A osteonecrose dos maxilares induzida por medicamentos (MRONJ) caracteriza-se por exposição óssea ou osso que pode ser sondado através de fístula intra ou extraoral, em região maxilofacial, e que não cicatriza dentro de oito semanas. A MRONJ é uma condição rara e debilitante que pode causar dor, disfagia e odor desagradável na cavidade oral, afetando pacientes com histórico ou sob uso contínuo de terapia antirreabsortiva, isolada ou associada a imunomoduladores ou drogas antiangiogênicas, mas sem histórico de radioterapia nos maxilares. O objetivo desta revisão narrativa de literatura é compilar os principais aspectos sobre a etiopatogenia da MRONJ e as opções terapêuticas disponíveis. A etiologia da MRONJ é multifatorial, complexa, e não está totalmente compreendida, não havendo um tratamento definitivo, mas diversas modalidades terapêuticas que visam o controle da dor e da progressão da osteonecrose. Conclui-se com essa revisão que o entendimento da etiopatogenia da MRONJ pelo cirurgião-dentista lhe permite adotar medidas preventivas, bem como o conhecimento das modalidades terapêuticas disponíveis lhe possibilita oferecer o manejo adequado para seu paciente, conforme o estágio da doença.
Medication-related osteonecrosis of the jaw (MRONJ) is characterized by exposed bone or bone that can be probed through an intra or extraoral fistula, in the maxillofacial region, which does not heal within eight weeks. MRONJ is a rare and debilitating condition that can cause pain, dysphagia and unpleasant odor in the oral cavity, affecting patients with a history or continuous use of antiresorptive therapy, alone or associated with immunomodulators or antiangiogenic drugs, but without a history of radiotherapy to the jaws. The aim of this narrative literature review is to compile the main aspects about the etiopathogenesis of MRONJ and the available therapeutic options. The etiology of MRONJ is multifactorial, complex, and is not fully understood, with no definitive treatment, but several therapeutic modalities that aim to control pain and the progression of osteonecrosis. It is concluded from this review that the understanding of the etiopathogenesis of MRONJ by the dental surgeon allows him to adopt preventive measures, as well as the knowledge of the therapeutic modalities available allows him to offer the appropriate management for his patient, depending on the stage of the disease.

ABSTRACT Purpose: The epithelial-mesenchymal transition of human lens epithelial cells plays a role in posterior capsule opacification, a fibrotic process that leads to a common type of cataract. Hyaluronic acid has been implicated in this fibrosis. Studies have investigated the role of transforming growth factor (TGF)-β2 in epithelial-mesenchymal transition. However, the role of TGF-β2 in hyaluronic acid-mediated fibrosis of lens epithelial cell remains unknown. We here examined the role of TGF-β2 in the hyaluronic acid-mediated epithelial-mesenchymal transition of lens epithelial cells. Methods: Cultured human lens epithelial cells (HLEB3) were infected with CD44-siRNA by using the Lipofectamine 3000 transfection reagent. The CCK-8 kit was used to measure cell viability, and the scratch assay was used to determine cell migration. Cell oxidative stress was analyzed in a dichloro-dihydro-fluorescein diacetate assay and by using a flow cytometer. The TGF-β2 level in HLEB3 cells was examined through immunohistochemical staining. The TGF-β2 protein level was determined through western blotting. mRNA expression levels were determined through quantitative real-time polymerase chain reaction. Results: Treatment with hyaluronic acid (1.0 μM, 24 h) increased the epithelial-mesenchymal transition of HLEB3 cells. The increase in TGF-β2 levels corresponded to an increase in CD44 levels in the culture medium. However, blocking the CD44 function significantly reduced the TGF-β2-mediated epithelial-mesenchymal transition response of HLEB3 cells. Conclusions: Our study showed that both CD44 and TGF-β2 are critical contributors to the hyaluronic acid-mediated epithelial-mesenchymal transition of lens epithelial cells, and that TGF-β2 in epithelial-mesenchymal transition is regulated by CD44. These results suggest that CD44 could be used as a target for preventing hyaluronic acid-induced posterior capsule opacification. Our findings suggest that CD44/TGF-β2 is crucial for the hyaluronic acid-induced epithelial-mesenchymal transition of lens epithelial cells.

Introdução: Na medida em que envelhecemos os lábios estreitam-se, ocasionando perda de volume e contorno e como forma de minimizar este efeito fisiológico o preenchimento labial de escolha utilizado é o ácido hialurônico. É possível perceber efeitos adversos advindos do emprego deste material, e pelo fato da informação ao paciente ser assegurada pelo Código de Defesa do Consumidor e pelo fato da necessidade dos Cirurgiões-Dentistas terem de esclarecer seus pacientes, o Termo de Consentimento Livre e Esclarecido tornase necessário. Objetivo: identificar, por meio de aplicação de questionário, a percepção de profissionais que trabalham com Harmonização Orofacial em relação a necessidade do emprego do Termo de Consentimento Livre e Esclarecido (TCLE). O questionário apresentou 6 perguntas objetivas, que foram disponibilizadas na plataforma Google Forms®. Material e Método: os dados obtidos foram tabulados em uma planilha eletrônica do programa Microsoft Excel e após analisados descritivamente através de tabelas de frequência, porcentagens e gráficos estatísticos. Resultados: dentre os entrevistados foi constatado que a maioria, 87,5% dos especialistas em Harmonização Orofacial realizam o procedimento de preenchimento labial em sua rotina clínica, e 12,5% não. Conclusão: no presente estudo identificamos que os especialistas realizam o emprego do TCLE, em sua maioria, porém, alguns destes ainda negligenciam o seu uso(AU)
Introduction: As we age, the lips become thinner and to minimize this effect, the lip filler used is hyaluronic acid. It is possible to notice adverse effects arising from the use of this material, and it is extremely important that Dental Surgeons have to clarify their patients, the Free and Informed Consent Form becomes necessary. Objective: to identify, through the application of a questionnaire, the perception of professionals who work with Orofacial Harmonization in relation to the need to use the Free and Informed Consent Form (TCLE). The questionnaire presented 6 objective questions, which were made available on the Google Forms® platform. Materials and Methods: the data obtained were tabulated in a Microsoft Excel spreadsheet and then analyzed descriptively using frequency tables, percentages and graphs. Results: among those interviewed, it was found that the majority, 87.5% of specialists in Orofacial Harmonization perform the lip filling procedure in their clinical routine, and 12.5% do not. With the high percentage of 59.4%, it was possible to verify that the majority of professionals perform 1 to 3 procedures per month; 31.3% perform 4 to 9 procedures per month; and 9.4% of 10 or more monthly procedures. Conclusion: in the present study it was possible to identify that the majority of specialists in Orofacial Harmonization use the informed consent form, however, some of them still neglect its use(AU)

Epilepsy is the chronic non-communicable disease of the nervous system most prevalent in the world. Valproic acid (VPA) is one of the most used drugs in the treatment of epilepsy but with various side effects. One of the organs that can be affected is the testis, where it has been seen that men treated with VPA reduce their fertility rates, in addition to causing endocrine disorders by decreasing androgens and gonadotropins. In animal models, it has been shown to reduce the weights of the glands attached to the male reproductive tract, as well as at the testicular level, decreasing sperm concentration and increasing apoptotic cell count. These effects are because VPA increases reactive oxygen species (ROS), causing damage to macromolecules and affecting all cellular processes sensitive to oxide reduction. Throughout testicular development, in utero, it has been seen that the expression of antioxidant enzymes such as superoxide dismutase, catalase and glutathione peroxidase, are lower during early embryonic development, as well as vitamin E (VE) is decreased. Therefore, they are not sufficient to reverse the toxic effects of ROS. The objective of this study was to review the use of VPA during pregnancy, its effect on testicular development, and to explore the potential protective role of vitamin E.
La epilepsia es una enfermedad crónica no transmisible que afecta al sistema nervioso más prevalente en el mundo. Dentro de los tratamientos, uno de los fármacos más utilizados es el ácido valproico (AVP), el que ocasiona diversos efectos secundarios. Entre los órganos que se pueden ver afectados se encuentra la gónada masculina, en donde se ha visto que hombres en tratamiento con AVP reducen sus tasas de fecundidad, además de causar trastornos endocrinos disminuyendo andrógenos y gonadotrofinas. En modelos animales, se ha visto que disminuye los pesos de las glándulas anexas al tracto reproductor masculino, como también a nivel testicular, disminuyendo la concentración espermática y aumentando el recuento de células apoptóticas. Estos efectos se deberían a que el AVP aumenta las especies reactivas de oxígeno (ROS), ocasionando daño en macromoléculas, afectando todos los procesos celulares sensibles a óxido reducción. A lo largo del desarrollo testicular, in utero se ha visto que la expresión de enzimas antioxidantes como superóxido dismutasa, catalasa y glutatión peroxidasa, son más bajos durante el desarrollo embrionario temprano, como también la vitamina E (VE) se encuentra disminuida. Por tanto, no resultan suficientes para revertir los efectos tóxicos de las ROS. El objetivo de esta revisión fue asociar el uso de AVP durante la gestación y sus efectos a nivel del desarrollo testicular y describir el potencial rol protector de la VE.

Background: This study was conducted to gather expert opinion regarding the clinical use of a high dose of amoxicillin 600 mg + clavulanic acid 42.9 mg in pediatric practice in Indian settings.Methods: This cross-sectional study comprised 19 questions and gathered perspectives from pediatricians across India regarding the prescription practices of amoxicillin (600 mg) and clavulanic acid (42.9 mg) for various bacterial infections in pediatric patients. Data were analyzed using descriptive statistics.Results: Majority (85.65%) of clinicians opined that the high dose of amoxicillin (600 mg) and clavulanic acid (42.9 mg) was very effective in treating bacterial infections in pediatric patients. About 54% of the respondents reported prescribing 600 mg amoxicillin + 42.9 mg clavulanic acid as the high dose for pediatric patients, while 41% of them prescribed 400 mg amoxicillin + 57 mg clavulanic acid as the high dose for pediatric patients. According to 51% of clinicians, the benefits of prescribing the higher-strength formulation of amoxicillin (600 mg) + clavulanic acid (42.9 mg) compared to the standard-strength formulation of amoxicillin (500 mg) + clavulanic acid (125 mg) include increased effectiveness against resistant bacteria, reduced risk of adverse effects, and reduced frequency of dosing.Conclusions: This study highlighted the clinicians' preference for the high dose of amoxicillin (600 mg) and clavulanic acid (42.9 mg) as an effective treatment for bacterial infections in pediatric patients. The respondents preferred the high dose over other doses of amoxicillin and clavulanic acid due to the lower percentage of adverse effects.

Background: The bacterium Mycobacterium tuberculosis causes tuberculosis (TB), a contagious, airborne infection that destroys body tissue. Pulmonary TB occurs when M. tuberculosis primarily attacks the lungs. However, it can spread from there to other organs. Pulmonary TB is curable with an early diagnosis and antibiotic treatment. Methods: The present study was conducted at Adichunchanagiri Institute of Medical Sciences, Mandya with total of 100 sputum positive TB cases. The analysis was done between January 2023 to June 2023. Results: The proportion of sputum positive pulmonary tuberculosis and sputum positivity was highest in 20 to 40 years age group. The number of sputum positive pulmonary tuberculosis was three times more common in male compares to female. Conclusions: The highest burden of sputum positive pulmonary tuberculosis and maximum sputum positivity rate was found in productive age group. The proportion of sputum positive PTB was more in male than female patients. All age group except in patient between 0-19 year抯 ratio between male and female were almost equal.

Background: Total knee replacement is one of the common orthopaedic procedures performed worldwide. Blood transfusion is one of the major requirements in TKR procedure due to the amount of blood loss during and after the procedure. We carried out a prospective study to determine the efficacy of tranexamic acid in controlling blood loss during TKR procedure. Methods: Study was conducted at a tertiary care centre, involving cases operated by a single surgeon. Study included 140 patients undergoing primary TKR for advance degenerative disease of knee and were divided into two groups of 70 each, one group that received tranexamic acid before surgery and another group that did not receive tranexamic acid before surgery. Patient with allergy to the drug, hepato/renal dysfunction, DVT, abnormal PT and INR were not included. Tranexamic acid was given intravenously as well as intra-articular. Results: Pre-operative haemoglobin ranged from 10.2 gm% to 14.4 gm% in the group getting tranexamic acid and from 10% to 14 % in the group not getting tranexamic acid. Post operatively haemoglobin varied from 8.4 gm% to 12.8 gm% in Group 1 and from 7.8 gm% to 12 gm% in Group 2. Difference of mean post-operative Hb (p=0.0045) and PCV (p=0.0024) in two groups was statistically significant. Conclusions: We concluded that administration of tranexamic acid reduces the blood loss as well as need of blood transfusion in a patient undergoing total knee replacement.

Background: Cervical cancer arises when cells in the cervix undergo abnormal changes, leading to tumor formation. Although preventable, it remains the fourth most commonly diagnosed cancer and the leading cause of cancer-related deaths among women. This study aimed to assess the prevalence of cervical cancer among village women.Methods: This prospective observational study was conducted at Upazilla Health Complex (UHC), Sariakandi, Bogura, Bangladesh from January 2023 to December 2023. As the study subjects, a total of 1186 village women who participated in the cervical cancer screening program of the mentioned UHC were enrolled purposively. For data analysis, MS Office tools were applied.Results: In this study, the majority of participants belonged to the >40 years’ age group (53.2%), were housewives (73%), married at the age of 20 or younger (59%), and reported negative vaginal discharge (53%). Among the total participants, the prevalence of cervical cancer, as determined by visual inspection with acetic acid (VIA) tests, was found to be 1.18%.Conclusions: In comparison to other South Asian or African countries, the prevalence of cervical cancer among village women in Bangladesh is relatively low. However, housewives aged over 40 years, experiencing negative vaginal discharge, and marrying at age 20 or younger are particularly susceptible to such diseases here.

Colorectal cancer is the third most frequently diagnosed cancer in both genders and the fourth foremost cause in respect to cancer-associated mortality. The metastasis mechanism of colorectal cancer shows that it gradually developed in the form of polyps under granular cells in the large intestine, which damages blood and lymph vessels. At present, chemotherapy and surgery are the major clinical approaches in treatment. Therefore, developing novel and effective drugs are requiring use of natural derivative compounds, a potent and significant effect in restricting the progression of colorectal cancer (CRC). The present research investigates the in vitro anticancer properties of various extracts of plant Oxalis corniculata counter cell viability of CRC cell line (HCT116) using an MTT assay. In silico docking studies of hexadecanoic acid were performed with cytochrome P450CYP17A1 protein (3RUK) to predict potential inhibitors and druglikeness as potential CRC inhibitors using ADME profiling adhered to five rule of Lipinski. The finding indicated the extracts of the Oxalis corniculata have anticancer and anti-proliferative activity. The cell cytotoxic observed against chloroform extract exhibited the highest inhibition against the HCT116 cell line, following ethanol and aqueous extract. The determined IC50 values for the ethanol, aqueous, and chloroform extracts were 53.94±1.29 µg/ml, 61.85±0.43 µg/ml, and 47.34±1.24 µg/ml, respectively The in silico molecular docking result shows that n-hexadecanoic acid compound was found to be effective against 3RUK protein associated with CRC. Therefore, the finding suggests Oxalis corniculata plants could be used to formulate potential therapeutic drugs for the development of anticancer agents.

Resumen Introducción: El ataque cerebrovascular (ACV) en pacientes jóvenes está asociado a factores de riesgo no convencionales y su búsqueda activa puede disminuir la recurrencia de estos eventos. Presentación del caso: Se presentaron dos casos de pacientes jóvenes con infección por virus de inmuno-deficiencia humana (VIH) que mostraron síntomas neurológicos, con evidencia en resonancia magnética (RM) de un evento isquémico de evolución subaguda y niveles de homocisteína elevados, a quienes se les realizó una búsqueda de mutación de metilenotetrahidrofolato reductasa (MTHFR), la cual fue positiva para mutación homocigota. Discusión: La infección por VIH aumenta el riesgo de ACV en la población joven, sin embargo, su efecto en pacientes con viremia controlada está poco claro (1). La presencia de déficit de proteína S y la hiperhomocisteinemia son los estados procoagulantes más frecuentes en pacientes con VIH (2). El papel que juegan la presencia de mutaciones genéticas en relación con la hiperhomocisteinemia en pacientes con VIH aún está por establecerse. Conclusiones: La búsqueda activa de factores de riesgo no frecuentes en pacientes jóvenes con ACV juega un rol importante en la prevención de futuros eventos y modificación de la enfermedad. Así, las pruebas genéticas abren nuevas posibilidades para entender la fisiopatología de la enfermedad y encontrar nuevas relaciones entre factores de riesgo.
Abstract Introduction: Stroke in young patients is associated with unconventional risk factors. The active search for these risk factors can reduce the recurrence of such events. Case presentation: We present two cases of young patients with human immunodeficiency virus (HIV) infection presenting neurological symptoms, with evidence of a subacute ischemic event in the magnetic resonance, and high homocysteine levels who underwent a search for the methylenetetrahydrofolate reductase (MTHFR) mutation, which was positive for the homozygous mutation. Discussion: HIV infection increases the risk of stroke in the young population; however, its effect in patients with controlled viremia is unclear. The presence of protein S deficiency and hyperhomocysteinemia are the most frequent procoagulant states in patients with HIV. The role of genetic mutations and hyperhomocysteinemia in patients with HIV is yet to be established. Conclusion: The active search for rare risk factors in young stroke patients plays an important role in preventing future events and modifying the course of the disease. Genetic tests open up new possibilities to understand the pathophysiology of the disease and find new relationships between risk factors.

Abstract Introduction : Chia and flax seeds are rich in alpha-linolenic acid (ALA), which is bioconverted into the active derivatives eicosapentaenoic (EPA) and doco sahexaenoic (DHA) having multiple beneficial effects. However, there is limited knowledge about the anti-inflammatory effects of chia and flax integral flours diets rich in ALA. Objective : The study aimed to evaluate the anti-inflammatory effect of dietary supplementation with integral chia and flax flours in a murine model of LPS-induced systemic inflammation. Methods : Balb/c mice were distributed into three groups: diet A (control), diet B (supplemented with inte gral chia flour), and diet C (supplemented with integral flax flour). Nutritional, hematological, and biochemical determinations were performed. ALA, EPA, and DHA were assessed by GC-MS in the liver, brain, cardiac and skeletal muscles. NF-kB immunoassays were per formed in kidney, liver, and peritoneal macrophages, respectively. The phagocytic capacity was determined in peritoneal macrophages and the expression of the pro- and anti-inflammatory cytokines was assessed by RT-qPCR in the kidney, liver, and spleen. Results : Diets B and C exhibited optimal nutritional adequacy and caused increased levels of ALA, EPA, and DHA in critical tissues compared to the control. The phagocytic capacity of murine peritoneal macrophages (p< 0.01) and IL-10 transcription increased, whereas the expression of NF-κB, IL-1β, IL-6, and TNF-α decreased in animals fed both experimental diets. Conclusions : This work contributes to the current knowledge of the anti-inflammatory effects of chia and flax integral flours rich in ALA and reinforces the health advantages of their consumption.
Resumen Introducción : Las semillas de chía y lino son ricas en ácido alfa-linolénico (ALA), sus derivados activos eico sapentaenoico (EPA) y docosahexaenoico (DHA) ejercen probados efectos beneficiosos. Existe un conocimiento limitado sobre los efectos protectores de ambas semillas bajo la forma de harinas integrales, siendo de particular interés el efecto antiinflamatorio. Objetivo : El objetivo de este trabajo fue evaluar el efecto antiinflamatorio de la suplementación dietaria con harinas integrales de semillas de chía y lino en un modelo murino de inflamación sistémica inducido por LPS. Métodos : Ratones de la cepa Balb/c fueron distribui dos en tres grupos: dieta A (control), dieta B (suplemen tada con harina integral de chía) y dieta C (suplementa da con harina integral de lino). Se efecturaron determi naciones nutricionales, hematológicas y bioquímicas. El contenido de ALA, EPA y DHA en hígado, cerebro, corazón y músculo esquelético se determinó por cromatografía GC-MS. Se realizó la inmunodetección de NF-kB en macrófagos peritoneales, riñón e hígado. Se determinó la capacidad fagocítica de macrófagos peritoneales y se evaluó la expresión de citoquinas pro y antiinflamatorias por RT-qPCR en riñón, hígado y bazo. Resultados : Las dietas B y C mostraron una adecua ción nutricional óptima y generaron niveles elevados de ALA, EPA y DHA en tejidos críticos. La capacidad fagocítica de los macrófagos peritoneales (p< 0.01) y la transcripción de IL-10 aumentó, mientras que la expre sión de NF-κB, IL-1β, IL-6 y TNF-α disminuyó en animales de los grupos B y C. Conclusiones : Este trabajo contribuye al conocimien to actual de los efectos antiinflamatorios de ambas hari nas integrales y refuerza los beneficios de su consumo.

Abstract In recent years, drug repurposing (DR) has gained significant attention as a promising strategy for iden tifying new therapeutic uses of existing drugs. One po tential candidate for DR in cancer treatment is sodium dichloroacetate (DCA), which has been shown to alter tumor metabolism and decrease apoptosis resistance in cancer cells. In this paper, we present a scoping review of the use of DCA for cancer treatment in adult patients, aiming to identify key research gaps in this area. This scoping review aims to explore the existing scientific literature to provide an overview of the use of DCA (any dose, frequency, or route of administration) in adults with cancer. A comprehensive literature search of the medical databases MEDLINE/PubMed, LILACS, EPISTEMONI KOS, the Cochrane Library, and ClinicalTrials was performed. We included publications reporting on adult patients diagnosed with any type of cancer treated with sodium dichloroacetate in combination or not with other drugs. All types of study design were included. A total of 12 articles were included, most of them were case reports. We found a high degree of heteroge neity between them. The most frequent adverse events in the evaluated studies were asthenia, reversible toxic ity, and an increase in liver enzymes. Effectiveness was difficult to evaluate. We conclude that there is insufficient evidence to affirm that treatment with DCA in cancer patients is effective or is safe.
Resumen El reposicionamiento de fármacos (RF) es un enfo que terapéutico reciente que se presenta como una estrategia prometedora para identificar nuevos usos terapéuticos de fármacos existentes. Un candidato potencial para RF en el tratamiento del cáncer es el dicloroacetato de sodio (DCA), el cual ha mostrado la capacidad de alterar el metabolismo tumoral y dismi nuir la resistencia a la apoptosis de células tumorales. La presente es una revisión (scoping review) del uso del DCA para el tratamiento del cáncer en pacientes adultos, que tiene como objetivo identificar brechas de investigación claves en esta área. Esta revisión pretende explorar la literatura científica existente, para propor cionar una visión general del uso del DCA (cualquier dosis, frecuencia o vía de administración) en individuos adultos con cáncer. Se llevó a cabo una búsqueda exhaustiva de la lite ratura en las bases médicas de datos MEDLINE/PubMed, LILACS, EPISTEMONIKOS, the Cochrane Library y Clini calTrials. Se incluyeron publicaciones que informaban sobre pacientes adultos diagnosticados con cualquier tipo de cáncer, tratados con DCA, en combinación o no con otros fármacos. Dichos estudios presentaban distintos tipos de diseño. Se incluyó un total de 12 artículos, la mayoría de los cuales fueron reportes de casos. Se encontró un alto grado de heterogeneidad entre los mismos. Los even tos adversos más frecuentes fueron astenia, toxicidad reversible y aumento de las enzimas hepáticas, siendo la efectividad terapéutica difícil de evaluar. Concluimos que existe evidencia insuficiente para afirmar que el tratamiento con DCA en pacientes con cáncer es efectivo y/o seguro.

Background and Objectives: It is not clear whether the increase in nutrition students' knowledge is associated with healthier eating behavior and fewer micronutrient deficiencies that can cause DNA damage. Deficiency in some vitamins can be a risk factor for increased homocysteine (Hcy) levels, a marker of cardiovascular risk. Therefore, this study aimed to verify whether dietary and serum folate and vitamin B12 are associated with Hcy levels and DNA damage in female university students. Methods: A cross-sectional study was conducted with female university students from southern Brazil. Folate, vitamin B12, and Hcy levels were determined in their diet or serum. DNA damage levels were assessed by the alkaline comet assay (index and frequency) and the buccal micronucleus assay (micronuclei frequency and binucleated cells frequency). Results: Correlation analyses did not show an association between Hcy levels and dietary or serum folate and vitamin B12 consumption. Dietary folate and vitamin B12 were associated with the index and frequency of damages; however, only serum folate was negatively associated with the index and frequency of damages. Additionally, the frequency of binucleated cells was negatively associated with dietary vitamin B12 and positively associated with serum levels. Serum folate was negatively associated with the frequency of micronuclei. Hcy levels were associated with the index and frequency of damages. Conclusion: These findings strengthen the role of healthier dietary patterns with adequate micronutrients as a preventive strategy to reduce the risk of cardiovascular diseases. This approach should play a pivotal role in shaping health policies and advocating for appropriate food choices.(AU)
Justificativa e Objetivos: Não está claro se o aumento do conhecimento dos estudantes de nutrição está associado a um comportamento alimentar mais saudável, com menores deficiências de micronutrientes que podem induzir danos no DNA. A deficiência de algumas vitaminas pode ser um fator de risco para o aumento dos níveis de homocisteína (Hcy), um marcador de risco cardiovascular. Portanto, este estudo verificou se folato e vitamina B12 dietético e sérico estão associados aos níveis de Hcy e danos no DNA em estudantes universitárias. Métodos: Estudo transversal com universitárias do sul do Brasil. Determinou-se folato, vitamina B12 e Hcy dietético e séricos. Os níveis de danos no DNA foram avaliados pelo ensaio do cometa alcalino (índice e frequência) e pelo ensaio de micronúcleos bucais (frequência de micronúcleos e células binucleadas). Resultados: Análises de correlação não mostraram associação entre os níveis de Hcy com o consumo de folato e vitamina B12 dietético ou sérico. Folato e vitamina B12 dietéticos associou-se ao índice e frequência de danos, entretanto, somente folato sérico associou-se negativamente ao índice e frequência de danos. Ainda, a frequência de células binucleadas estava negativamente associada à vitamina B12 da dieta e positivamente associada aos níveis séricos. Folato sérico associou-se negativamente à frequência de micronúcleos. Os níveis de Hcy associou-se ao índice e frequência de danos. Conclusão: Esses achados fortalecem o papel de padrões alimentares mais saudáveis com micronutrientes adequados como estratégia preventiva visando a redução do risco de doenças cardiovasculares. Esta abordagem deve desempenhar um papel fundamental na formulação de políticas de saúde e na defesa de escolhas alimentares apropriadas.(AU)
Justificación y Objetivos: No está claro si el aumento del conocimiento de estudiantes de nutrición está asociado con un comportamiento alimentario más saludable, con menores deficiencias de micronutrientes que puedan inducir daños en ADN. La deficiencia de algunas vitaminas puede ser un factor de riesgo para el aumento de los niveles de homocisteína (Hcy), marcador de riesgo cardiovascular. Consiguiente, este estudio verificó si folato y vitamina B12 dietéticos y séricos están asociados con niveles de Hcy y daños en el ADN en estudiantes universitarias. Métodos: Estudio transversal con universitarias del sur de Brasil. Se determinaron folato, vitamina B12 y Hcy dietéticos y séricos. The non-invasive prenatal test is a screening method for fetal aneuploidies and if the result is at high risk, it must be confirmed through diagnostic genetic test. It is the most sensitive and specific detection test for common fetal aneuploidies and minimizes the use of invasive techniques, only for pregnant women at high risk. Genetic counseling should be performed before and after screening. This study aims to describe the basic fundamentals of non-invasive prenatal testing by analyzing free circulating deoxyribonucleic acid in maternal plasma for aneuploidy screening, and the primary methods and advances in molecular biology, including next-generation sequencing technologies, which have facilitated it, considering its benefits and limitations when applying it in clinical practice, in this rapidly changing field(AU)

Background: Ursodeoxycholic acid (UDCA), a bile acid, protects the liver through various mechanisms, including bile composition modulation and enhanced secretion. In ALL chemotherapy, 6MP is hepatotoxic, requiring dose reduction. UDCA is used to alleviate liver toxicity in ALL and other chronic cholestatic conditions. The study aims to evaluate the effectiveness of UDCA with chemotherapy in reducing 6MP treatment interruptions and its impact on treatment continuity in pediatric ALL.Methods: This randomized controlled trial study conducted at the Department of Pediatric Hematology and Oncology, BSMMU in pediatric ALL patients during chemotherapy from September 2018 to August 2019. Fifty children aged 1 to 18 years with ALL were enrolled, half receiving UDCA alongside chemotherapy and the rest forming the control group. Serum hepatic transaminases, total bilirubin, and CBC were monitored every 14 days. Statistical analysis was performed using SPSS, with significance set at p<0.05.Results: In this study of 50 pediatric ALL patients, there were no statistically significant age or gender differences between the "Case" (UDCA-treated) and "Control" groups. However, the UDCA group showed a significant decrease in abnormal liver function tests (32.0%) compared to controls (60.0%). Moreover, 6MP dose reduction was significantly lower in cases (4.0%) than controls (40.0%), indicating UDCA's potential hepatoprotective effects. Multivariate logistic regression revealed male gender and mean AST levels as significant factors associated with hepatotoxicity in pediatric ALL patients.Conclusions: Co-administration of UDCA with chemotherapy demonstrates a significant effect in treatment interruption by hepatotoxic drug specially 6 MP in pediatric ALL patients.

Aims: The general objective of this study is to isolate certain strains capable of producing citric acid.Place and Duration of Study: Sample collection was carried out on two types of organic matter. Sampling was done from February to April 2021 in 3 municipalities in Abidjan with 10 samples per site.Methodology: To carry out this work, isolation and purification of Aspergillus sp strains were carried out. Macroscopic and microscopic identifications of the mold isolates were carried out. The search for aflatoxin-producing molds was carried out. The analysis ended with a screening of molds capable of synthesizing citric acid.Results: Aspergillus niger, Aspergillus sp1 and Aspergillus sp2 showed no fluorescence, while Aspergillus flavus, Aspergillus candidus and Aspergillus fumigatus showed fluorescent spots, indicating the presence of aflatoxin.Conclusion: Aspergillus isolates capable of producing citric acid were isolated during this study. Aspergillus niger, Aspergillus sp1 and Aspergillus sp2 do not produce aflatoxin and have the capacity to synthesize citric acid.

Background: Mild Colombian coffees are recognized worldwide for their high-quality coffee cup. However, there have been some failures in post-harvest practices, such as coffee grain fermentation. These failures could occasionally lead to defects and inconsistencies in quality products and economic losses for coffee farmers. In Colombia, one of the fermentation methods most used by coffee growers is wet fermentation, conducted by submerging the de-pulped coffee beans for enough time in water tanks to remove the mucilage. Objectives: We evaluated the effect of the water (g)/de-pulped coffee (g) ratio (I: 0/25, II: 10/25, III: 20/25) and final fermentation time (24, 48, and 72 hours) on the total number of microbial groups. We also identified microorganisms of interest as starter cultures. Methods: We used a completely randomized experimental design with two factors; the effect of the water (g)/de-pulped coffee (g) ratio (I: 0/25, II: 10/25, III: 20/25) and final fermentation time (24, 48, and 72 hours), for 9 treatments with two replicates. During the coffee fermentation (1,950 g), the pH and °Brix were monitored. Total counts of different microbial groups (mesophiles, coliforms, lactic-acid bacteria, acetic-acid bacteria, and yeasts) were performed. Various isolates of microorganisms of interest as starter cultures (lactic-acid bacteria and yeasts) were identified using molecular sequencing techniques. Results: 21 lactic-acid bacteria (LAB) isolates and 22 yeasts were obtained from the different mini-batch fermentation systems. The most abundant lactic-acid bacteria species found were Lactiplantibacillus plantarum (46%) and Levilactobacillus brevis (31%). Pichia kluivery (39%) and Torulaspora delbrueckii (22%) were the most abundant yeast species. Conclusion The studied factors did not have effect over the microorganism's development. The identified bacterial and yeasts species have potential as starter cultures for better-quality coffees and in fermentation-related applications.
Antecedentes: Los cafés suaves lavados colombianos son reconocidos a nivel mundial por su buena puntuación sensorial; sin embargo, se han detectado fallas en las prácticas de postcosecha, como lo es la fermentación de los granos de café. Dichas fallas pueden causar defectos y carecer de consistencia en la calidad del producto, ocasionando pérdidas económicas para los caficultores. En Colombia, uno de los métodos más usados por los caficultores es la fermentación húmeda, la cual consiste en sumergir los granos de café despulpado en tanques con agua por un período de tiempo que permita la remoción del mucílago. Objetivos: La presente investigación evaluó la incidencia que tienen la proporción agua/granos despulpados de café (I: 0/25, II: 10/25, III: 20/25) y el tiempo final de fermentación (24, 48 y 72 horas) en el recuento final de grupos microbianos. Por otra parte, se identificaron taxonómicamente microorganismos de interés para su uso como cultivos iniciadores. Métodos: Mini-lotes consistieron en café despulpado (1950 g) puesto en recipientes de plástico abiertos y sumergidos en agua. Se aplicó un diseño experimental completamente aleatorizado de dos factores (proporción agua/ granos de café despulpado y tiempo) a tres niveles, para un total de nueve tratamientos con dos replicas. Durante las fermentaciones de café (1,950 g), el pH y los grados ºBrix, fueron monitoreados. Se realizaron los recuentos totales de los diferentes grupos microbianos: mesófilos, coliformes, bacterias ácido-lácticas, bacterias ácido-acéticas y levaduras. Se identificaron molecularmente diferentes aislados con potencial para ser usados como cultivos iniciadores (bacterias ácido-lácticas y levaduras). Resultados: Los resultados obtenidos mostraron que no hubo diferencia estádisticamente significativa entre los tratamientos aplicados y el recuento final de microorganismos. Un total de 21 aislados de bacterias ácido-lácticas (BAL) y 22 levaduras lograron obtenerse a partir de los diferentes sistemas de fermentación en mini-lote. Las especies de bacterias ácido-lácticas con mayor porcentaje acorde a su identificación taxonómica, corresponden a Lactiplantibacillus plantarum (46%), Levilactobacillus brevis (31%). Las especies de levaduras con mayor porcentaje acorde a su identificación taxonómica corresponden a Pichia kluivery (39%) y Torulaspora delbrueckii (22%). Conclusión Los factores estudiados no afectaron el crecimiento de ninguno de los grupos microbianos presentes en la fermentacion del café. Las especies de microorganismos identificados tienen potencial para se usados como cultivos starter o en aplicaciones dentro de las ciencias de fermentación.

Background: Potato peel extract has demonstrated the ability to reduce platelet aggregation in vitro, suggesting its potential as a dietary intervention for preventing atherothrombotic disorders. Objective: This study aims to evaluate the impact of a potato peel-rich diet on platelet aggregation. Methods: A randomized, crossover-controlled, open two-period study was carried out with the participation of 12 healthy volunteers. Platelet aggregation was assessed before and after a seven-day dietary intervention. Participants consumed either a diet rich in potato peel (2 g/kg/d) or acetylsalicylic acid (ASA) as a reference (100 mg/d). Platelet aggregation percentages were measured following stimulation with arachidonic acid (AA, 150 µg/mL), adenosine diphosphate (ADP, 10 µM), and collagen (COL, 10 µg/mL). Results: The potato peel-rich diet resulted in a slight but significant reduction in platelet aggregation when stimulated with arachidonic acid compared to baseline values (85.0±2.0% vs. 91.3±1.7%, p<0.05). This effect was less pronounced than the reduction achieved with ASA (16±1.9%, p<0.001). Conclusion: The administration of a diet rich in potato peel reduces platelet aggregation induced by arachidonic acid, suggesting its potential role in the prevention of atherothrombotic disorders.
Introducción: El extracto de cáscara de patata ha demostrado su capacidad para reducir la agregación plaquetaria in vitro, lo que sugiere su potencial como intervención dietética para prevenir trastornos aterotrombóticos. Objetivo: Evaluar el impacto de una dieta rica en cáscara de patata en la agregación plaquetaria. Materiales y métodos: Se llevó a cabo un estudio aleatorizado, controlado, cruzado y abierto con la participación de 12 voluntarios sanos. Se evaluó la agregación plaquetaria antes y después de una intervención dietética de siete días. Los participantes consumieron una dieta rica en cáscara de patata (2 g/kg/d) o ácido acetilsalicílico (ASA) como referente (100 mg/d). Se midieron los porcentajes de agregación plaquetaria después de la estimulación con ácido araquidónico (AA, 150 µg/mL), difosfato de adenosina (ADP, 10 µM) y colágeno (COL, 10 µg/mL). Resultados: La dieta rica en cáscara de patata resultó en una ligera pero significativa reducción en la agregación plaquetaria cuando se estimuló con ácido araquidónico en comparación con los valores iniciales (85,0 ± 2,0% vs. 91,3 ± 1,7%, p <0,05). Este efecto fue menos pronunciado que la reducción lograda con ASA (16 ± 1,9%, p <0,001). Conclusión: La administración de una dieta rica en cáscara de patata reduce la agregación plaquetaria inducida por ácido araquidónico, lo que sugiere su papel potencial en la prevención de trastornos aterotrombóticos.

This rapid synthesis comprised seven studies on inherited macular dystrophy, known as Stargardt disease, which primarily affects children and young adults and is caused by the breakdown of a particular type of light-sensing cell in the eye called ABCA4. One database was selected, and search was carried out using the keywords Stargardt disease and interventional techniques by the application of the appropriate inclusion and exclusion criteria. Most studies were randomized controlled trials, and other systematic reviews and meta-analyses were included, which provided information about the study's methodology and results. The interventional therapies used in this review, including the use of saffron, docosahexaenoic acid, emuxistat hydrochloride, and biofeedback therapy, had a positive effect on Stargardt disease. Therefore, additional studies and syntheses, including controlled trials, case series, adequate sample sizes, and multiple centers, are needed in the future.

Compression of myometrium and occlusion of uterine artery by Dr. Mahesh Gupta (COMOC-MG) technique is an improved B-lynch stitch technique used to control postpartum hemorrhage (PPH). Objective was to study clinical perspective of the COMOC-MG technique on the effect of PPH in Indian women. Data of 10 patients with PPH who were managed by using the COMOC-MG technique in a tertiary care center in Gujarat from April 2022 to December 2022 were retrieved. Baseline characteristics, medical problems and history of women were recorded. Decision to perform COMOC-MG stitch was taken based on the indications such as PPH, per vaginal bleeding and degree of uterine contractility. Out of 10 women, four women had medical problems such as pregnancy-induced hypertension, dysfunctional uterine bleeding and using psychiatric medicines; two women had uterine surgery/ Laparotomy; four women were diagnosed with ailments such as pregnancy induced hypertension (PIH) and hypertension during the third trimester of pregnancy. Indications such as twins during a previous pregnancy, deflexed head, previous lower segment caesarean section (LSCS) with intrauterine growth restriction (IUGR), breech, prim parity, severe oligohydramnios, placental insufficiency and second baby were considered to carry out the CS delivery. Women were managed by COMOC-MG stitch followed by Misoprostol tablets; Carbetocin, Oxytocin, Methylergometrine and Carboprost injections to control PPH. Mean time between uterine closure to COMOC-MG was 10 min. COMOC-MG stitch resulted in good outcomes in seven patients except one patient had atonic PPH. Out of ten patients, one patient required a hysterectomy and three patients required blood transfusion. COMOC-MG stitch is an easy, simple method to control PPH.
