different ages.</p><p><b>METHODS</b>From January 2004 to March 2005,212 patients (300 knees) were followed up, all patients were divided into 6 groups according to the age. All the complications and knee society scores (KSS) were documented and retrieved for analysis.</p><p><b>RESULTS</b>KSS of all groups was significantly improved after TKA. The patients whose age was 70 years or older, and underwent bilateral TKA achieved the best improvement of knee function. The complication rate increased with the patients' age.</p><p><b>CONCLUSIONS</b>TKA predictably alleviates knee pain and restores knee function. Aging itself does not compromise clinical results of TKA. However perioperative complication rate is a little bit higher in aged patient group and simultaneously bilateral TKA patient group.</p>

ABSTRACT

Objective To study the sequence variation in the env V3-V4 human immunodeficiency virus (HIV)-1 predominant subtype B strains in Hubei Province and to understand the epidemic characteristics and mutations of HIV-1. Methods Epidemiologic survey was done in the HIV-1 carriers in Hubei area. HIV-1 env V3-V4 regions were amplified by nested-polymerase chain reaction (nPCR). The sequences were determined and then phylogenetic analysis was performed. The difference of gene distance were checked by chi square test and the variation of gene distance were descriptively analyzed. Results Four HIV-1 strains or circulating recombinant forms (CRF) were identified in Hubei Province subtype B', B'/C, CRF01_AE and C were 82.69%, 7.69%, 7.69% and 1.92% ,respectively. B' strains were closely related with B. CN. RL 42 from Yunnan Province and B. CN. 02. 02HN from Province Henan, the gene distances were 7.08 ± 2.19 and 7.88 ± 2.28, respectively. Genetic divergence of env of B' strains showed that subtype B' has existed in Hubei area for about 10 years. Amino acid sequence analysis of section env showed V4 was more variable than C3, V3. The top four peptides of V3 loop were GPGR (46.5%), GPGK (30.2%), GPGQ (13.6%) and GQGR (9.3%). Predictions for the potential use of co-receptors on the basis of the critical amino acids within V3 loop disclosed that 16.28% were CCR5-using (R5/NSI), 13.95% were CXCR4-using (X4/SI) while the co-receptor usage of the vast majority (69.77%) could not be predicted. The analysis of glycosylation sites showed that there were 9 sites in env V3-V4 regions of HIV-1 strains in Hubei area and there were deletions in 8 of them, Conclusions Subtype B' is still the main epidemic subtype in Hubei Province and high homologous to the strains from Yunan and Henan Province.

<p><b>OBJECTIVE</b>To study the drug resistance status on HIV-1 patients who had been treated with highly active antiretroviral therapy (HAART) and those treatment-naive ones in Hubei province.</p><p><b>METHODS</b>Nested polymerase chain reaction (PCR) was used to amplify 2 kb DNA fragment in HIV pol gene from peripheral blood of the HIV infected patients and the PCR products were sequenced. The sequences were compared to the Stanford HIV drug resistance database.</p><p><b>RESULTS</b>Nineteen patients were treated with regimens composed of two Nucleoside Reverse Transcriptase Inhibitors (NRTIs) and one Non-Nucleoside Reverse Transcriptase Inhibitors (NNRTI), with 25 patients as treatment-naive. Some protease (PR) drug-resistant mutations were found in these samples, such as D30N (2.27%), D30G (2.27%), M46I (4.55%), M46N (2.27%), I47V (4.55%), I84V (4.55%), I84L (2.27%), N88S (2.27%) and L90S (2.27%) that all belonged to major drug-resistant but A71T (29.55%) belonged to minor resistance mutations Five treated patients were detected having mutations associated RT drug resistance: M41L (5.26%), A62V (5.26%),D67N (5.26%), L210W (5.26%), T215Y (15.79%); K103E (5.26%), K103N (10.53%), Y181C (5.26%), G190A (5.26%), K238N (5.26%), while five treatment-naive patients were detected to have had mutations associated RT drug resistance M184V (4%), K65N (4%), Y115M (4%), F116L (4%), M184I (4%), V179D (4%), G190R (4%).Some additional mutations were detected in RT whose role involve in drug resistance still remained unknown. F214L was positively associated with HAART treatment (P = 0.03).</p><p><b>CONCLUSION</b>Significant differences were found between drug resistance mutations to RTIs in treated and treat-naive patients in Hubei province, indicating that drugs had affected the occurrence of drug resistance mutations. At the same time, novel RT mutations F214L might be associated with HAART or some other drugs.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the distribution of human immunodeficiency virus-1 (HIV-1) genotypes in Hubei province.</p><p><b>METHODS</b>Epidemiological survey was carried out to HIV-1 carriers who were identified in Hubei province. HIV-1 env V3-V4, gag P17/24 and the first exon of tat region were amplified by nested-polymerase chain reaction(nPCR) .The sequences were determined, and phylogenetic analyses were then performed.</p><p><b>RESULTS</b>4 HIV-1 strains or circulating recombinant forms (CRFs) were identified in Hubei province with subtype B' the predominant which covered 5 kinds of populations including former blood donors, blood receivers, spouses of the infected people, sex workers and their clients, homosexuals, mainly distributed in the areas with many former blood donors. CRF08-BC and CRF01-AE were found distributed in economically more developed cities or southern area of the province, and the major transmission routes was through sexual contact. Only 1 patient, an injecting drug user, was identified having subtype C.</p><p><b>CONCLUSION</b>Subtype B' was the main epidemic subtypes in Hubei province while CRF08-BC, CRF01-AE and subtype C were also circulating in the province, indicating the transmission of the disease might to become more complex.</p>

ABSTRACT

Objective To study The influence and preventive measures for the test results of total bile acid about blood Lipid testing reagent. Methods In Mindray BS -300 automatic biochemical analyzer, we detected total bile acid of the fresh mixed serum separately, then we tested the same samples after the test items of cholesterol, triglycerides, high -density lipoprotein, low - density lipoprotein. Then using fresh serum mixed with normal saline and blood lipid reagent according to 1: 1 dilution ,we measured TBA. We did cross - contamination corrected test (by the anti - cross - contamination to repeat the first step after the test) and recorded all results and had statistics analysis. Results This experiment found that reagents Ⅰ of TC, TG and the reagent Ⅱ of HDL - C , LDL - C had significant positive interference in determination of TBA results. After being corrected by anti - cross - contamination, the results returned to normal. Conclusion When using the automatic biochemical analyzer, we not only acquired the worker fououed of the equipment, and reagents, determined principle, but also pay attention to cross - contamination among reagents, and then in accordance with the equipment work characteristics to establish the appropriate measures to eliminate them, so made sure the reliability of the experimental data to provide clinic with the true objective and actual diagnosis evidence fundamentally.
