RESUMO

Objective@#To determine the heterogeneity for caries prevention service preferences among children in Anhui Province, so as to provide reference for the promotion and popularization of caries prevention services for school age children.@*Methods@#Based on a discrete selection experiment, a face to face questionnaire survey was administered using a multi stage sampling method among 785 parents with children 3-12 years of age who were hospitalized in the stomatology clinics of 7 prefectures and cities in Anhui Province from October 2021 to October 2022. A mixed Logit model was used to evaluate caries prevention service preferences for children.@*Results@#Four discrete choice experiment attributes included in the study were statistically significant for choice preference ( P <0.05). Compared with the control group, parents with a high school education or above preferred caries prevention services with 70%-<80% preventive effectiveness, 2-<5 and <2 km from the service point, and a high service cost ( β =0.38, 1.66, 1.64, 0.00); female parents preferred preventive services with 70%-<80% preventive effectiveness and a high service cost ( β =0.35, 0.01 ); parents of children <7 years of age preferred services with 70%-<80% preventive effectiveness ( β =0.75); parents of children with oral health preferred preventive services during winter and summer vacations ( β =-0.28); parents of children with caries preferred preventive services with a high cost per denticle ( β =0.00)( P <0.05).@*Conclusions@#Parents with different education levels, gender, child age, and oral health status have heterogeneity in dental caries prevention service preferences. The provision of targeted and precise services can improve the participation and coverage of caries prevention services for school age children.

@#Abstract: Objective To investigate the clinical characteristics and pathogenic genetic mutation of a case with encephalopathy due to defective mitochondrial and peroxisomal fission-1 (EMPF1). Methods The clinical data and genetic test results of a patient with EMPF1 admitted to the Department of Pediatrics, the Affiliated Hospital of Xiangya Medical College of Central South University in August 2020 were retrospectively analyzed. Results An 8-year-old girl, her main clinical features were developmental regression, microcephaly, hypotonia, refractory epilepsy, cranial MRI suggesting brain atrophy and abnormal signals in the right temporal-occipital-parietal cortex, aEEG showing slow wave discharge in the right hemisphere; Whole-exome sequencing of families suggested that the child had a heterozygous missense variant at the c.1040C>G site in the DNM1L gene and the verification results by Sanger sequencing showed that her parents had no variant in this site, which was a novel mutation in accordance with autosomal dominant inheritance; bioinformatics analysis predicted that the mutation was pathogenic. After 2 years of outpatient follow-up, the patient's condition was stable after mitochondrial cocktail therapy and antiepileptic drugs, no epileptic seizure occurred in the past year, mental state and swallowing function improved, and she could be fed orally with occasional nausea and vomiting. Conclusions The main clinical manifestations of EMPF1 are psychomotor developmental delay or regression, dystonia, limb paralysis, epilepsy and so on. According to the clinical phenotype and genetic test results, the rare disease can be diagnosed early.

@#Abstract: Objective To investigate the clinical phenotype and genotype characteristics of mitochondrial encephalomyopathy (ME) families in children. Methods The clinical data and genetic test results of eleven ME families who were admitted to the department of pediatrics of three tertiary hospitals in Hainan Province from January 2007 to December 2021 were retrospectively analyzed. Results A total of 13 cases were diagnosed in eleven ME families, including 6 males (46.15%) and 7 females (53.85%). The age of onset ranged from 6 months to 12 years, the interval from onset to diagnosis was 9 months to 8 years and Morava score was 6-11. Clinical symptoms mainly included abnormal movement, developmental retardation or regression, seizures, stroke-like episodes; among the 13 children, 11 (84.62%) had elevated blood lactic acid and 4 (30.77%) had elevated blood creatine kinase. Cranial MRI mainly involved temporal parietal occipital lobe, cerebellum, brainstem and basal ganglia, some with brain atrophy. Gene detection showed that 8 families (72.72%) were caused by mtDNA mutation, of which 5 families and 6 patients were caused by MT-TL1, m.3243A>G, and 5 asymptomatic carriers of 4 families (80.00%) were detected; MT-ND5, m.13513 G>A was detected in 2 families and 3 patients, and an asymptomatic mutation carrier was detected in a family (50.00%); MT-ND3, m.10191T>C was detected in one family and one patient, and 2 asymptomatic mutation carriers were detected. Three families were caused by nDNA mutations (27.27%). A compound heterozygous mutation of c.751C>T and c.516-2A >G in SURF1 gene was found in one family and one patient, which followed autosomal recessive inheritance. The pathogenic loci were inherited from mother and father, respectively. Two new spontaneous mutations c.1040C>G and c.2060_2062delTAG in DNM1L gene were respectively detected in two families and two patients. All children were given mitochondrial cocktail therapy and symptomatic treatment after diagnosis by genetic testing. Follow-up to June 2022, two families were lost to follow-up and 9 families were followed up regularly; three of the 11 children were still survived. Conclusions For children diagnosed with ME, genetic testing of family members can screen out early asymptomatic pathogenic mutation carriers, achieve early diagnosis of ME and guide clinical genetic counseling. Two new pathogenic sites of DNM1L gene were found in this study, which expanded the genotype spectrum.

Aim To investigate the effect of DERL1 on migration and invasion of human breast cancer cells ZR-75-1 and its potential mechanism. Methods Plasmid of DERL1 overexpression or DERL1 interference was used to explore the function of DERLI in breast cancer cells ZR-75-1. The migration and inva-sion of ZR-75-1 cells were analyzed using wound healing assays and Transwell assays. In addition, the expression of E-cadherin mRNA and protein in ZR-75-1 cells was detected by Real-time quantitative PCR and immunofluorescence. Results After plasmid of DERLI overexpression transfection for 48 h, the expression of DERLI in ZR-75-1 cells significantly in-creased compared with that of control group ( P < 0.01) , and the migration and invasion of cells were significantly up-regulated ( P < 0. 05 ) , cell morphology was transformed from cobblestone-like epithelial to spindle-shaped mesenchymal, and the expression of E-cadherin mRNA and protein significantly decreased (P <0.05). After plasmid of DERLI interference transfection for 48 h, compared with control group, the ex-pression of DERLI in ZR-75-1 cells was significantly down-regulated ( P < 0. 01 ) , DERLI knockdown inhibited the migration and invasion of ZR-75-1 cells (P < 0.05 ) , cell morphology changed from spindle-shaped mesenchymal to cobblestone-like epithelial, and the expression of E-cadherin mRNA and protein was significantly up-regulated ( P < 0. 05 ). Conclusions DERLI can promote the migration and invasion of human breast cancer cells ZR-75-1, which may be related to its inhibition of E-cadherin expression to promotion of epithelial mesenchymal transition.

Background and purpose: Cervical conization is a common operation to treat precancerous tissues performed under non-intubated anesthesia. As common opioid analgesics have side effects of inhibiting respiration and circulation, other kinds of analgesic drugs should be coordinated to improve the anesthetic effect, without interfering the respiration and circulation. This study aimed to evaluate the effects of dezocine or flurbiprofen combined with propofolremifentanil in cervical precancerosis conization. Methods: Sixty patients who underwent cervical conization were equally randomized into dezocine group (group D), flurbiprofen group (group F) and 0.9% natural saline (group N) with 20 patients in each group, and received dezocine 0.1 mg/kg, flurbiprofen 1 mg/kg or 0.9% natural saline in 5 mL respectively before anesthesia induction. During the anesthesia induction, the targeted control infusion of remifentanil in effect concentration was set at 1.5 ng/mL, and the plasma concentration of propofol was set at 2 μg/mL. Heart rate (HR), respiratory rate (RR), surplus pulse O2 (SPO2) and mean arterial pressure (MAP), MAP were monitored before the anesthesia induction (T0) and after (T1), at the start of cervical conization (T2), and at the end of operation (T3). The incidence of respiratory depression and body movements during surgery were observed. The satisfaction degree of the surgeon to the opening status of cervix was evaluated. The post-operative recovery time, visual analogue scale (VAS) scores, nausea and vomiting in the following 12 hours were also recorded. Results: The HR, RR, SPO2 and MAP in three groups did not have any significant change (P>0.05) at T0, T1 and T3. At T2 the HR and MAP decreased significantly in group D and group F compared with group N (P<0.05), and there was no significant difference between group D and group F (P>0.05). The surgical satisfaction degree of "Good" in group D was 80%, significantly higher than that in group N (30%) and group F (50%), indicating a better cervix opening in group D. The recovery time in three groups had no significant difference, and the VAS scores in group D and group F were lower than those in group N (P<0.05) after operation, and patients did not have nausea or vomiting in the following 12 hours. Conclusion: Both the dezocine and flurbiprofen could improve the anesthetic effect in cervical conization and post-operative comfort, with less respiratory or circulation depression. Dezocine showed better improvement than flurbiprofen in cervix opening and the inhibition of stress response and body movements during surgery.

Background and purpose:Postoperative sore throat (POST) is one of the common complaints of patients after radical thyroidectomy. Tracheal intubation is the main cause of POST. This study compared the effect of intubation with visual endoscopy and general laryngoscope on POST in patients undergoing radical thyroidectomy. Methods:One hundred patients (18-60 years, ASAⅠ-Ⅱ) undergoing elective radical thyroidectomy were randomized into two groups:patients in group A (n=50) were intubated with visual endoscope while patients in group B (n=50) were intubated with general laryngoscope. Endotracheal tube cuffs pressure was maintained at 20mmHg in all patients. Visual analogue scale (VAS) and Bruggrmann comfort scale (BCS) were recorded at the time points of 1, 6 and 24 h after extubation. Results:Compared with group B, the incidence of POST in group A was signifcantly reduced (42%vs 64%, P=0.027). The VAS of group A was lower than that of group B (3.05±1.56 vs 4.25±1.30, 3.05±1.56 vs 4.01±1.98, 2.72±1.77 vs 3.31±1.12) (P<0.05). The BCS of group A was higher than that of group B (0.99±0.46 vs 0.69±0.30, 1.95±0.47 vs 1.51±0.58, 2.82±0.87 vs 2.31±0.72) (P<0.05). Conclusion:Using visual endoscopic intubation can reduce the incidence of the POST in patients undergoing radical thyroidectomy.

RESUMO

A measurement system based on the image processing technology and developed by LabVIEW was designed to quickly obtain the range of motion (ROM) of spine. NI-Vision module was used to pre-process the original images and calculate the angles of marked needles in order to get ROM data. Six human cadaveric thoracic spine segments T7-T10 were selected to carry out 6 kinds of loads, including left/right lateral bending, flexion, extension, cis/counterclockwise torsion. The system was used to measure the ROM of segment T8-T9 under the loads from 1 Nm to 5 Nm. The experimental results showed that the system is able to measure the ROM of the spine accurately and quickly, which provides a simple and reliable tool for spine biomechanics investigators.

Objective To investigate the effect of reactive oxygen species(ROS) on P2X3 receptor-mediated neuropathic pain. Methods Neuropathic pain model was induced in female Sprague Dawley rats by chronic compression of dorsal root ganglia (CCD), and the CCD rats were randomly divided into 4 groups (each group containing 10 rats): Saline group (NS group); PBN 100 mg/kg treatment group; PBN 30 mg/kg treatment group; and PBN 10 mg/kg treatment group. The specific agonist of P2X3 receptor, α, β-meATP (50 nmol in 50 μL), was subcutaneously injected into the plantar surface of the right hind paws of each rat 30 minafter PBN or NS injection. The spontaneous paw flinching times and withdrawing time were observed for 15 min after injection and the paw lift time per minute (PLTPM) in every 2 minutes was calculated. Results Pre- treatmentwith PBN inhibited α, β-meATP-induced spontaneous pain in a dose-dependent manner. Compared with the NS group, PBN 100 mg/kg group significantly inhibited flinching response during the first 6 min (P<0. 05), while the rats in PBN 30 mg/kg group only had significantly attenuated flinching response during the second to the fourth minute compared with NS group(P<0. 05). Conclusion Oxygen free radical scavenger can effectively alleviate the neuropathic pain caused by CCD and P2X3 receptor agonist-induced spontaneous pain. ROS may act as a messenger in P2X3 receptor-mediated pain signaling transmission.

Objective To develop an automatic segmentation and mesh generation technique for abdominal aortic aneurysm (AAA) and to build a 2D numerical analysis model that can be used for finite element analysis (FEA). Methods A method that totally based on morphology processing was developed to segment all the components of the AAA. For each closed curve that obtained, its signed distance function was then calculated. According to the set relationships between each curve, the final signed distance function was calculated. Under the control of this function and an equilibrium relationship, iterated Delaunay algorithms were used until the equilibrium relationship was satisfied or the set conditions were reached. Then the program ended and the finite element model was generated. Results Automatic segmentation of the lumen as well as semiautomatic segmentation of the wall and calcification were achieved. Different parts of the AAA were meshed, and the type and density of the mesh could be controlled. Two finite element models were established for stress analysis: one was the coupling mesh of the thrombus and the wall, and the other was the coupling mesh of the thrombus, the wall and the calcifications. Conclusions An automatic segmentation and mesh generation algorithm with high accuracy has been developed, without any complicated computation or initial curve. The mesh generation algorithm tends to produce high quality meshes and the generation is easy to be controlled by only two parameters. The generated mesh has been verified to be useful in FEA simulation.

The study was aimed to investigate the optimum conditions of freeze drying preservation of amniotic membrane (AM). The AM from the health puerperal woman was preserved by freeze drying at optimized way. The key factors of freeze drying process, including abstersion aqua, conservation liquor, the curve of freezing temperature, and the ingredient of protective agent, were optimized. All their morphologic structure was observed by light microscope and scanning electron microscope. The degradation rates by collagenase IV and the characterization of biomechanics were analyzed. The radio-immunologic method was used to investigate the cytokines quantity. All properties of freeze dried AM were compared with those of fresh AM. Light micrographs showed that the five structure-layers exist both in the fresh AM and in those preserved by freeze drying, while the fibro-material was tight-structured in the fresh AM, but loose slightly; the thickness of fibro-material was larger slightly in freeze dried AM. Scanning electron micrographs show that the micro-hairs of epithelial cells in fresh AM were decreased slightly in optimized drying AM, the collagen fibre of fresh AM and of optimized drying AM were well in morphological structures and arranged tightly. The degradation rate by collagenase IV was faster in optimized drying AM,compared with that of the fresh AM. There were insignificant diversity in biomechanical characters (tensile strength, elongation at break and elastic modulus) of the optimized drying AM compared with fresh AM. The cytokines quantity in optimized drying AM decreased significantly compared with fresh AM. The improved freeze drying process has better advantage in keeping the morphological structure, preferable biomechanics and biological vitality of AM, compared with the early research.

RESUMO

<p><b>OBJECTIVE</b>To compare the therapeutic effect difference between electroacupuncture combined with rehabilitation techniques and simple rehabilitation techniques for treatment of the shoulder subluxation after stroke.</p><p><b>METHODS</b>Sixty cases of shoulder subluxation patients were divided into an acupuncture combined with rehabilitation group (30 cases) and a rehabilitation group (30 cases). The acupuncture combined with rehabilitation group was treated with electroacupuncture at Jianyu (LI 15), Jianliao (TE 14) and Jianzhen (SI 9) etc. combined with rehabilitation treatment, the rehabilitation group was treated with rehabilitation only. The therapeutic effects were evaluated and the pain score of the shoulder joint, passive range of motion of shoulder joint, the muscle strength of middle group of the deltoid and the function grade of the upper limbs motion of the two groups were evaluated before and after treatment.</p><p><b>RESULTS</b>The acupuncture combined with rehabilitation group was superior to that of the rehabilitation group in pain score of the shoulder joint, passive range of motion of shoulder joint, the muscle strength of middle group of the deltoid and the function grade of the upper limbs motion with significant differences between the two groups (all P<0.05); the total effective rate of 93. 3% in the acupuncture combined with rehabilitation group was superior to that of 73.3% in the rehabilitation group (P<0.05).</p><p><b>CONCLUSION</b>The therapy of electroacupuncture combined with rehabilitation techniques can act a good regulating role on muscle strength and the muscular tension of shoulder joint and the muscles around the scapula through many steps, and can effectively improve the shoulder subluxation.</p>

<p><b>OBJECTIVE</b>To investigate approach and possibility of transferring basic fibroblast growth factor (bFGF) gene into rabbit bone marrow stromal cells (BMSCs).</p><p><b>METHODS</b>The eukaryotic expression vectors harboring bFGF cDNA were constructed and transfected into rabbit BMSCs mediated by liposome. The transcription and expression of bFGF gene in the transfected BMSCs were detected by means of morphological observation, immunohistochemistry, enzyme-linked immunosorbent assay (ELISA) and RT-PCR. The changes in the biological characteristics of the transfected MSCs were also observed.</p><p><b>RESULTS</b>Stable overexpression of bFGF protein was detected in the transfected BMSCs, which showed differentiation towards chondrocyte lineage.</p><p><b>CONCLUSION</b>Stable expression of bFGF gene in transfected BMSCs can induce cell differentiation into chondrocyte lineage.</p>

<p><b>OBJECTIVE</b>To evaluate the biocompatibility of polylactic-co-glycolic acid (PLGA) for culturing bFGF gene-transfected bone marrow stromal cells (BMSCs) and assess the feasibility of this cell complex for repairing cartilage defect in rabbits using tissue engineering method.</p><p><b>METHODS</b>BMSCs transfected by bFGF gene were cultured on PLGA matrix to assess the biocompatibility of PLGA. The cell complex was then implanted into the cartilage defect in rabbits, and its effect in cartilage defect repair was evaluated by histological observation and immunohistochemical staining.</p><p><b>RESULTS</b>BMSCs transfected by bFGF gene grew normally on PLGA matrix. After implantation, the complex showed good effect for cartilage defect repair in rabbits.</p><p><b>CONCLUSION</b>PLGA has good biocompatibility with the transfected BMSCs, and the cell complex can be used for repairing rabbit cartilage defect and may potentially serve as a substitute of cartilage autograft.</p>

<p><b>OBJECTIVE</b>To summarize 4 schools of thought in studies of meridians.</p><p><b>METHODS</b>Consult literature and review the history of studies of meridians.</p><p><b>RESULTS</b>The studies of meridians in the past dozens years at home and abroad have gradually formed four main theories, namely the nerve-conduction meridian theory of neurophysiology, body fluid circulation meridian theory of the physio-biochemistry, bio-field meridian theory of biophysics, and the connective tissue structure meridian theory of the overall-gap viewpoint, which are summarized and analyzed.</p>

This study was aimed to explore a new method of alleviating graft-versus-host disease (GVHD) after allogeneic bone marrow transplantation through selective elimination of human alloreactive T cells expressing either CD25(+) or CD69(+) by immuno-magnetic cell sorting (MACS). Healthy donor peripheral blood mononuclear cells were cocultivated with bone marrow mononuclear cells from HLA-nonidentical leukemia recipient with remission in one-way mixed lymphocyte culture (OWMLC). After 3 days, both CD25(+) and CD69(+) lymphocytes were removed by MACS. The depleted donor fraction and untreated donor cells were then rechallenged in a secondary mixed lymphocyte culture (MLC) with the original stimulator cells or a third party to assess relative alloreactivity. The results showed that 50% inhibition of the secondary MLC was observed in the depleted donor fraction. Alloreactivity against unrelated third-party cells was largely preserved. It is concluded that this method reduces alloreactivity while retaining reactivity against a third party target in vitro.
